Abstract

Abstract: Documentation of patient nursing records is important as evidence of accountability for nursing actions provided in the service. This study aimed to analyze the relationship between leadership, rewards, and attitudes of nurses in documenting nursing care at the inpatient ward of Ratatotok Buyat General Hospital. This was an analytical and observational study with a cross-sectional design. Sampls were all nurses, totaling 77 people. Data were analyzed with univariate and bivariate analysis of chi-square test. The results showed that documentation of good nursing care was 80.5% and poor nursing care was 19.5%; good leadership was 87% and poor leadership was 13%; rewards experienced by health workers were 67.5% and poor rewards were 32.5%; good attitude was 70.1% and poor attitude was 29.9%. The chi square test obtained a p-value of 0.000 as a whole from each variable.  In conclusion, there is a relationship between leadership, rewards, and attitudes with the documentation of nursing care at the inpatient ward of Ratatotok Buyat General Hospital. Keywords: documentation of nursing care; leadership; rewards; attitudes; nurses   Abstrak: Pendokumentasian catatan keperawatan pasien merupakan hal penting sebagai bukti pertanggungjawaban terhadap tindakan keperawatan dalam pelayanan. Penelitian ini bertujuan untuk menganalisis hubungan antara kepemimpinan, imbalan, dan sikap perawat dalam mendokumentasikan asuhan keperawatan di ruang rawat inap Rumah Sakit Umum Pusat Ratatotok Buyat. Jenis penelitian yaitu observasional analitik dengan studi potong lintang. Sampel ialah seluruh perawat yang berjumlah 77 orang. Analisis data penelitian secara univariat dan bivariat dengan uji chi-square. Hasil penelitian mendapatkan variabel pendokumentasian asuhan keperawatan yang baik sebesar 80,5% dan yang kurang baik 19,5%; kepemimpinan yang baik sebesar 87% dan yang kurang baik 13%; imbalan yang dialami oleh petugas kesehatan sebesar 67,5% dan yang merasa imbalan kurang baik sebesar 32,5%; sikap petugas kesehatan yang baik sebesar 70,1% dan yang kurang baik sebesar 29,9%. Hasil uji chi square mendapatkan nilai p=0,000 secara keseluruhan dari setiap variabel yang diteliti. Simpulan penelitian ini ialah terdapat hubungan kepemimpinan, imbalan dan sikap dengan pendokumentasian asuhan keperawatan di ruang rawat inap Rumah Sakit Umum Pusat Ratatotok Buyat. Abstract

Abstract: Condition of mental health problems is reflected in its impact including a high enough number of people with mental disorders associated with a high economic burden. This study aimed to analyze the community mental health program at Tombulu Health Center and its obstacles in implementing the program. This was a qualitative and descriptive study. Data were obtained through interviews with informants. The results showed that the mental health program was implemented through a home visit program with health examination, monitoring patient progress, and counseling about family involvement in the treatment. Obstacles experienced in implementing this program included the community's stigma against the patients, therefore, the community and families were less involved in the healing process. Efforts to develop the mental health program included increasing family and community involvement in the treatment process, number of health cadres, and mental health promotions. In conclusion, implementation of the mental health program at Tombulu Health Center in the treatment processs of patients is carried out through home visits, early detection program, and screening to identify mental health problems for the community and health workers. Obstacles include the absence of family support, negative stigma from the community, lack of supporting facilities and infrastructure for the patients, and the hospital location which is quite far away. Keywords: mental health; development of community mental health program   Abstrak: Kondisi masalah kesehatan jiwa tercermin dari dampak yang ditimbulkannya, antara lain angka yang cukup tinggi dari orang dengan gangguan jiwa (ODGJ) diikuti beban ekonomi yang tinggi. Penelitian ini bertujuan untuk menganalisis program kesehatan jiwa masyarakat di Puskesmas Tombulu serta hambatan yang dihadapi dalam  pelaksanaan program tersebut. Jenis penelitian ialah deksriptif kualitatif. Data penelitian diperoleh melalui wawancara dengan informan. Hasil penelitian menunjukkan program kesehatan jiwa dilaksanakan melalui program kunjungan rumah (home visit), disertai kegiatan pemeriksaan kesehatan, pemantauan perkembangan pasien serta penyuluhan tentang keterlibatan keluarga dalam proses pengobatan dan penyembuhan ODGJ. Hambatan yang dialami antara lain stigma dari masyarakat terhadap ODGJ sehingga masyarakat dan keluarga kurang terlibat dalam proses penyembuhan ODGJ. Upaya pengembangan program kesehatan jiwa antara lain dengan meningkatkan keterlibatan keluarga dan masyarakat dalam proses perawatan ODGJ, menambah jumlah kader kesehatan, dan meningkatkan upaya promosi kesehatan jiwa. Simpulan penelitian ini ialah pelaksanaan program kesehatan jiwa di Puskesmas Tombulu dilakukan melalui kegiatan kunjungan rumah (home visit), program deteksi dini, serta skrining untuk mengenali masalah kesehatan jiwa baik bagi masyarakat maupun tenaga kesehatan. Hambatan meliputi tidak adanya dukungan keluarga, stigma negatif dari masyarakat, kurangnya sarana dan prasarana pendukung untuk pasien ODGJ, dan letak rumah sakit yang cukup jauh. Abstract

Abstract: Patient safety is now a top priority for hospitals. This study aimed to analyze patient safety incident reporting at inpatient unit of  Siloam Hospitals Manado. This was a qualitative study using eight informants. The results showed that patient safety incident reporting at the hospital inpatient unit was carried out using the Q-Pulse application. The incident register on the incident report form in the Q-Pulse application was a maximum of 24 hours after the incident occurred or the risk was known. The inhibiting factors or obstacles in patient safety incident reporting was the slow performance of accessing the Q-Pulse application, the busyness of staff or employees with their main duties, and employees who were still afraid to report due to be blamed. Efforts to overcome obstacles in patient safety incident reporting were inter alia refreshing the internet network, adding signal boosters or wifi boosters which will be coordinated with the Siloam Head Office. In addition, Siloam Hospitals Mando planned to migrate from the Q-Pulse application where the function was still the same but used a different platform, namely the website-based Medblaze. In conclusion, patient safety incident reporting at the Siloam Hospitals Manado inpatient unit was carried out using the Q-Pulse application with a maximum of 24 hours after the incident occurred or the risk was known. There were varied inhibiting factors or obstacles in the patient safety incident reporting and several efforts had been done to overcome them. Keywords: patient safety; safety incidents; incident reporting   Abstrak: Keselamatan pasien kini menjadi prioritas utama bagi rumah sakit. Penelitian ini bertujuan untuk menganalisis pelaporan insiden keselamatan pasien di unit rawat inap Siloam Hospitals Manado. Penelitian ini menggunakan metode kualitatif dengan delapan orang informan. Hasil penelitian mendapatkan bahwa pelaporan insiden keselamatan pasien di unit rawat inap Rumah Sakit menggunakan aplikasi Q-Pulse. Register insiden pada formulir laporan insiden di aplikasi Q-Pulse maksimal 24 jam setelah insiden terjadi atau risiko diketahui. Faktor penghambat atau kendala dalam pelaporan insiden keselamatan pasien di unit rawat inap yaitu performa yang lambat terhadap akses aplikasi Q-Pulse, kesibukan para staf atau karyawan dengan tugas pokoknya dan karyawan yang masih merasa takut melaporkan karena akan disalahkan. Upaya yang dilakukan untuk mengatasi kendala dalam pelaporan insiden keselamatan pasien di unit rawat inap ialah melakukan refreshment jaringan internet, menambah penguat signal atau penguat wifi yang akan dikoordinasikan bersama Siloam Head Office. Simpulan penelitian ini ialah pelaporan insiden keselamatan pasien di unit rawat inap Siloam Hospitals Manado dilakukan menggunakan aplikasi Q-Pulse maksimal 24 jam setelah insiden terjadi atau risiko diketahui.
