Abstract

ABSTRACTIntroduction: Shortness of breath in patients with congestive heart failure is caused by impaired lung function due to pulmonary. One of the efforts that can be done to overcome shortness of breath and improve oxygenation status is to change the position of the trunk posture in fowler 30o. Objectives: This case study was conducted to analyze the application of the 30o trunk posture in fowler position in patients with congestive heart failure who experience shortness of breath. Method: The research used a a case study of patients with congestive heart failure who experience shortness of breath. Results: After giving a 30o trunk posture in fowler position for three days of treatment, there was a change of blood pressure, pulse rate, respiration and oxygen saturation of the patient. Conclussion: The position of the trunk in fowler posture 30o can reduce shortness of breath in patients with congestive heart failure.ABSTRAKPendahuluan: Sesak nafas pada penderita gagal jantung kongestif disebabkan oleh terganggunya fungsi paru karena adanya edema pulmonal. Salah satu upaya yang dapat dilakukan untuk mengatasi sesak nafas dan memperbaiki status oksigenasi adalah dengan mengubah posisi trunk posture in fowler 30o. Studi kasus ini dilakukan untuk menganalisis penerapan posisi trunk posture in fowler 30o pada pasien gagal jantung kongestif yang mengalami sesak nafas. Metode: Penelitian ini menggunakan desain evaluatif yaitu studi kasus pada pasien dengan kongestif heart failure dengan sesak nafas. Hasil: Setelah dilakukan oberservasi pemberian posisi trunk posture in fowler 30o selama 3 hari perawatan, terdapat perubahan tekanan darah, denyut nadi, pernafasan dan saturasi oksigen pasien. Simpulan: Posisi trunk postur in fowler 30o dapat mengurangi sesak napas pada pasien gagal jantung kongestif.

Abstract

The unpleasant experience of post-fracture patient pain can affect hemodynamics and impaired mobilization. One of the non-pharmacological therapies, cold compress therapy has proven to be effective for treating acute pain in fracture patients. This review is to explore and evaluate the association with cold compress therapy in the management of acute pain in fracture patients and the outcomes provided for patients with various types of fractures. A systematic review was carried out using three electronic databases, namely Google Schoolar, PubMed and Journal of Islamic Nursing using key words in the search for articles determined using PICO. P: Patients with fractures, I: acute fracture pain, C: the effectiveness of cold compresses O: lowering the pain scale. Quasi-Experimental Articles published between 2013-2019 in English and Indonesian. This review looks at various types of fractures which assesses the effect of cold compress therapy in reducing the acute pain scale in fracture patients. Based on the search found 378 articles. Obtained 7 Quasi Experimental articles (165 participants) were identified and assessed for pain. Cold compress therapy is generally given with companion intervention starting from 10-15 minutes, with a temperature of 5 -10˚C using cold water (could pack) is effective for reducing pain and edema. Cold compress therapy is an easy and simple therapy that can be used as a companion to pharmacological therapy and has proven effective for fracture patients. We recommend using this therapy for fracture patients from the first 4 hours post-fracture or 7-8 hours post-surgery to reduce pain from 4-5 to a 3-2 scale.

Abstract

Background: Oxygen saturation is one of the important parameters in assessing the patient's oxygenation condition, especially during the recovery phase in the Post-Anesthesia Care Unit (PACU). This phase is very crucial because patients who have just undergone anesthesia are at risk of oxygenation disorders due to the effects of anesthesia, surgical procedures, or the patient's physiological condition. Objective: to identify and analyze factors that influence oxygen saturation in post-operative patients in the Post-Anesthesia Care Unit (PACU) and are expected to contribute to improving the quality of post-operative care and preventing serious, potentially life-threatening complications. Method: the data analysis technique uses descriptive analysis, namely presenting the distribution of oxygen saturation data and patient characteristics descriptively in the form of tables or graphs. The sample in this study was post-operative patients in the PACU room of RSI Purwokerto with a total of 62 patients. And using inferential analysis, namely the spearman Rho Test: to examine the relationship between age, gender, smoking status, and blood pressure with oxygen saturation and logistic regression test: to determine the most dominant factor influencing oxygen saturation with n = 62 patients. Results: shows a significant relationship between blood pressure and oxygen saturation, with a correlation coefficient value of 0.742 (very strong relationship category, p <0.05), and in gender, a correlation coefficient value of -0.332 (weak relationship, p <0.05) was obtained, the same as in smoking status showing a correlation coefficient of -0.369 (weak relationship, p <0.05). Conclusion: blood pressure, gender, and smoking status factors are related to oxygen saturation in postoperative patients. Suggestion: for further researchers, it is hoped that they will conduct research in various scopes, for example peri-operative.
