Abstract

Introduction: RPP is used as a marker of cardiac function. RPP reflects the function of the heart in fulfilling oxygen demand stably and continuously during activity. Slow decline of HRR is associated with risk factors for atherosclerosis, similar to hypertension and dyslipidaemia There has not been much research on HRR and RPP in coronary heart disease. The aim of this study is to examine the relationship between RPP and syntax score in stable coronary heart disease. Methods: This study is an observational analytic study with a cross sectional design to determine the relationship between HRR and RPP to the severity of coronary lesions using the syntax score in stable angina pectoris patients. Sampling was carried out at the Haji Adam Malik Hospital Medan from February. HRR and RPP scores were obtained from cardiac exercise testing and syntax scores were obtained from coronary angiography after validation by an interventionist. EF data were taken from echocardiography results validated by consultant echocardiographers. Statistical data analysis using computer statistical tools, p value <0.05 was considered statistically significant. Results: From angiographic data in samples with syntax score ≥ 23, there were 23 (100%) LAD, 21 (91.3%) LCx, and 22 (95.7%) RCA involvement. In samples with Syntax score < 23, there were single vessel lesions as many as 11 (40.07%), two vessels as many as 12 (44.4%), and three vessels as many as 4 (14.8%). From the test results, there was a significant relationship between HRR and RPP and SYNTAX score with a p value <0.05 (<0.001) in both variables. Conclusion: There was a significant association in HRR scores and RPP scores between groups with SYNTAX scores ≥ 23 and SYNTAX scores < 23.

Abstract

Kebutuhan internet semakin meningkat seiring berkembangnya populasi pengguna internet. Saat ini jaringan internet telah masuk ke pelosok desa yang ada di seluruh negara Indonesia. Jaringan tersebut berupa jaringan RT RW net yang dikelola oleh penyedia jasa internet kecil. Menurut badan pusat statistik kabupaten cirebon terdapat 2740 RW yang tersebar di 412 kecamatan, salah satu penyedia jasa internet kecil adalah TN.Net yang bermitra pada Internet Service Provider (ISP) yaitu Akses Bersama Sedaya (ABS) dan ISP sebagai backup menggunakan kartu GSM. TN. Net menggunakan 2 internet service provider (ISP) agar 2 ISP dapat dimanfaatkan dengan optimal maka perlu diterapkan metode load balancing. Ada banyak metode untuk menerapkan load balancing pada penelitian ini peneliti membandingkan 2 metode penerapan load balancing yaitu Per Connection Classifier (PCC) dan NTH. Ada beberapa tahapan untuk menggunakan metode load balancing yaitu tahap pertama peniliti melakukan penerapan load balancing yaitu Per Connection Classifier (PCC) dan NTH, tahap kedua peneliti melakukan rekaman hasil penerapan 2 metode menggunakan wireshark, tahap ketiga peneliti melakukan perhitungan Quality of service dari 2 metode dan tahap keempat peneliti mengambil kesimpulan dari hasil perhitungan mana metode yang terbaik. Hasil yang terbaik dari pengambilan nilai rata-rata antara metode PCC dan Nth yaitu untuk metode terbaik PCC dengan nilai throughput 2,216, packet loss 0%, delay 3.163, jitter 3.329

Abstract

Al-Jarh wat Ta’dil is an important field in hadith studies that provides information about the conditions of hadith narrators based on critiques by critical scholars. These critiques follow a hierarchical structure, with shaduq being a level two endorsement after tsiqat. Despite being an endorsement, a narration from a shaduq narrator is not accepted outright and requires further scrutiny. This research analyzes the acceptance status of narrations from shaduq narrators according to critical scholars. It critically examines the case of the shaduq narrator named ar-Rabi bin Anas and investigates the reception status of his narrations. The methodology employed is qualitative with a descriptive-analytical approach, using references such as al-Jarh wat-Ta’dil literature, mustalah hadith, hadith collections, relevant books, and journals. The study demonstrates that among shaduq narrators, some receive negative critiques leading to partial rejection of their narrations. Furthermore, it shows that narrations from shaduq like ar-Rabi bin Anas are rejected if: First, they have negative critiques from scholars supported by indications after comparison with parallel narrations. Second, their hadiths are transmitted by problematic disciples; in ar-Rabi's case, his problematic disciple is Abu Ja’far ar-Razi, and the narration by this disciple occurs in isolation.

Abstract

Introduction: Coronary artery calcium score (CACS) is a specific indicator of coronary atherosclerosis that plays a role in assessing the degree of calcification in atherosclerosis. Diastolic function is the first aspect of cardiac function to be impaired in ischemic heart disease. This study aims to determine the relationship between calcium scoring and diastolic dysfunction. Methods: This analytical observational study with cross-sectional design evaluated the relationship between coronary artery calcium score (CACS) and left ventricular diastolic function in patients with stable CAD. Data were collected retrospectively from medical records at RSUP H. Adam Malik Medan during Nov 2023-Nov 2024. CACS was assessed using coronary CT scan, while left ventricular diastolic function was measured by echocardiography. Data analysis used chi-square test, Mann-Whitney U test, and ROC curve analysis to evaluate CACS threshold in predicting diastolic dysfunction. Results: Among 158 analyzed samples, 113 patients had diastolic dysfunction. A calcium score ≥100 was found in 46.2% of patients, showing 1.318 times higher risk of diastolic dysfunction versus those with scores <100 (p = 0.006; 95% CI 1.083–1.605). ROC analysis showed CACS had moderate predictive ability for diastolic dysfunction with AUC of 0.647 (p = 0.004). A calcium score threshold of 45 had 65.5% sensitivity and 62.2% specificity in detecting diastolic dysfunction. Type 2 diabetes mellitus, urea, and creatinine levels were also significantly associated with diastolic dysfunction (p < 0.05). Conclusion: Calcium score shows a significant relationship with diastolic dysfunction in stable CAD patients and can predict diastolic dysfunction in patients undergoing coronary CT scan.
