Abstract

a complete medical record reflects the quality of good documentation, the complete of diagnosis writing reduces incorrect and inaccurate code and appropriate reporting to support decision makers. this study aims to measure the completeness and accuracy of the death code and the basic cause of death of patients. This study uses a quantitative descriptive design using a completeness checklist instrument. researchers used secondary data from the medical records of patients who died in 2021, the total sample was 199 using the slovin formula. secondary data analyzed with STATA, presented with tabulations and descriptive narratives. the basic cause of the basic cause of death was highest in code J80 at 33 (17%), J12.8 at 21 (11%) and the third being E11.9 . Based on the category of completeness of filling out of the 199 medical records analyzed, there were 37% (75) death forms that were not filled in completely

Abstract

Abstract

The quality of blood products, particularly Packed Red Cells (PRC), is a crucial factor in ensuring safe and effective blood transfusions. Donor conditions are thought to influence the quality of produced blood components, including factors such as Body Mass Index (BMI), hemoglobin (Hb), and hematocrit levels. This study aimed to analyze the relationship between donor BMI, hemoglobin, and hematocrit levels with the quality of PRC products, which include PRC volume, hemoglobin content, and hematocrit. This was an analytical observational study with a cross-sectional design, involving 30 blood donors at the Blood Transfusion Unit of the Indonesian Red Cross (PMI) in Gunungkidul. Donor BMI was calculated from weight and height measurements, while hemoglobin and hematocrit levels were examined from capillary blood samples. The volume, hemoglobin, hematocrit, and physical appearance of PRC were evaluated according to standard procedures and analyzed using the Pearson correlation test at a 95% confidence level (α = 0.05). The results showed that the donors had an average BMI of 26.58 kg/m², hemoglobin 14.62 g/dL, and hematocrit 43.17%. Meanwhile, the PRC units had an average volume of 200.76 mL, hemoglobin content of 37.38 g/unit, and hematocrit of 54.33%, with all plasma samples appearing clear. Statistical analysis revealed no significant correlation between donor BMI, hemoglobin, and hematocrit with PRC volume (p>0.05), PRC hemoglobin (p>0.05), or PRC hematocrit (p>0.05). These findings indicate that variations in donor BMI and baseline hematological parameters do not affect the quality of PRC products. This study concludes that donor BMI, hemoglobin, and hematocrit are not associated with PRC quality. Future studies are recommended to include additional parameters, such as triglyceride levels, to provide a more comprehensive understanding of donor factors affecting blood product quality.

Abstract

Kata Kunci: Manajemen Informasi Asuhan Keperawatan; Akurasi Diagnosis; Cedera Luar ABSTRACTSituation Analysis: Disease coding, particularly for external causes in emergency patient medical records, is essential but its accuracy is often challenged due to suboptimal completeness of nursing care documentation. This issue necessitates increasing the information management competency of Professional Nursing students, as they are future healthcare personnel. Objective: This community service program aimed to enhance the understanding of UNJAYA Professional Nursing students regarding the importance of nursing care information management in supporting the accuracy of external injury diagnoses. Implementation Method: The activity was carried out through training and counseling based on the transfer of knowledge and technology. Target Partner and Participants: The partner for this activity was 51 Professional Nursing students at UNJAYA. Evaluation of understanding improvement was conducted using pre-test and post-test methods. Activity Results: The program achieved a significant increase in understanding. The class-average accuracy improved from 91% to 100%, accompanied by a decrease in the average completion time from 2 minutes 48 seconds to 1 minute 21 seconds. The outputs of this activity are the publication of a scientific article in an accredited journal and a measurable increase in student competence in clinical information management. Keywords: Nursing Information Management; Diagnosis Accuracy; External Injury; Professional Nursing Students
