Abstract

Infeksi Saluran Kemih (ISK) merupakan infeksi berupa pertumbuhan dan perkembangbiakan mikroorganisme dalam saluran kemih yang meliputi ginjal sampai kandung kemih,diantaranya Sistitis ( infeksi kandung kemih). Ultrasonografi (USG) dasawarsa terakhir ini merupakan pemeriksaan yang sering digunakan sebagai pilihan penunjang diagnostik pada beberapa kasus yang berhubungan dengan infeksi kandung kemih. Sedimen urin leukosit merupakan pemeriksaan semikuantitatif sebagai penunjang diagnosis Sistitis dengan acuan kadar sedimen urin leukosit positif. Penelitian ini bertujuan untuk mengetahui hubungan penebalan dinding kandung kemih pada pemeriksaan USG dengan sedimen urin leukosit pada penderita dengan klinis Sistitis. Desain penelitian ini observasional dengan studi cross sectional,menggunakan data sekunder dari catatan rekam medis pasien RS PKU Muhammadiyah I-II Yogyakarta untuk semua kasus ISK periode 1 Juli 2010 sampai 31 Agustus 2011. Data rekam medis yang digunakan adalah subyek penelitian dengan suspek infeksi kandung kemih yang mempunyai hasil laboratorium urin (sedimen urin lekosit) dan tebal dinding kandung kemih potongan transversal dan longitudinal pada pemeriksaan USG. Hasil analisis data dengan uji chi square didapatkan nilai p 0,631, sehingga dapat disimpulkan bahwa tidak terdapat hubungan penebalan dinding kandung kemih pada USG dengan hasil pemeriksaan sedimen urin leukosit.Urinary Tract Infection (UTI) is a form of infection that involve growth and proliferation of microorganisms in the urinary tract includes kidney to the bladder, one type of UTI is cystitis (bladder infection). Ultrasonography (USG) examination in the last decade is frequently used as a diagnostic support in some cases associated with bladder infection. Examination of leukocyte urine sediment is a semiquantitative test that could be supporting a diagnosis of bladder infection with reference levels of urine sediment positive leukocytes. This study aims to determine the relationship between bladder wall thickening on ultrasonography with urine sediment of leukocytes in patients with clinical bladder infection. The study design was observational with cross sectional study using secondary data from the medical records of PKU Muhammadiyah Hospital of Yogyakarta I-II for all cases of Urinary Tract Infection in the period July 1, 2010 until August 31, 2011. Medical record data used in this study were research subjects with suspected bladder infection who had a urine laboratory results (urine sediment leucocytes) and bladder wall thickness transverse and longitudinal cuts on ultrasound examination. The results of data analysis with Chi-Square test p-value obtained 0.631. There is no relationship between the thickening of the bladder on  ultrasonography with urine sediment leukocyte.

Abstract

BACKGROUND : Type 2 diabetes mellitus (DM) is one of the major problems faced by the health system. Diabetes is a multifactorial, chronic and progressive metabolic disorder characterized by chronic hyperglycemia. Persistent hyperglycemia is associated with long-term damage to the  liver. Ultrasound elastography is one of imaging modality that can assess liver stiffness, thus can assist clinicians in determining therapy, evaluating, and predicting disease progression. AIMS :  Aim of this study was to determine the relationship between liver stiffness degree by using ultrasound elastography with the duration of T2DM and HbA1c levels. METHOD : This study was an observational analytic research with a cross-sectional approach which carried out from May to September 2022 at the radiology department of Dr. Kariadi Hospital Semarang. Fifty subjects were included in this study and Rank-Spearman correlation test was used to analyze the relationship between liver stiffness degree with T2DM duration and HbA1c levels. RESULT :  Using ultrasonography elastography, the liver stiffness average was 7.92 + 6.72 kPa. The average of HbA1c levels was 8.55 + 2.63%. T2DM duration average was 9.10 + 6.50 years.   The rank-spearman test showed a significant correlation between HbA1c levels and the degree of liver stiffness (p = 0.000) with a correlation coefficient (r) of 0.68. There was no significant correlation between the T2DM duration and the degree of liver stiffness (p = 0.052). CONCLUSION : There was a significant correlation between liver stiffness and HbA1c levels, but there is no correlation between liver stiffness and T2DM duration

Abstract

Type 2 Diabetes Mellitus (DM2) is a chronic disease that requires high compliance in treatment to prevent complications. However, many DM2 patients are not compliant in undergoing drug therapy, which can hinder the effectiveness of treatment. Various factors have been identified as determinants of patient compliance, including demographic factors, knowledge, social support, and technology utilization.This study aims to analyze factors that influence medication adherence in DM2 patients based on a systematic review of several studies that have been conducted. The method used in writing this article is a literature review study with a systematic review method. The articles reviewed were obtained through searches on Google Scholar, Pubmed, Proquest, and ScienceDirect in the period 2015-2025 with the search keywords being Type 2 Diabetes Mellitus, Medication Compliance, Compliance Factors, Medication Supervisors . By using PRISMA, from 10,697 articles found, 10 journal articles were obtained to be reviewed. Based on the analysis of several studies, factors that have a significant influence on patient compliance in taking medication are the level of knowledge (p = 0.002), family support (p = 0.000; PR = 8.17 ) , and support from health workers (p = 0.000; PR = 6.10). Social Marketing, Frailty Syndrome and Age were also found to have a relationship with DM medication compliance. In addition, the use of technology such as medication reminder applications (PMO) has been shown to help improve patient compliance in undergoing DM2 therapy. Compliance of DM2 patients in undergoing drug therapy is influenced by various factors, including level of knowledge, social support, social marketing, age , frailty syndrome, and use of technology. Therefore, strategies to improve compliance must involve an educational approach, strengthening the role of families and health workers, and utilizing digital-based technology to increase the effectiveness of DM2 therapy.

Abstract

Background: Diabetes Mellitus (DM) is a chronic disease with a prevalence rate that continues to increase in Indonesia. Compliance with taking medication is a key factor in managing DM to prevent serious complications. However, the level of patient compliance with treatment is still a challenge. One factor that can increase compliance is education about the importance of DM therapy. Objective: This study aims to explore the characteristics of education about DM treatment that influence patient compliance in taking medication based on factors such as age, gender, education level, occupation, economic status, and place of residence in Indonesia. Method: This study used a descriptive analytical design with secondary data from the 2023 Indonesian Health Survey (SKI). The study sample included DM patients who were diagnosed and received drug therapy. The main variables analyzed were the proportion of patients who received education and their level of compliance in taking medication, which were categorized by age group, gender, education, occupation, economic status, and place of residence. Data analysis was carried out descriptively to identify the proportion of education in compliance with taking DM medication in Indonesia. Results: A study from the Indonesian Health Survey (SKI) showed that 81.4% of patients received education about the importance of taking medication regularly, and of that number, 90% showed high compliance in taking medication. Meanwhile, only 81% of patients who did not receive education remained compliant with their treatment. Conclusion: Education has been shown to have a significant relationship with adherence to taking DM medication in Indonesia. Patients who receive education have a higher level of adherence than those who do not receive education. Age, education, occupation, and economic status factors play a role in determining the level of patient adherence. The results of this study can be the basis for designing education-based interventions to improve adherence to DM therapy in Indonesia.
