Abstract

Abstract: Diabetes is one of the most common diseases found in COVID-19 patients. People with diabetes have a higher risk of developing a more severe form when infected with the SARS-CoV2 virus. This study aimed to determine the relationship of hyperglycemia and the outcome of COVID-19 patients. This was a literature review study using three databases, namely Pubmed, ClinicalKey, and Google Scholar. The outcomes studied were the degree of COVID-19 disease and the mortality rate of COVID-19 patients. The results obtained 10 articles that met the inclusion and exclusion criteria. COVID-19 patients suffering from hyperglycemia, both those with and without a history of diabetes, experienced more severe degree of illness and higher mortality rate compared to patients without hyperglycemia. In conclusion, there is a relationship between hyperglycemia and the severity of illness and mortality in COVID-19 patients.Keywords: hyperglycemia; COVID-19 Abstrak: Diabetes melitus (DM) merupakan salah satu penyakit yang paling sering ditemukan pada pasien COVID-19. Penyandang DM berisiko lebih tinggi untuk berkembang menjadi bentuk yang lebih parah ketika terinfeksi virus SARS-CoV2. Penelitian ini bertujuan untuk mengetahui hubungan hiperglikemia dengan luaran pasien COVID-19. Jenis penelitian berbentuk literature review dengan pencarian data menggunakan tiga database yaitu Pubmed, ClinicalKey,dan Google Scholar. Luaran yang diteliti yaitu derajat penyakit dan angka kematian pasien COVID-19 dengan DM. Hasil penelitian mendapatkan 10 artikel yang memenuhi kriteria inklusi dan ekslusi. Pasien COVID-19 dengan hiperglikemia baik yang penyandang DM maupun tanpa riwayat DM mengalami keparahan penyakit yang lebih berat dengan angka kematian yang lebih tinggi dibanding pasien tanpa hiperglikemia. Simpulan penelitian ini ialah terdapat hubungan antara hiperglikemia dengan keparahan penyakit dan angka kematian pasien COVID-19.Kata kunci: hiperglikemia; COVID-19

Abstract

Abstract: Increased level of blood lipid is one of the important causes that contribute to coronary heart disease (CHD). Physical activity is known to affect blood cholesterol level. Moreover, physical activity in the form of exercise performed for 30 minutes regularly 3-5 days a week can reduce the level of low-density lipoprotein (LDL) cholesterol and increase the level of high-density lipoprotein (HDL) cholesterol. This study aimed to determine the relationship between physical activity and lipid profile in patients with CHD. This was a literature review study. There were 10 articles in this study. The results showed that there was a change in the lipid profile after physical activity in patients with CHD, however, in several studies, the lipid profile did not change significantly. In conclusion, patients with coronary heart disease (CHD) without contraindications who do moderate to high intensity physical activity will get benefit of improving blood lipid profiles. Keywords: coronary heart disease; llipid profile; physical activity Abstrak: Peningkatan kadar lipid darah merupakan salah satu penyebab penting yang berkontribusi terhadap penyakit jantung koroner (PJK). Aktivitas fisik diketahui dapat memengaruhi kadar kolesterol dalam darah. Aktivitas fisik berupa olahraga yang dilakukan selama 30 menit secara rutin 3-5 hari dalam seminggu dapat menurunkan kadar kolesterol low-density lipoprotein (LDL) dan meningkatkan kadar kolesterol high-density lipoprotein (HDL). Penelitian ini bertujuan untuk mengetahui hubungan aktivitas fisik terhadap kadar profil lipid pada penderita penyakit jantung koroner. Jenis penelitian ialah literature review. Hasil penelitian berdasarkan 10 literatur mendapatkan adanya perubahan profil lipid penderita PJK setelah melakukan aktivitas fisik, namun pada beberapa penelitian kadar profil lipid tidak berubah secara bermakna. Simpulan penelitian ini ialah penderita penyakit jantung koroner (PJK) tanpa kontraindikasi yang melakukan aktivitas fisik dengan intensitas sedang sampai tinggi akan memperoleh manfaat berupa perbaikan profil lipid darah. Abstract

Abstract: Remdesivir is used as the treatment for coronavirus disease 2019 (Covid-19). Moreover, its pharmacological effect has an impact on the improvement of inflammatory biomarkers. Neutrophil lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR), and ferritin are simple inflammation parameters that are widely available and can describe inflammation in Covid-19 patients. This cross-sectional study involved 52 Covid-19 patients at Prof. Dr. R. D. Kandou Hospital from March 2020 to March 2021. Blood tests were performed on the day before remdesivir administration and one week after administration. The results obtained a significant correlation between NLR and PLR with a value of r=0.641-0.670 (p<0.001) before and after administration of remdesivir. In all patients there was a decrease in ferritin with remdesivir administration (∆= -111.41). In conclusion, there are decreases in NLR, PLR and ferritin values ​​in Covid-19 patients with increased transaminase after being treated with remdesivir, especially living patients. Efek farmakologik remdesivir juga berdampak terhadap perbaikan biomarker inflamasi. Neutrophil lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR), dan feritin merupakan parameter inflamasi sederhana yang banyak tersedia dan dapat menggambarkan inflamasi pada pasien Covid-19. Studi potong lintang ini menelaah 52 pasien Covid-19 di RSUP Prof. Dr. R. D. Kandou sejak Maret 2020 hingga Maret 2021. Pemeriksaan darah diambil pada hari sebelum pemberian remdesivir dan satu minggu setelah pemberian. Hasil penelitian mendapatkan korelasi bermakna antara NLR dan PLR dengan nilai r=0,641-0,670 (p<0,001) baik sebelum dan setelah pemberian remdesivir. Pada keseluruhan pasien terdapat penurunan ferritin dengan pemberian remdesivir (∆=-111,41). Simpulan penelitian ini ialah terdapat penurunan nilai NLR, PLR dan ferritin pada pasien Covid-19 setelah diberikan remdesivir terutama pasien hidup.
