Abstract

Kanker prostat merupakan kanker yang lebih banyak ditemukan di negara Barat dibandingkan Asia. Berbagai faktor prognostik telah diteliti untuk memprediksi angka kesintasan pasien yang diterapi hormonal. Pada penelitian ini dievaluasi peranan usia, PSA, jumlah lesi metastasis, skor Karnofsky, hemoglobin, dan kreatinin sebagai faktor prognostik untuk menilai angka kesintasan pada kanker prostat metastasis di Indonesia yang dilakukan kastrasi dengan orkhidektomi. Penelitian dilakukan secara retrospektif dengan mengumpulkan data rekam medik penderita kanker prostat dengan metastasis yang dilakukan orkhidektomi subkapsular dan belum mengalami hormone resistance prostate cancer (HRPC) di Klinik Khusus Urologi RSCM dan RS Kanker “Dharmais” Jakarta, periode Januari 1995- Desember 2008 dengan follow-up 24 bulan. Selama periode penelitian, terdapat 194 pasien yang memenuhi kriteria dan 99 pasien di antaranya memiliki data lengkap untuk dianalisis. Dari analisis multivariat didapat kekuatan hubungan dari yang terbesar sampai yang terkecil, yaitu jumlah lesi (HR=8,56), kreatinin (HR=3,24), hemoglobin (HR=0,94), dan skor Karnofsky (HR=0,28). Disimpulkan bahwa jumlah lesi dan kreatinin secara statistik signifikan mempengaruhi kesintasan.Kata kunci: jumlah lesi, kanker prostat, kesintasan, kreatinin, metastasis

Abstract

Objective: To assess the contribution of internal iliac artery ligation to bleeding control during surgery. Methods: This retrospective study used secondary data from medical records. All patients diagnosed with PASD from January 2019 – to December 2022 were included in this study. Participants were grouped based on operation technique, and the blood loss and operative duration were evaluated. The tests used were the Kruskal-Wallis and the Mann-Whitney U tests. Results: 108 PAS patients were discovered. The most age group was between 20-35 years with parity of more than or equal to 4, history of Cesarean section once, gestational age at termination 34-36 weeks, and maternal death in 7 out of 101 cases. There were 49 resections, 13 resections with internal iliac artery ligation, 34 hysterectomies, and 12 hysterectomies with internal iliac artery ligation. There was no difference in bleeding and operative duration between resection vs. resection with internal iliac artery ligation (p: 0.113; p: 0.639), hysterectomy vs. a hysterectomy with internal iliac artery ligation ((p:0.052; P:0.723), and resection with ligation vs hystetectomy with the internal iliac artery ligation (p:0.052; p:0.723). Bleeding and operative duration differed significantly between resection vs. hysterectomy (p:0.002; p:0.013). All patients underwent tourniquet placement. Conclusion An Internal iliac artery ligation was not shown to reduce bleeding in treating PASD. Keywords: accreta, internal iliac artery, placenta .

Abstract

Objective: In this report, we aim to present a case of a prepubertal child with left varicocele who underwent successful microsurgical ligation. Case(s) Presentation: A 9-year-old boy was admitted to urology clinic due to his parents complaint about a mass in his left scrotum. We diagnosed this patient with grade III varicocele (Dubin-Amelar classification). The patient underwent microsurgical ligation surgery. After surgery, the mass such as "bag of worms" disappeared and the testicular volume increased by 53% from preoperative volume (1.8 to 2.3 cc). Discussion: The results of this case report indicate that microsurgical ligation of varicocele in children is a safe and effective therapeutic option. Conclusion: Microsurgical ligation of varicocele in pre-pubertal children represents a safe and effective therapeutic option, offering symptom relief and potential preservation of future fertility. Further studies are warranted to elucidate the long-term outcomes and reproductive implications of microsurgical varicocelectomy in this population.
