Abstract

Introduction: RPP is used as a marker of cardiac function. RPP reflects the function of the heart in fulfilling oxygen demand stably and continuously during activity. Slow decline of HRR is associated with risk factors for atherosclerosis, similar to hypertension and dyslipidaemia There has not been much research on HRR and RPP in coronary heart disease. The aim of this study is to examine the relationship between RPP and syntax score in stable coronary heart disease. Methods: This study is an observational analytic study with a cross sectional design to determine the relationship between HRR and RPP to the severity of coronary lesions using the syntax score in stable angina pectoris patients. Sampling was carried out at the Haji Adam Malik Hospital Medan from February. HRR and RPP scores were obtained from cardiac exercise testing and syntax scores were obtained from coronary angiography after validation by an interventionist. EF data were taken from echocardiography results validated by consultant echocardiographers. Statistical data analysis using computer statistical tools, p value <0.05 was considered statistically significant. Results: From angiographic data in samples with syntax score ≥ 23, there were 23 (100%) LAD, 21 (91.3%) LCx, and 22 (95.7%) RCA involvement. In samples with Syntax score < 23, there were single vessel lesions as many as 11 (40.07%), two vessels as many as 12 (44.4%), and three vessels as many as 4 (14.8%). From the test results, there was a significant relationship between HRR and RPP and SYNTAX score with a p value <0.05 (<0.001) in both variables. Conclusion: There was a significant association in HRR scores and RPP scores between groups with SYNTAX scores ≥ 23 and SYNTAX scores < 23.

Abstract

Kebutuhan internet semakin meningkat seiring berkembangnya populasi pengguna internet. Saat ini jaringan internet telah masuk ke pelosok desa yang ada di seluruh negara Indonesia. Jaringan tersebut berupa jaringan RT RW net yang dikelola oleh penyedia jasa internet kecil. Menurut badan pusat statistik kabupaten cirebon terdapat 2740 RW yang tersebar di 412 kecamatan, salah satu penyedia jasa internet kecil adalah TN.Net yang bermitra pada Internet Service Provider (ISP) yaitu Akses Bersama Sedaya (ABS) dan ISP sebagai backup menggunakan kartu GSM. TN. Net menggunakan 2 internet service provider (ISP) agar 2 ISP dapat dimanfaatkan dengan optimal maka perlu diterapkan metode load balancing. Ada banyak metode untuk menerapkan load balancing pada penelitian ini peneliti membandingkan 2 metode penerapan load balancing yaitu Per Connection Classifier (PCC) dan NTH. Ada beberapa tahapan untuk menggunakan metode load balancing yaitu tahap pertama peniliti melakukan penerapan load balancing yaitu Per Connection Classifier (PCC) dan NTH, tahap kedua peneliti melakukan rekaman hasil penerapan 2 metode menggunakan wireshark, tahap ketiga peneliti melakukan perhitungan Quality of service dari 2 metode dan tahap keempat peneliti mengambil kesimpulan dari hasil perhitungan mana metode yang terbaik. Hasil yang terbaik dari pengambilan nilai rata-rata antara metode PCC dan Nth yaitu untuk metode terbaik PCC dengan nilai throughput 2,216, packet loss 0%, delay 3.163, jitter 3.329

Abstract

Al-Jarh wat Ta’dil is an important field in hadith studies that provides information about the conditions of hadith narrators based on critiques by critical scholars. These critiques follow a hierarchical structure, with shaduq being a level two endorsement after tsiqat. Despite being an endorsement, a narration from a shaduq narrator is not accepted outright and requires further scrutiny. This research analyzes the acceptance status of narrations from shaduq narrators according to critical scholars. It critically examines the case of the shaduq narrator named ar-Rabi bin Anas and investigates the reception status of his narrations. The methodology employed is qualitative with a descriptive-analytical approach, using references such as al-Jarh wat-Ta’dil literature, mustalah hadith, hadith collections, relevant books, and journals. The study demonstrates that among shaduq narrators, some receive negative critiques leading to partial rejection of their narrations. Furthermore, it shows that narrations from shaduq like ar-Rabi bin Anas are rejected if: First, they have negative critiques from scholars supported by indications after comparison with parallel narrations. Second, their hadiths are transmitted by problematic disciples; in ar-Rabi's case, his problematic disciple is Abu Ja’far ar-Razi, and the narration by this disciple occurs in isolation.

Abstract

Introduction: Coronary artery calcium score (CACS) is a specific indicator of coronary atherosclerosis that plays a role in assessing the degree of calcification in atherosclerosis. Diastolic function is the first aspect of cardiac function to be impaired in ischemic heart disease. This study aims to determine the relationship between calcium scoring and diastolic dysfunction. Methods: This analytical observational study with cross-sectional design evaluated the relationship between coronary artery calcium score (CACS) and left ventricular diastolic function in patients with stable CAD. Data were collected retrospectively from medical records at RSUP H. Adam Malik Medan during Nov 2023-Nov 2024. CACS was assessed using coronary CT scan, while left ventricular diastolic function was measured by echocardiography. Data analysis used chi-square test, Mann-Whitney U test, and ROC curve analysis to evaluate CACS threshold in predicting diastolic dysfunction. Results: Among 158 analyzed samples, 113 patients had diastolic dysfunction. A calcium score ≥100 was found in 46.2% of patients, showing 1.318 times higher risk of diastolic dysfunction versus those with scores <100 (p = 0.006; 95% CI 1.083–1.605). ROC analysis showed CACS had moderate predictive ability for diastolic dysfunction with AUC of 0.647 (p = 0.004). A calcium score threshold of 45 had 65.5% sensitivity and 62.2% specificity in detecting diastolic dysfunction. Type 2 diabetes mellitus, urea, and creatinine levels were also significantly associated with diastolic dysfunction (p < 0.05). Conclusion: Calcium score shows a significant relationship with diastolic dysfunction in stable CAD patients and can predict diastolic dysfunction in patients undergoing coronary CT scan.

Abstract

Perkembangan  bisnis  di  Indonesia  pada  tahun  2020  sangatkan  berkembang. Salah satu sektor bisnis yang mengalami peningkatan yang signifikan adalah e- commerce. Dari  beberapa  perusahaan  yang  berstatus  unicorn  di  Indonesia,  terdapat  beberapa  e-commerce. Salah satunya Blibli.com yang menerapkan konsep online shopping mall atau menjual  semua  jenis  produk  untuk memenuhi  kebutuhan  sehari  hari  dan  gaya  hidup. Tentunya  di  balik  kesuksesan  Blibli,  terdapat  orang  orang  hebat  di  dalamnya,  seperti karyawannya yang tentunya bekerja keras untuk menggapai cita cita perusahaan, serta menerapkan  value  dari  Blibli.com  yaitu  RESPECT.  Blibli  ingin  nilai  tersebut  dapat  di implementasikan dalam situasi atau platform apapun, terutama seluruh karyawan yang bekerja  di dalamnya baik  senior  ataupun  baru.  Karywan baru  juga menjadi  salah  satu sasaran yang perlu mengetahui nilai RESPECT tersebut. Salah satu caranya dengan cara mensosialisasikan  pada  saat  orientasi  perusahaan.  Selain  mensosialisakan  melalui orientasi,  pemberian  welcome  kit  juga  menjadi  hal  tahapan  penting  yang  perlu  di dapatkan oleh karyawan baru. Maka dari itu peneliti melihat adanya potensi perancangan welcome  kit  untuk  karyawan  Blibli.com.  Perancangan  ini  menggunakan  metode kuantitatif  dengan  cara  wawancara  dan  pengumpulan  data  eksisting  yang  di  dapat langsung  dari  Blibli.com.  Perancangan  ini  diharapkan  dapat  memenuhi  kebutuhan karyawan baik secara visual dan keperluan. Kata Kunci: Blibli, nilai perusahaan, RESPECT, welcome kit.

Abstract

Implementasi hukum perlindungan konsumen dalam pelayanan kesehatan di rumah sakit di Indonesia dijalankan terutama melalui Undang-Undang Nomor 8 Tahun 1999 tentang Perlindungan Konsumen (UUPK), yang menegaskan bahwa pasien merupakan konsumen yang berhak menerima pelayanan kesehatan yang aman, bermutu, serta memperoleh informasi yang lengkap dan jujur mengenai kondisi medis maupun tindakan yang akan dilakukan. UUPK juga memberikan jaminan bahwa pasien berhak menuntut ganti rugi apabila mengalami kerugian akibat kelalaian medis, termasuk tindakan malpraktik. Rumah sakit sebagai pelaku usaha berkewajiban memenuhi standar pelayanan, memastikan prosedur informed consent dilaksanakan oleh dokter, memberikan akses terhadap rekam medis, dan menyediakan mekanisme kompensasi sebagaimana diatur dalam Pasal 19 UUPK serta diperkuat oleh ketentuan dalam Undang-Undang Nomor 17 Tahun 2023 tentang Kesehatan. Namun, implementasi di lapangan masih menghadapi berbagai kendala. Banyak pasien belum memahami hak-haknya, sehingga sering kali tidak mempertanyakan prosedur atau tindakan medis yang diberikan. Beban kerja rumah sakit yang tinggi menyebabkan proses informed consent dilakukan terburu-buru dan tidak mendalam. Selain itu, dominasi tenaga perawat dalam kegiatan administratif terkadang membuat peran dokter dalam komunikasi medis menjadi kurang optimal. Rumah sakit juga kerap kesulitan memenuhi standar karena keterbatasan sumber daya dan lemahnya komunikasi internal. Meski mekanisme pengaduan serta gugatan perdata maupun pidana telah tersedia melalui Pasal 45 UUPK, penguatan pelaksanaan UUPK tetap diperlukan. Upaya perbaikan dapat dilakukan melalui sosialisasi hak pasien, peningkatan kualitas komunikasi dokter-pasien, pemanfaatan teknologi rekam medis untuk meningkatkan transparansi, serta penerapan pendekatan keadilan restoratif yang didukung oleh majelis disiplin profesi untuk mendorong peningkatan mutu pelayanan kesehatan secara berkelanjutan.

Abstract

Health law is a field that has a strategic position in ensuring the protection of human rights, especially in the context of modern health services that continue to develop. The development of medical technology, improving the quality of services, and strengthening the rights of patients have increasingly complex ethical, moral, and regulatory implications. This study aims to analyze health law from the perspective of legal philosophy by reviewing the concepts of values, principles of justice, equality, and morality that are the foundation of its regulation. Through a theoretical approach, this study discusses how health law is positioned as a system of norms that protect patients, direct the practice of health workers, and balance the relationship between professional obligations and the interests of society. In addition, the analysis is focused on the application of health law in daily life, including service ethics, patient rights, medical confidentiality, and legal protection mechanisms through regulations such as the Health Law No. 17 of 2023. The results of the study show that strengthening professional ethics, the integrity of health workers, and the legal system that favors patient safety is the key to improving the quality of health services. Thus, health law not only functions as a regulatory instrument, but also as a moral and philosophical pillar in maintaining human dignity and the quality of health services.
