Abstract

Perawatan pasien di Intensive Care Unit (ICU) tidak hanya melibatkan petugas kesehatan, tetapi juga memerlukan peran keluarga. Keluarga juga mengalami stres dan kecemasan selama menunggu pasien di ICU. Patient-Family Centered Care (PFCC) merupakan paradigma baru dalam pelayanan kesehatan yang menempatkan pasien dan keluarga sebagai fokus pemberian asuhan keperawatan. Kemiraan keluarga sangat berperan dalam merawat pasien di ICU. Penelitian ini bertujuan untuk mengeksplorasi perspektif keluarga dan perawat tentang kemitraan keluarga dalam merawat pasien di ICU. Desain penelitian adalah kualitatif dengan pendekatan fenomenologi intepretatif. Informan adalah 10 perawat ICU dan 10 keluarga pasien sesuai dengan kriteria inklusi dan ekslusi yang diambil dengan metode purposive sampling. Penelitian dilakukan di Rumah Sakit Umum Pusat di Kota Semarang pada bulan April sampai Mei 2018. Analisis data dengan teknik intepretative phenomenological analysis (IPA). Hasil penelitian menunjukkan 4 tema utama tentang makna kemitraan keluarga, bentuk kemitraan keluarga, pengaruh kemitraan keluarga, dan tantangan penerapan kemitraan keluarga di ICU. Kemitraan antara perawat dan keluarga diartikan sebagai keadaan dimana perawat sebagai sumber informasi tentang kondisi pasien, perawat memberikan dukungan dan menghargai keluarga, kolaborasi antara pasien dan perawat, partisipasi keluarga dalam memberi dukungan bagi pasien, keluarga bisa mengungkapkan perasaanya pada perawat. Kemitraan keluarga meningkatkan kenyamanan pasien, tetapi mempengaruhi pemberian asuhan keperawatan dan menambah beban kerja perawat. Perspektif keluarga tentang perawatan ICU dan ketidaksiapan perawat menjadi tantangan penerapan kemitraan keluarga di ICU. Kesimpulan dari peneliian ini adalah keluarga dan perawat memahami makna dan pengaruh kemitraan dalam merawat pasien di ICU, tetapi masih terdapat tantangan dan hambatan dalam penerapannya.

Abstract

AbstractBackground: Haematoma is the main vascular complication in the patients after percutaneous coronary intervention (PCI). This incidents is important to be prevented and be overcome because the internal bleeding is not easy to be control and it can be make extend the mass of patients to inpatient in the hospital. Artery-femoral Band (arfeband) is a device designed that worthy and safe to keep the stability of haemostasis after PCI. Aim: it is to know the influence of use arfeband to haematoma incidents in patients after PCI. Methods: this research used the quasy - experiment pre-post control trial design, it involves 121 patients after the elective of PCI that receives the intervention group (IG) and control group (CG) that is done for 6 hours after the removal of the sheath femoral artery, 60 IG used arfeband and 61 CG with the sand bag. The measurement of diameter haematoma is done when before and after intervention. Results: this research shows that before the intervention, there are no differences that significant between IG and CG p=0,909. There are the significant differences to haem atom incidents after the use of IG with P0,05, OR 0,398 with CI 95%  0,172 – 0,919. It means that arfeband is reducing the haematoma incidents for 60,2 %. The size of the diameter of pre and post procedure, it is decrease in average of 30,4(±13,6)mm to be 10,9 (±15,9) p0,05. Conclusion: The intervention of use arfeband influences to decrease the haematoma incidents significantly after percutaneous coronary intervention (PCI).

Abstract

BACKGROUND: More than 75% of people with type 2 DM will anguish diabetic retinopathy 20 years after being diagnosed. However, the current changing of people’s lifestyle contributes to the tendency in type 2 DM that diagnosed at a younger age. This causes diabetic retinopathy can be occurred earlier and blindness will occur more quickly. It is important to prevent diabetic retinopathy by controlling some associated factors. OBJECTIVE: To investigate associated factors of diabetic retinopathy in people with type 2 DM. METHOD: Analytic observational study with cross-sectional design. Subjects were determined by purposive sampling. The subjects were interviewed using a questionnaire meanwhile, funduscopic examination is aimed to assess diabetic retinopathy and followed by deep interview. This study also used the subject’s medical record to complete the required data. The results were analyzed by using statistic software. RESULT: From 43 subjects, there wes no significant relation among age (p=0.965), gender (p=0.437), level of knowledge (p=0.640), accessibility (p=1.000), economic level (p=0.680), duration of DM (p=0.477), history of hypertension (p=0.708), and history of dyslipidemia (p=0.244) with diabetic retinopathy in people with type 2 DM. However, there was significant relation among controlling blood glucose (p=0.014) with diabetic retinopathy in people with type 2 DM. Uncontrolled blood glucose was 13 times more affected in diabetic retinopathy than controlled blood glucose (OR 13.417, 95% CI:1.474-122.117). CONCLUSION: There was a significant relation between controlling blood glucose and diabetic retinopathy in people with type 2 DM. Controlling blood glucose is the most dominant risk factor among the other factors in this study. Keywords: diabetic retinopathy; risk factor; type 2 diabetes mellitus

Abstract

BACKGROUND: Liver cirrhosis is an end-stage liver disease characterized by pathologic fibrosis and regenerative nodules with resultant liver dysfunction. The diagnostic hallmark of portal hypertension is slow flow velocities in addition to the increased caliber of the mean portal vein. That is, the diagnosis of portal hypertension requires the measurement of mean portal velocity and portal diameter, and the correlation between cirrhosis and mean portal velocity and port diameter is the correlation between liver cirrhosis and its complications.OBJECTIVE: To analyze the correlation between portal vein dimensions and liver stiffness in patients of Child-Pugh A cirrhosis.METHOD: This study was a cross-sectional study on 30 subjects with Child-Pugh A liver cirrhosis. The subjects of this research are patients who come to the Radiology Department of the Dr. Kariadi Hospital in Semarang for point shear wave elastography and abdominal ultrasound examination from January to December 2022. Spearman test correlation was used for the analysis.RESULT: Spearman test showed no correlation between liver stiffness and portal vein diameter (p=0.250, r= -0.217), liver stiffness and mean portal vein velocity (p=0.883, r= -0.028), and portal vein diameter with mean venous velocity in Child-Pugh A liver cirrhosis (p = 0.979, r = 0.005)

Abstract

Background: Hepatitis C virus (HCV) infection remains a significant public health burden in Indonesia, affecting approximately 2.5 million people. The introduction of direct-acting antivirals (DAAs) has revolutionized HCV management by offering highly effective and well-tolerated treatment options. Since 2017, Indonesia has implemented an HCV cascade of care and a government-funded DAA program to support the World Health Organization (WHO) goal of HCV elimination by 2030. However, challenges in diagnostic accessibility and treatment follow-up continue to hamper progress. This study aims to evaluate the implementation of the HCV cascade of care at Dr. Kariadi General Hospital, Semarang, Indonesia.Methods: A retrospective descriptive study was conducted at Dr. Kariadi General Hospital, which included data from patients who tested positive for anti-HCV antibodies between January 2019 and November 2024. HCV treatment was categorized into five stages: (1) positive anti-HCV antibody test, (2) HCV RNA test, (3) positive HCV RNA result, (4) treatment initiation, and (5) sustained virologic response 12 weeks post-treatment (SVR12). Data analysis was performed using SPSS Statistics version 26. Categorical variables were expressed as frequencies and percentages, and findings were presented as numbers and proportions across each stage of the care cascade.Results: Among 317 patients with positive anti-HCV antibody results (55% men, 45% women; median age: 56 years), 280 (88%) underwent HCV RNA testing, and 213 (76%) were confirmed positive. Of these, 185 patients (87%) initiated treatment, 144 (78%) completed therapy, and 108 (75%) achieved SVR12.Conclusion: This study shows that significant gaps remain at various stages of the HCV care cascade, particularly in confirmatory HCV RNA testing, treatment initiation, and post-treatment follow-up.
