Abstract

Background: Hand washing is among the most effective ways to prevent diseases. In Indonesia, only a quarter of the entire population practice proper handwashing techniques. Of these, children are the most vulnerable group for contracting diseases. Nevertheless, they also are crucial agent for behavior transformation as they are keen and open to new ideas. Therefore, this study was conducted to determine if training would have an improvement on a proper hand washing practices among elementary school students.Methods:An observational descriptive study design using random sampling was conducted from September to November 2013 in Jatinangor Subdistrict, West Java, Indonesia using primary data of one hundred elementary school students from four elementary schools. Questionnaires were given after informed consent. A demonstration on hand washing techniques and education on proper hand washing practices was then given. Two weeks later, the same questionnaire was given to measure the influence of the training. The collected data were presented using frequency tabulation.Results: Before the training on proper hand washing practices was conducted, only 86.9% students were practicing it properly. After the training was given, 90.7% of the students were doing it properly. For the hand washing technique, only 66.8% of students knew the correct steps before the intervention was given and 78.7% students did them correctly after the intervention.Conclusions:The training shows an overall improvement on the students’ hand washing practices. DOI: 10.15850/amj.v4n2.1073

Abstract

Background: Severe sepsis and septic shock are at high mortality rate. This high mortality persists as important aspect in the term of patient treatment which take account in determining aggressiveness of relevant therapy. Lactate level consideration was considered important among patient under shock, septicemia, post-operative, acute lung injury, and critical condition. Lactate concentration in static blood was widely studied and suggested as prognostic value among severe sepsis and septic shock patient due to the nature of lactate as the result of anaerobic metabolism. Several study documented the use of lactate as prognostic indicator for shock condition. The increase of lactate concentration could be useful as the indicator of inadequate oxygen delivery and the existence of anaerobic metabolism. Lactate clearance investigation is more superior therapeutic target compared with others oxygen derivate variables. Methods: This research is a cohort observational study involving secondary data which was collected from laboratory examination results of study subjects. The research held from August 2015 to December 2015 in Digestive Division, Surgery Department, Hasan Sadikin Hospital. 42 patient involved in this study. Results: Comparative test results revealed significant lactate clearance based on mortality in LCH (6) (p = 0.000) and H (24) (p =0.000) as well. The level of LC H (6) and H (24) seemed lower in died patient compared with life patient. Conclusion: This study concluded that lactate clearance in died patient was lower in comparison with life patient.

Abstract

Background. For decades, ​​colorectal surgical procedures have been associated with high postoperative infections complication. This is expected because of the large number of bacteria in the colon and rectum content. The aim of this study was to investigate wether preoperative oral kanamycin can reduce colonic bacterial concentration, furthermore reduce postoperative infection complication in colorectal malignancy patients who will be performed elective surgical tumor removal. Method. This was an experimental, pre-post test with control group desiagn study, ie groups of patient that prepared with mechanical bowel preparation and administration of oral Kanamycin 3 x 2000 mg a day before the operation day and group of patient that prepared with mechanical bowel preparation only. This study was comparing the concentration of bacteria in the colon and postoperative infection incidence between the two groups. Result. Thirty two patients were randomly assigned to one of two groups, the treatment group and the control group. Obtained equitable distribution of age, sex, tumor location and type of surgery. There was significant bacterial concentration reduction difference between two groups (p = 0.01). There was significant surgical site infection (SSI) incidence difference between two groups, 5 patients (31,3 %) developed an SSI in treatment group and 11 patients (68,8 %) in control group (p=0,03), with OR 0,21 ( CI 95%; 0,046-0,92). Conclusion. Preoperative oral kanamycin could reduce colonic bacteria concentration and SSI incidence in patients with colorectal malignancy who would be performed elective surgical tumor removal. Thus, these methods can be considered its use as a colorectal surgery preparation procedures.

Abstract

Latar Belakang: Perforasi ulkus peptikum sering membutuhkan tindakan operasi dengan faktor risiko yang dapat meningkatkan mortalitas. Beberapa prognostic score mengukur gangguan pada sejumlah faktor fisiologis yang mewakili sistem organ utama dan memungkinkan stratifikasi pasien menurut tingkat keparahan, membantu dalam identifikasi pasien berisiko tinggi dan memberikan informasi prognostik. Sistem penilaian PULP dan Jabalpur mempunyai kemiripan paramater yang dapat dinilai sesegera mungkin saat pasien pertama datang dan perbandingan akurasi keduanya dalam memprediksi kematian pada pasien perforasi ulkus peptikum belum tersedia. Metode: Penelitian merupakan uji diagnostik retrospektif terhadap pasien perforasi ulkus peptikum yang diintervensi operasi. Pasien perforasi gaster diakibatkan trauma dan keganasan, serta operasi lebih satu kali sebagai kriteria eksklusi. Data skor PULP dan Jabalpur didapatkan dari rekam medis, dengan cut off <8 dan ≥8 pada skor PULP serta <9 dan ≥9 pada skor Jabalpur, kemudian hasil perawatan dinilai. Tabulasi silang masing-masing skor terhadap hasil perawatan dengan chi-square dilakukan untuk melihat sensitivitas, spesifisitas, nilai duga positif dan nilai duga negatif masing-masing skor serta dilihat juga signifikansi akurasi masing-masing skor. Hasil: Sebanyak 36 pasien perforasi ulkus peptikum dengan 28 (77.8%) adalah laki-laki dan rerata usia 59.56 tahun. Komorbid ditemukan pada 5 pasien yaitu gagal jantung kelas IV dan PPOK masing-masing sebanyak 2 kasus dan dialisis reguler sebanyak 1 kasus. Angka mortalitas sebesar 16.67% (6 pasien), dengan sensitivitas, spesifisitas, nilai duga positif dan nilai duga negatif Skor PULP sebesar 33.33%; 73.33%; 20%; 84.61% dan Skor Jabalpur sebesar 100%; 60%; 33.33%; 100%. Kedua skor mempunyai akurasi 66.67% dengan p pada Skor PULP sebesar 0.739 dan Skor Jabalpur sebesar 0.007. Subjek dengan skor Jabalpur <9 mempunyai kecenderungan sembuh 1.5 kali dari pasien dengan skor Jabalpur ≥9. Kesimpulan: Skor Jabalpur mempunyai signifikansi keakuratan dalam memprediksi mortalitas pasien perforasi ulkus peptikum jika dibandingkan skor PULP yang tidak signifikan.

Abstract

Latar Belakang: Perdarahan terselubung pada pasien trauma tumpul menjadi penyebab kedua tertinggi kematian. Tidak terdeteksinya cedera abdomen menjadi penyebab morbiditas dan mortalitas lambat pada pasien yang dapat bertahan pada fase-fase awal trauma. Manajemen nonoperatif (NOM) pada trauma organ solid adalah aman dan efektif, dan strategi ini telah menjadi praktek yang digunakan secara luas. Lactate clearance memiliki manfaat klinis yang penting pada pasien dengan trauma akut, yang superior terhadap pemeriksaan inisial laktat. Penelitian ini bertujuan untuk mencari hubungan pemeriksaan nilai lactate clearance pada jam-jam pertama setelah resusitasi terhadap keberhasilan penanganan manajemen nonoperatif. Metode: Bentuk penelitian ini adalah cohort prospektif observasional dengan analisis hubungan lactate clearance 2 jam dan 4 jam dengan keberhasilan NOM pada pasien trauma tumpul abdomen dengan riwayat syok perdarahan di RS. Hasan Sadikin Bandung. Penelitian ini dilakukan pada bulan Agustus 2015 sampai Juli 2016.. Hasil: Terdapat 34 subjek pada studi ini. Terdapat hubungan yang signifikan antara lactate clearance 2 jam (LCD2) dan lactate clearance 4 jam (LC4) dengan kesuksesan NOM (p <0.001). Tidak terdapat perbedaan signifikan antara LC2 dan LC4 dalam menentukan kesuksesan NOM (p>0,05). Kesimpulan: terdapat hubungan yang sangat signifikan antara lactate clearance 2 jam (LC2) maupun lactate clearance 4 jam (LC4) dalam menentukan keberhasilan manajemen non operatif (NOM) pada pasien dengan syok perdarahan yang disebabkan oleh trauma tumpul abdomen.

Abstract

Colorectal cancer is the third most encountered malignancy worldwide in 2018. Some earlier studies indicate a significant influence of vitamin D supplementation on the 5-year survival rate and the rate of metastasis progression in colorectal cancer patients. Studies investigating the effects of vitamin D supplementation on the outcomes of colorectal cancer patients are limited in Indonesia. Therefore, a double-blinded Randomized Controlled Trial (RCT) of the effect of vitamin D supplementation on the outcome of colorectal cancer patients was conducted from April 2022 to March 2023 at the digestive surgery outpatient clinic of Dr. Hasan Sadikin General Hospital Bandung, Indonesia. In this study, 36 patients received vitamin D and 34 patients received a placebo. Data analysis performed using the multivariate analysis with multiple regression revealed no significant relationship between vitamin D supplementation and colorectal cancer patient outcomes. Furthermore, ANOVA analysis indicated no relationship between the analyzed independent and dependent variables in this study.  No relationship was found between vitamin D supplementation and the outcomes of metastasis, mortality, and Karnofsky scores in colorectal cancer patients. Further research with a larger population is still needed to determine the benefits of vitamin D supplementation on the outcomes of colorectal cancer patients.

Abstract

Background: Rectal cancer is a type of malignancy that occurs in the rectum. One of the many options of therapy for rectal cancer includes radiotherapy. This research was done to learn about the profile of rectal cancer patients undergoing radiation therapy at Dr. Hasan Sadikin General Hospital from 2016 until 2021. Methods:  A retrospective descriptive study that uses data obtained from patient’s medical records from Dr. Hasan Sadikin General Hospital from 2016 until 2021. The sampling technique used is total sampling from 109 data of rectal cancer patients who underwent radiation therapy in Dr. Hasan Sadikin from 2016 until 2021 Results: : From all 109 rectal cancer patients undergoing radiotherapy, the most often patients found have the age group of 40-49 years old (28.4%), females (50.5%), are mostly found from Priangan area (65.1%), have the chief complaint of a problem with defecating (51.3%), at stage III (36.7%), and have previously received operative management from their previous institutions (42.2%). The patients are mostly undergoing radiotherapy for curative indications (68.8%) and most have a waiting time of more than six weeks before obtaining radiotherapy (61.4%). Conclusions:  Cases of rectal cancer in radiotherapy patients in Dr. Hasan Sadikin General Hospital from the year 2016 to 2021 are seldom compared to other types of cancer and mostly occur in patients aged 40 to 49 years old. Most of the patients come for a curative reason and must wait for more than six weeks before obtaining radiation therapy. Further research may be performed to find the most effective method of giving curative radiotherapy as well as the time given in the hope of making radiotherapy planning more effective in the future.
