Abstract

Pasien stroke di Indonesia banyak mengalami keterlambatan kedatangan ke rumah sakit atau prehospital delay. Pencegahan dalam mengurangi kematian dan meminimalkan kerusakan otak adalah penanganan yang cepat dan tepat sesuai golden period. Prehospital delay akan menghalangi terapi reperfusi pada pasien stroke iskemia. Derajat kerusakan saraf pada pasien stroke akan mempengaruhi kualitas hidup pasien. Penelitian ini betujuan untuk mengetahui hubungan antara prehospital delay dengan derajat kerusakan saraf pada pasien stroke iskemia. Desain dari penelitian ini adalah analitik observasional dengan pendekatan cross sectional dengan data sekunder dari rekam medik NIHSS pasien stroke iskemia di RSUD Margono Soekarjo dari bulan April sampai dengan November 2021. Didapatkan sebanyak 78 pasien yang memenuhi kriteria eksklusi dan inklusi terdiri dari 43 pasien yang mengalami prehospital delay < 24 jam dan 35 pasien yang mengalami prehospital delay > 24 jam. Hasil analisis bivariat antara prehospital delay dengan derajat kerusakan saraf berdasarkan kriteria NIHSS menunjukkan nilai (p= 0,831 dan r= -0,25) pada pasien stroke iskemia. Tidak terdapat korelasi yang bermakna antara prehospital delay dengan derajat kerusakan saraf pada pasien stroke iskemia dengan kekuatan korelasi yang lemah dan arah korelasi negatif, artinya semakin rendah skor NIHSS semakin lama prehospital delay. Kesimpulan penelitian ini adalah tidak terdapat korelasi prehospital delay dengan derajat kerusakan saraf berdasarkan kriteria NIHSS pada pasien stroke iskemia di RSUD Margono Soekarjo

Abstract

Salah satu faktor risiko dari penyakit Parkinson adalah hipertensi, namun apakah hipertensi berhubungan dengan progresivitas penyakit Parkinson masih belum jelas. Kriteria Hoehn & Yahr merupakan salah satu kriteria yang dapat digunakan untuk menggolongkan progresivitas penyakit Parkinson menjadi 5 stadium. Tujuan penelitian ini adalah mengetahui hubungan antara hipertensi dengan stadium penyakit Parkinson berdasarkan kriteria Hoehn & Yahr pada pasien Parkinson RSUD Margono Soekarjo Purwokerto. Metode penelitian ini menggunakan design cross-sectional dengan sampel penelitian sebanyak 48 pasien di poli saraf RSUD Margono Soekarjo Purwokerto. Variabel hipertensi diukur pada saat pasien datang ke poli saraf RSUD Margono Soekarjo Purwokerto untuk kontrol penyakit Parkinson dan variabel stadium penyakit Parkinson berdasarkan kriteria Hoehn & Yahr dinilai dengan pengamatan peneliti. Data penelitian dilakukan analisis menggunakan uji Somers’d. Hasil analisis bivariat antara hipertensi dengan stadium Hoehn & Yahr menunjukkan nilai p = 0,517 dan r = -0,084. Artinya tidak terdapat korelasi yang bermakna antara hipertensi dengan stadium penyakit Parkinson berdasarkan kriteria Hoehn & Yahr dengan kekuatan korelasi yang sangat lemah dan arah korelasi negative. Hal tersebut berarti semakin tinggi stadium hipertensi maka semakin rendah stadium Hoehn & Yahr. Dari penelitian ini dapat disimpulkan bahwa tidak terdapat korelasi yang bermakna antara hipertensi dengan stadium penyakit Parkinson berdasarkan kriteria Hoehn & Yahr.

Abstract

Sleep deprivation (SD) can modulate the production of various cytokines, including pro-inflammatory cytokines such as IL-6, TNF-α, and IFN-γ, and anti-inflammatory cytokines such as IL-10. Paradoxical sleep deprivation (PSD) increases the risk of inflammation but can be relieved by sleep recovery (SR). This study aimed to determine the differences in levels of cortisol and inflammatory cytokines (IL-6, IL-10, TNF-α, dan IFN-γ) in male Wistar rats (Rattus norvegicus) after induction of various sleep deprivation stress models. Twenty-five of male Wistar rats were randomly divided into five groups: control, PSD (20 hours of SD/day for five days), Total Sleep Deprivation or TSD (24 hours of SD/day for five days), PSD+SR (PSD followed by SR), and TSD+SR (TSD followed by SR). The plasma cortisol levels were measured with ELISA, and inflammatory cytokine levels were measured with immunoassay and calculated with fold change. Mean cortisol levels were significantly increased in treatment groups compared to the control group (p=0.029). Multivariate analysis showed no statistically significant difference in inflammatory cytokine levels of IL-6 (p=0.658), IL-10 (p=0.065), TNF-α (p=0.399), and IFN-γ (p=0.283) in all groups. In conclusion, various sleep deprivation stress models affect cortisol levels but not inflammatory cytokine levels of IL-6, IL-10, TNF-α, and IFN-γ among male Wistar rats.

Abstract

Background: The combination of tuberculous meningitis and tuberculous spondylitis as a manifestation of disseminated tuberculosis (TB) is a rarely reported condition often associated with poor clinical outcomes due to delayed diagnosis. This case report aims to highlight the importance of maintaining clinical suspicion for disseminated TB in patients presenting with a combination of neurological symptoms and chronic back pain. Case Presentation: A 30 year old woman presented with a 15 month history of progressive chronic low back pain radiating to both legs, which subsequently progressed to headache and altered consciousness. Neurological examination revealed signs of meningeal irritation, paraparesis, and sensory impairment at the T4 level. Imaging studies demonstrated multiple foci of tuberculous spondylitis and meningeal involvement, while cerebrospinal fluid analysis and molecular testing confirmed tuberculous meningitis. Based on the combnation of clinical manifestations, imaging findings, and ancillary tests, the patient was diagnosed with disseminated TB. She received anti-tuberculosis therapy, corticosteroids, and underwent a laminectomy with posterior fusion. Discussion: This case demonstrates that the combination of chronic back pain and progressive neurological symptoms should raise suspicion of disseminated TB, even though these manifestations rarely occur concurrently. Early diagnosis achieved through the integration of clinical, radiological, and microbiological findings enables appropriate multidisciplinary therapy, thereby contributing to improved clinical outcomes. Conclusion: Disseminated tuberculosis should be considered in patients presenting with a combination of neurological symptoms and chronic back pain. Clinical vigilance, early diagnosis, and adequate multidisciplinary management play a crucial role in improving the patient's prognosis and clinical outcomes.

Abstract

Background: Working positions in the manual stemped of batik production process involve static body posture, either sitting or bending over for a long time are contributing factors of cause Low Back Pain (LBP). The prevalence of Low Back Pain in Indonesia between 7.6% - 37% in individuals aged 30-50 years, with risk factors including physical activity involving lifting heavy objects and in cases of obesity. Complaints of LBP among the populations of stamped batik craftsman in Cipocok Jaya Subdistrict have not been previously study, thus it is necessary to understand the relationship between working positions and the incidence of LBP. Objective: Identify the relationship between work position and the incidence of LBP in batik stamped craftsman in Cipocok Jaya. Method: This analytical observational research with a cross-sectional study approach using purposive sampling method involving 42 participants. The exclusion criteria were related to analgesic consumption, the use of vertebral support devices, systemic disease, and pregnancy. Work position risk levels were assessed using the Rapid Entire Body Assessment (REBA) instrument while LBP complaints were based on the Nordic Body Map (NBM). Bivariate analysis was done using the Chi-square test. Result: The research results showed that 21 craftsmen (50%) had LBP, while 21 others did not had LBP. A total of 15 craftsmen (35.7%) were classified as mild–moderate risk (REBA 7) while the others 27 people (64.3%) were classified as high–very high risk (REBA >7). Bivariate analysis showed a moderate correlation between REBA scores and LBP complaints (p=0.001; contingency coefficient=0.480). Conclusion: There was a relationship between work position and the incidence of LBP among stamped batik craftsmen in Cipocok Jaya Subdistrict. Therefore, improvements in the ergonomic aspects of work positions that have the potential to cause LBP complaints are needed by performing muscle stretching activities before working and receiving support from the management of the central batik center in providing work facilities that promote health.
