Abstract

Idiopatik Trombositopeni Purpura (ITP) adalah kelainan yang berhubungan dengan penurunan jumlah platelet yang beredar dalam plasma darah yang dapat disebabkan oleh peningkatan destruksi platelet karena autoimun. Penurunan jumlah platelet akan menurunkan kemampuan hemostasis tubuh. Prevalensi ITP adalah 4 sampai 5,3 per 100.000 anak, dengan tingkat mortalitas ITP kronis sekitar 4%. Abses submandibula menempati urutan pertama abses leher yang paling sering dijumpai (42,30%) dengan prevalensi causa odontogenik sebesar 34,21%. Tujuan laporan kasus ini adalah untuk mempresentasikan keberhasilan evakuasi pus dan eliminasi gigi kausa pada kasus abses submandibula odontogenik pada seorang anak penderita ITP. Seorang anak perempuan berusia14 tahun penderita ITP dengan riwayat sakit gigi geraham kanan bawah dan pembengkakan pada submandibula kanan dengan fistula ekstra oral yang mengeluarkan darah dan pus datang ke IGD RSUP Dr. Sardjito dengan kondisi lemah. Kasus ini dirawat bersama dengan bagian hematologi onkologi anak untuk penanganan kondisi trombositopenia sehingga memungkinkan untuk dilakukan tindakan incisi drainase dan pencabutan gigi kausa. Tindakan perawatan gigi dan pembedahan dapat dilakukan pada penderita ITP dengan memperhatikan angka trombosit. Untuk mencapai angka trombosit yang cukup, diperlukan kerja sama dengan dokter bagian hematologi, sehingga resiko perdarahan durante dan pasca tindakan dapat di minimalisirABSTRACT: Odontogenic submandibular abscess in patient with Idiopathic Trombocytophenia Purpura at General Hospital Centre Dr. Sardjito. Idiopathic Thrombocytopenia Purpura (ITP) is a disorder associated with a number decreasing of platelets circulating in the blood which can be caused by platelet destruction increasing due to autoimmune. Low platelet count will decrease the body's ability to hemostasis. The prevalence of ITP is 4 to 5.3 per 100,000 children, with a mortality rate of approximately 4% of chronic ITP. Submandibular abscess is the first ranks of neck abscesses which are the most common (42.30%) with a prevalence of 34.21% odontogenic cause. The purpose of this case report is to present the early success of the evacuation of pus and elimination of causative tooth in the case of odontogenic submandibular abscess in a child with ITP. A 14 year old girl diagnosed with ITP and a history of right lower molar tooth pain and swelling in the right submandibular with extra-oral fistula that blood and pus came to the ER department of DR.Sardjito feebly. This case was treated together with pediatric department of hematology oncology for thrombocytopenia conditions allowing for incision drainage and extraction of causes tooth. Dental treatment and surgery can be performed on patients with ITP with regard platelet numbers. To achieve a sufficient number of platelets, it is neccessary to cooperate with hematologist, so the risk of bleeding during and following the surgery can be minimized.

Abstract

The accuracy of biopsy on oral malignant lesion examination depends on the adequacy of clinical information obtained, biopsy type and procedure, determination of biopsy site, and integrity of biopsy tissue. Fine needle aspiration biopsy (FNAB) is one of among cytology examinationthat can be used for oral lesion. However, the results are variative, either false positive or false negative. The adequate management of a malignant lesion started with an accurate diagnosis. Histopathology and tissue biopsy examination is a gold standard in diagnosis determination of suspected malignant lesion. The aim of this case study was to report a refferal case of misdiagnosed lower lip squamous cell carcinoma using FNAB examination. Patient reffered from local hospital with squamous cell carcinoma lesion misdiagnosed as a benign cystic lesion prone to mucocele through FNAB finding. From this case study, it was concluded that FNAB examination is not reliable to diagnose superficial lesions, such as cystic lesions and SCC in the lower lip. ABSTRAKKetepatan diagnosis dari pemeriksaan lesi ganas rongga mulut tergantung pada adekuatnya informasi klinis yang didapatkan, jenis dan prosedur biopsi yang tepat, pemilihan tempat biopsi dan pemenuhan syarat dari jaringan biopsi. Aspirasi jarum halus (AJH) merupakan salah satu pemeriksaan sitologi yang bisa dilakukan untuk lesi di mulut, namun hasil pemeriksaan AJH masih ada yang dilaporkan false negative maupun false positive dengan hasil yang bervariasi.Penanganan yang tepat pada lesi ganas dimulai dengan melakukan diagnosis yang akurat. Standar baku dalam mendiagnosa terletak pada pemeriksaan histopatologi dari biopsi jaringan yang dicurigai mengalami lesi ganas. Tujuan studi kasus ini adalah melaporkan suatu kasus rujukan kesalahan diagnosis squamous cell carcinoma bibir bawah dengan pemeriksaan AJH. Seorang pasien rujukan dari salah satu RSUD dengan lesi squamous cell carcinoma yangdidiagnosis benigna cystic lesion yang condong ke mucocele melalui pemeriksaan AJH. Kesimpulan studi kasus ini adalah pemeriksaan AJH tidak tepat digunakan untuk kasus lesi superfisial seperti lesi kistik dan SCC di bibir bawah.

Abstract

Autotransplantation of molar is a potential treatment option to restore perfect occlusion and to improve mastication following a substantial loss of molars. A succesful transplantation depends on the general patient condition, the donor tooth, and the recipient site. An ideal recipient site should have sufficient alveolar bone support, periodontal, and tissue and absence of chronic inflamation. We reported a case of third molar autotransplantation to first molar with periapical granuloma as recipient site with one year follow-up. Autotransplantation process started with an adequate curettage of the recipient site immediately, followed by atraumatic donor tooth extraction. The result of clinical and radiological examination showed no pain, no tooth mobility, and no inflammation at periapical first molar region. Periapical granuloma at the recipient site is not an absolute contraindication of autotransplantation. Extra-alveolar period and atraumatic extraction of the donor tooth during autotransplantation affected the condition of periodontal ligament

Abstract

Background: Autotransplantation is a surgical movement to reposition teeththat have been erupted, partially erupted or not yet erupted from one place to another in the same individual, either on post-extraction sockets or artifcial sockets made by surgery. Dental folicle from impacted teeth may develope dentigerous cysts that will result in destruction of the jaw bone. There are many factors that influence the success of autotransplantation, one of which is adequate bone support.The purpose of this poster is to present the successful autotransplantation treatment of impacted right maxillary incisor teeth associated with dentigerous cyst and bone support replacement after cyst enucleation using a carbonate apatite collagen graft, GAMACHA®, at RSUP Dr. Sardjito.Case management: A 9-year-old girl, referred to the Oral and MaxillofacialSurgery Department of RSUP Dr. Sardjito with the complaint of right deciduous maxillary incisor persistence. Radiological examination showing persistent of 51 and 52 radices and impaction of teeth 11 and 12 in a horizontal position above radix 53. Also visible radiolucent image with a frm limit around the crown of 11 that supports the image of cyst. This patient performed cyst enucleation under general anesthesia, removal of impacted 11 and 21, followed by an artifcial socket drilling at the alveolar bone that could support all aspects of the root wall although only on the apical part of the tooth root for teeth 11 and 12 placement. Autotransplantation of teeth11 and 12 followed by the application of GAMACHA® and wire fxation, strengthen with acrylic splint.Conclusion: Two years follow up after surgery, patient have no complaintseither from aesthetic or functional and the result of clinical and radiologicalexamination did not found existence of any pathological abnormalities around teeth 11 and 12.

Abstract

Resection of the jaw is a primary option of treatment in mandibular ameloblastoma with bone damage. Mandibular resection will cause mandibular stability disturbance due toloss of some part of the bone. Instability of the mandible can cause aesthetic, physiological, and psychological malfunctioning. To restore its stability, installment of mandibular reconstruction plate on the remaining mandibular by using screws. However, it is not uncommon that plate exposure occurs following mandibular reconstruction, caused by inaccurate adaptation of the plates to the mandibular bone. A  3D stereolithography modelcan help provide the best assesment on the bone defect, plan the making of the more easily pre-operative curved plates and provide more accurate faster surgery time. Objective: To evaluate plate exposure after mandibular resection with titanium reconstruction plates in Dr. SardjitoHospital. Case report: A case was reported on mandibular reconstruction after resection of ameloblastoma with titanium plate performed in Dr. Sardjito Hospital that experienced intra-oral plate exposure and oro-cutaneous fistula on the left mandible. The clinical evaluation showed the curved titanium plate did not adapt well with the remaining mandibular bone; therefore, the titanium plate was removed without replacing it with a new plate. Inappropriate adaptationof curved titanium plate is one of the main causes of plate exposure. The use of a 3D stereolithography model is very helpful for the success of mandibular reconstruction.

Abstract

Hemangioma merupakan tumor jinak pada sistem pembuluh darah. Lokasi pada rongga mulut sering ditemukan pada daerah bibir, lidah dan mukosa bukal. Terdapat tiga tipe hemangioma berdasarkan gambaran histologinya yaitu kapiler, kavernosa dan telangiektasia. Pilihan pengobatannya adalah dengan pembedahan, skleroterapi, pemberian kortikosteroid dan obat anti kanker. Salah satu bahan yang dapat digunakan untuk skleroterapi adalah boiling water. Panas dari injeksi boiling water akan merusak endotel sehingga menimbulkan inflamasi, terjadi fibrosis dan kemudian menimbulkan obliterasi. Tulisan ini bertujuan untuk melaporkan tindakan pengobatan hemangioma tipe kavernosa pada mukosa bukal dekstra dengan injeksi boiling water secara intralesi tanpa pembedahan. Pasien perempuan berusia 49 tahun didiagnosis mengalami hemangioma tipe kavernosa pada mukosa bukal dekstra. Riwayat ekstirpasi hemangioma 2 bulan yang lalu, namun masih tampak benjolan ukuran 2x1x0,5 cm, batas jelas, warna biru keunguan, permukaan licin, palpasi kenyal, dan nyeri tekan. Penanganan dilakukan dengan injeksi boiling water secara intralesi sebanyak 2,5 ml pada sisi anterior dan 2,5 ml pada sisi posterior. Diberikan dua tahap dengan jarak satu bulan, menggunakan prosedur yang sama. Hasil kontrol satu bulan setelah tindakan kedua didapatkan keluhan rasa tebal pada mukosa bukal dekstra saat diraba, nyeri tidak ada. Pemeriksaan klinis menunjukkan tidak ada benjolan, permukaan mukosa licin, warna sama dengan jaringan sekitar. Pemeriksaan palpasi teraba jaringan kenyal ukuran diameter 0,5 cm, nyeri tekan tidak ada. Tindakan injeksi boiling water pada hemangioma tipe kavernosa dapat memberikan hasil yang baik dengan tidak ada keluhan lagi dan tidak terlihat adanya lesi. Tindakan ini dapat dijadikan pilihan penanganan hemangioma tanpa pembedahan.

Abstract

Human Immunodeficiency Virus (HIV) merupakan virus yang menyebabkan penurunan kekebalan tubuh, dengan target utama limfosit T CD4. Jumlah limfosit T CD4 dapat digunakan untuk mengambil keputusan tentang inisiasi Antiretroviral (ARV) dan pemantau perkembangan penyakit. Pertimbangan tindakan operasi bedah mulut harus memperhatikan kondisi imunosupresi penderita HIV. Pengobatan HIV menggunakan ARV dalam jangka panjang dapat menimbulkan efek samping gangguan fungsi hati, anemia, dan alergi, sehingga penggunaannya perlu diperhatikan untuk mencegah infeksi oportunistik. Penulisan makalah ini bertujuan untuk memberikan pertimbangan tindakan bedah mulut pada pasien HIV/AIDS. Pasien laki-laki berusia 22 tahun, penderita HIV/AIDS dengan hitungCD4 >500 sel/μL. Pasien mengkonsumsi ARV dalam bentuk FDC (Fixed Dosed Combination) yang mengandung Tenofovir 300 mg, Hiviral 300 mg, dan Efavirenz 600 mg. Bius umum dilakukan pada tindakan odontektomi multipel dengan pemilihan obat injeksi Ceftriaxon, Ketorolac, Ranitidin, Asam Tranexamat, dan obat peroral Cefixime, Kalium Diklofenak, Paracetamol. Pemilihan obat-obatan tersebut berdasarkan interaksi dengan ARV yang digunakan pasien. Alat Proteksi Diri (APD) selama tindakan bedah perlu digunakan untuk mencegah penularan ke operator. Tindakan odontektomi multipel pada pasien ini menunjukkan hasil yang baik dengan tidak adanya keluhan, pembengkakan pada pipi, dan parestesi 7 hari setelah tindakan. Pasien juga menunjukkan pembukaan mulut normal dan penyembuhan luka yang baik. Kesimpulan: tindakan odontektomi multipel dapat dilakukan pada penderita HIV/AIDS dengan mempertimbangkan status CD 4 dan pemilihan obat.

Abstract

Neglected fracture adalah fraktur yang tidak mendapatkan perawatan adekuat sehingga terjadi displacement tulang dan perubahan tarikan otot yang menyulitkan tindakan reposisi. Penggunaan model mandibula 3D dapat membantu menentukan alignment mandibula normal, dan pembuatan plat akrilik untuk memfiksasi posisi mandibula. Laporan kasus ini menampilkan pemanfaatan model mandibula 3D dan plat akrilik pada proses reposisi neglected fracture mandibula. Seorang anak laki-laki usia 3 tahun dirujuk ke RSUP Dr Sardjito karena bentuk rahang bawah miring ke sebelah kiri. Menurut orangtuanya pada saat dilahirkan pasien terjatuh dengan posisi wajah terbentur dilantai. Pemeriksaan objektif ekstra oral terdapat deviasi mandibula ke kiri, palpasi anterior tragus condyle sinistra tidak terdapat pergerakan saat membuka dan menutup mulut. Pemeriksaan intraoral palpasi regio angulus mandibula sinistra teraba tidak rata. Gambaran CT Scan terdapat fraktur angulus mandibula dengan gap sebesar 1 cm. Hasil pemeriksaan terdapat neglected fracture mandibula sehingga dilakukan tindakan operasi rekonstruksi. Tindakan reposisi dan reduksi area fraktur durante operasi dilakukan dengan pemanfaatan plat akrilik yang dibuat dengan model mandibula 3D sebelum operasi. Hasil operasi setelah 1 tahun menunjukkan gambaran wajah simetris dan hasil pemeriksaan ronsen panoramik dan CBCT posisi plate pada angulus mandibula baik serta tampak penyembuhan tulang. Reposisi dengan memanfaatkan model mandibula 3D dan plat akrilik dapat membantu tindakan reposisi, meningkatkan akurasi penempatan plate, dan mempercepat waktu operasi.

Abstract

Improper placement of the reconstruction plate can lead to postoperative complications. Placing reconstruction plates that have arch for operation is made easier with the plate positioning guide (PPG) as a transfer method of reconstruction plate. This study aimed to assess the impact of PPG on the placement accuracy of reconstruction plate as determined by the symmetrical breadth of the mandibular arch and the consistent placement of reconstruction plate following hemimandibulectomy. According to the inclusion criteria, this was a retrospective cross-sectional study with 12 participants (n = 12) after hemimandibulectomy. Subjects were divided into PPG (n = 6) and Non-PPG (n = 6) groups. Measurement of the difference in the width of the mandibular arch and the height of the placement of the reconstruction plate between the reconstructed and non-resected sides in three positions (anterior, posterior, and angulus) was conducted using submentovertex radiographs and orthopantomogram (OPG) through the RadiAnt DICOM Viewer software. The placement accuracy of the reconstruction plates was analyzed using a one-sample t-test with p-value 0.05. The results showed that the placement accuracy of the reconstruction plate in the PPG group were accurate (p = 0.590), and the Non-PPG group was inaccurate (p = 0.000); with the independent sample test. There was a significant difference between the two groups (p = 0.000). Plate positioning guide provides better symmetry of the mandibular arch width post-hemimandibulectomy.

Abstract

Tooth avulsion is a severe condition of traumatic dental injury. The long-time effect of untreated tooth avulsion is alveolar bone loss. The mini dental implant can be a treatment for single tooth replacement in a late tooth avulsion with bone resorption. The purpose of this study is to present a case report of the mini dental implant in a late avulsion of an anterior maxillary tooth without bone augmentation. Thirty years old male patients came to clinic with the left upper central incisor avulsed due to a traffic accident three years before. There has been mild vertical and horizontal bone resorption and diastemas. He wanted to replace the missing tooth with a fixed denture, and the patient refused any bone augmentation. The mini dental implant was an alternative treatment for this case. Orthodontic treatment was performed to regain space before mini dental implant placement because some teeth had been migrated, causing some diastemas. After seven months of orthodontic treatment, we inserted a 3.0 x 13.0 mm nobel active implant bone level and installed the healing abutment. The prosthetic crown was screwed carefully using a manual torque wrench with a maximum torque force not exceeding 15 Ncm. The mini dental implant could be a treatment for a late avulsion case without bone augmentation. DOI : 10.54052/jhds.v2n3.p405-414
