Abstract

ABSTRACTPalliative cancer patients undergo severe pain, and pharmacological therapy in some cases cannot fully relieve pain. Self-selected Individual Music Therapy (SeLIMuT) is non-pharmacological relaxation stimulating complementary therapy which is safe, accessible, inexpensive and effective. The study aimed to identify effect of SeLIMuT to pain in palliative cancer patients. The study was a quasi experiment- pre-test and post-test design using comparison group with purposive and consecutive sampling carried out at inpatient ward I of Dr Sardjito Hospital Yogyakarta. Respondentswere divided into intervention group (n=23) with SeLIMuT therapy four times each within 15-20 minutes and control group (n=23) without therapy. Pain was assessed in both groups using Visual Analog Scale (VAS). The result of the study showed that there was significant difference in average pre-post in both groups with score of p=0.001 (p<0.05). Pain decrease occurred in SeLIMuT group after intervention with score of mean 2.144 (0.91). Pain decrease in SeLIMuT group was also clinically significant (mean≥1.0). Increase in pain level occurred in the control group with score of mean-0.03 (0.15). SeLIMuT intervention both statistically and clinically affected pain level in palliative cancer patients. Penelitian ini bertujuan untuk mengetahui pengaruh SeLIMuT terhadap tingkat nyeri pasien kanker paliatif. Penurunan nyeri terjadi pada kelompok SeLIMuT setelah mendapatkan intervensi dengan nilai mean (SD) 2,144 (0,91). Penurunan nyeri pada kelompok SeLIMuT juga bermakna secara klinis (mean ≥ 1,0). Peningkatan skor nyeri terdapat pada kelompok kontrol dengan nilai mean (SD) -0,03 (0,15). Dapat disimpulkan bahwa secara statistik dan klinis, intervensi SeLIMuT berpengaruh terhadap tingkat nyeri pasien kanker paliatif.Pengaruh tersebut berupa efektivitas SeLIMuT dalam menurunkan nyeri.

Abstract

Abstract : The unstabled blood pressure and pulse rates of palliative care cancer patients were due to the psychological stressed, pain, and anxiety. Complementary And Alternative Medicine (CAM) have been apply to reduce the commons symptoms of advanced illness.The aim of this study was to explain the effect of SeLIMuT to hemodynamic palliative care cancer patients in dr. Sardjito Yogyakarta hospital. A quasi experiment pre-posttest with control group designed apply to this study. A purposive sampling technique carried out in IRNA I RSUP Dr. Sardjito Yogyakarta which were divided into two groups: the intervention and control group. The intervention group (n=23) received 4 times SeLIMuT therapy for 2 days with each session about 15 to 20 minutes, and controlled group (n=23) did not receive any therapies. Pre and post test measured of pulse rates and blood pressure. It was clinically significant if there was cut off point was a ≥10 mmHg on both systolic and diastolic level and >4 x/minute on the pulse rates. Statistically,a significant mean values difference of systolic (p=0,001), diastolic (p=0.024) dan pulse rates (p=0,001), as significanceas p<0,05. In addition, a decrease of 3,70 mmHg on systole level, 2,18 mmHg on diastole level, dan 0,52 on pulse rate, however these values had no clinically significance based on both of cut-off point. The control group systolic level hadn’t changed, which is the diastole level and pulse rates have been increased. Statistically, SeLIMuT has a positive effect on the hemodynamic status of patients with cancer taking palliative care, although there was not significance clinically.Keywords : Palliative cancer, Hemodynamic, SeLIMuT, Music TherapyAbstrak : Ketidakstabilan tekanan darah dan denyut nadi pada pasien kanker paliatif dilaporkan lebih besar terjadi sebagai akibat reaksi psikologis nyeri dan kecemasan yang dialami. Complementary and Alternative Medicine (CAM) menjadi tren pilihan terapi untuk mengurangi gejala yang mengganggu pasien kanker paliatif. Penelitian ini bertujuan untuk mengetahui pengaruh SeLIMuT terhadap perubahan status hemodinamik pasien kanker paliatif. Quasi Experiment pre-test and post-test design with Comparison Group dengan purposive sampling yang dilakukan di IRNA I RSUP Dr. Sardjito Yogyakarta. Responden dibagi dalam kelompok intervensi (n=23) menerima terapi SeLIMuT selama 4 kali dalam dua hari dengan durasi tiap terapi 15-20 menit dan kelompok kontrol (n=23) tidak diberikan terapi. Kedua kelompok dilakukan pengukuran tekanan darah dan denyut nadi pre dan post. Dikatakan bermakna secara klinis jika perubahan sebesar ≥10 mmHg pada sistolik dan diastolik dan >4 x/menit pada nadi. Hasil penelitian menunjukan bahwa terdapat perbedaan yang signifikan rerata sistolik (p=0,001), diastolik (p=0,024) dan denyut nadi (p=0,001), signifikan jika p<0,05. Terjadi penurunan sebesar 3,70 mmHg pada sistolik, 2,18 mmHg, dan 0,52 pada nadi, namun penurunan tersebut tidak bermakna secara klinis. Perbedaan nilai sistolik pre-post pada kelompok kontrol cenderung tetap, sedangkan pada diastolik dan nadi justru menunjukkan peningkatan. Secara statistik SeLIMuT berpengaruh positif terhadap status hemodinamik pasien kanker paliatif, namun hasil tersebut tidak bermakna secara klinis.Kata Kunci : Kanker paliatif, hemodinamik, SeLIMuT, terapi musik

Abstract

Background: Nurses are supposed to be able to show empathy to patients. Empathy is capability to feel the emotion, thought, and condition of the patient without involving self emotion. Capability of showing empathy in nurses is builtsince the period of education. Every stage in nursing education at nursing study program has different learning activities and load of materials. The difference may be affected by capability of showing empathy in students. The objective of this study is to identify difference in empathy capability of nursing students of Faculty of Medicine UGMMethod: This was a quantitative study with cross sectional design. Samples were taken through purposive proportional sampling involving 88 nursing students at Faculty of Medicine Universitas Gadjah Mada undergoing academic education (batch 2009 to 2012) and clinical education (batch 2007 and 2008). Empathy capability was assessed using Jefferson Scale of Physician Empathy – Nursing Student Version R (JSPE-R).Results: Average score of empathy capability was 113.26 with standard deviation 10.45. There was difference in empathy capability of students of batch 2007 to 2012 (p=0.014). Average score of empathy capability of the students decreased along with the duration of education. There was no difference in empathy capability based on gender (p=0.516), age (p=0.668), ethnic group (p=0.167), and stage of education (p=0.541). There was difference in score of standing in the patient shoes between academic and clinical students (p=0.022).Conclusion: There is difference in empathy capability in nursing students from different batch at Faculty of Medicine UGM.

Abstract

Background: Nurses have a key role in promoting patients’ involvement in decision-making process of palliative care to improve their dignity and satisfaction. However, there is a dearth of studies exploring this involvement, especially in public health centers in Indonesia. Objective: This study aimed to explore the perception of nurses in decision-making process and to understand what type of decision-making made by health care providers in palliative care. Methods: This was a descriptive explorative study with qualitative approach. Participants were selected using purposive and snowball sampling. Data were collected using Focus Group Discussion (FGD) and in-depth interview. Content analysis method by inductive approach was used for data analysis. Consolidated criteria for reporting qualitative research (COREQ) was also used. Results: Eight themes emerged in this study, namely: (1) Collecting information about patients’ current physiological condition, (2) Creating alternative strategies according to the patients’ current physiological condition, (3) Establishing implementation type to be performed, (4) Providing information to the patients, (5) Discussing decision-making between patients and family, (6) Choosing/rejecting an action by patients, (7) Performing selected actions, and (8) Evaluating action. Three points that are different from the existing theory were: (i) there was a discussion between patients and family, (ii) the absence of the process of collecting information after a patient rejects to act, and (iii) there were three types of decision-making: paternalistic, shared, and informed decision-making. Conclusion: This study serves as an input for nurses to pay more attention in decision-making process in palliative care in patients with cancer, and to encourage patients to give contribution in decision-making as part of shared decision-making.

Abstract

Family caregivers, especially in Asian countries, have a profound role in caring for a sick family member. However, there are wide variations between the Asian and western world in terms of culture and facilities. Therefore, the problems and needs of family caregivers between those two regions may also be distinct, and it is important to explore and elaborate based on our empirical evidence. In Indonesia, motives and values in caregiving and religion become the wheel-power of the family caregivers in providing care. This affects action and consequences for caregivers. This paper attempts to elaborate on common ethical dilemmas that usually face by family caregivers in Indonesia. Unfortunately, family caregivers typically are not prepared to make those challenging decisions. Therefore, we recommend not only that family caregivers need to be involved in the caring process, but also their issues and ethical dilemmas should be assessed and addressed by health care professionals, especially nurses, who have the most frequent contact with patient and family caregiver.

Abstract

Patients with Coronavirus Disease 2019 (COVID-19), particularly those with a severe condition, might not survive. Pandemic situation challenges the healthcare providers in addressing palliative care to the patients. This paper aimed to describe the importance of providing palliative care for patients with severe COVID-19 in Indonesia. We used a case scenario to illustrate the common condition experienced by a patient with severe COVID-19. Health care providers in Indonesia could address palliative care for patients with COVID-19 by focusing on controlling the symptoms, avoiding futile intervention, and connecting the patients and their families. Nurses need to consider the patients’ needs for family supports, even though not in physical or psychosocial support, and help the patients who need end-of-life care to be dying with dignity. Communication technology must be utilized optimally by healthcare providers to realize the communication among the patients, families, and health workers.

Abstract

This study aims to determine the effectiveness of the use of the Mentoring system in helping nurses to improve the quality of nurses in hospitals. The method used is analytical survey research using a cross sectional approach. The results of the study showed that the effectiveness of the mentoring system (82.5%), satisfaction with the mentoring system (76.6%), the usefulness in improving the Competence of Nurses (77.5%), and the technology and accessibility of the mentoring system (70%). It was concluded that the Mentoring System was more than 76% effective in improving the quality of nurses in hospitals. Keywords: Nurse Quality, Mentoring, Mentor
