Abstract

This research was conducted in an international company engaged in iron and steel products manufacturing industries. One of the equipment that is often damaged is a hot roller table machine in the furnace section mill unit. The availability results obtained in hot roller table equipment is 96.571% and is still below the company standard which is set at 98%. Therefore, we need an analysis of the root causes of the problem and search for the best solution to fix the existing problem by applying the method of Failure Mode and Effect Analysis (FMEA). FMEA is a method that can systematically and structurally analyze and identify the consequences of a system or process failure, and also reduce or analyze the probability of failure. The purpose of this study is to identify and analyze the level of damage and its causes with the application of the FMEA method. Based on the pareto diagram the damage to the hot roller table machine, it was found that the highest frequency of damage was in the rotary coupling with a down time percentage of 26.9%. From the FMEA Analysis, two components that have very high RPN values are categorized as potential severit i.e. bearing as the first with an RPN value of 392 and the second is a seal ring with an RPN value of 294. The two components are the main priority for repair of the furnace section. mill, especially for machine and human aspects.

Abstract

Deaths and injuries from traffic accidents have become health problems for people throughout the world including Indonesia. The saddest data from the victims who died due to traffic accidents found that 10,428 people were killed in 2017 because the drivers did not use seat belts. This research aims to see the relationship between the perception of safety risk i.e. the ability, knowledge and environmental factors with the behavior of the use of safety belts in truck drivers in mining companies. This research is a comparative causal research i.e. research that states the relationship of one variable causes other variables. What is affected is the dependent variable, namely the use of safety belt behavior and the influencing variable is the independent variable, namely the perception of the risk of driving safety. Participants are 25 mining company truck drivers. The data obtained is then processed and analyzed using the SPSS version 16. Based on the results of data processing and analysis, it is found that the ability, knowledge and work environment factors have an influence on the safety belt usage behavior.

Abstract

This study aims to maximize the decline in the level of product defects to lead to zero defects. Researchers use the six sigma method through 5 stages of completion, namely, define, measure, analyze, improve, and control. Based on the results of the study it can be concluded that the sigma level and the value of DPMO (Defect per Million Opportunities) of the company amounted to 3.47 with possible damage of 24,239 sheets for one million productions. After further analysis using the Pareto diagram, it found that the cutting fault transfer defect was the most common with a percentage of damage of 43%. And based on the results of the analysis of fishbone diagrams, it can be seen that the main factors causing contamination in the production process come from the worker factor (human), followed by machine factors, material, and work methods. The proposed improvements that can be made by the company include updating existing methods and SOPs (standard operating procedures) and monitoring the operator's performance and developing knowledge and planning proper maintenance scheduling.

Abstract

This study aims to determine the value of the risk of work hazard potential and the level of risk of potential occupational hazards in the blast furnace division of hot metal treatment plant which is currently in the commissioning stage. This study uses an approach with the HIRA (Hazard Identification and Risk Assessment) method to determine the value of the risk of potential occupational hazards and the level of risk of potential occupational hazards. The results obtained by using HIRA (Hazard Identification and Risk Assessment) obtained 26 risks classified by potential hazards, namely nine risks included in the high category (H), seven risks moderate category (M) and the remaining ten risks involved in the low category ( L). The recommendation that can give is to carry out detailed mitigation of the risk of high (H) and moderate (M) categories until the risk level falls to the risk of the low (L) category before the blast furnace plant is fully operating.

Abstract

Abstract— This study aims to determine the risk value of potential occupational hazards and the level of potential risk of occupational hazards and to determine the potential occupational hazards that can cause occupational accidents at PT. HPP which is engaged in consulting services as well as implementation in the field of hydraulic of heavy equipment. This study uses an approach with the HIRA (Hazard Identification and Risk Assessment) method to calculate the values of risk before there is control (basic risk) until after controlling for the risk (existing risk). The results of research on the work process activities at the hydraulic system facilities in heavy equipment of PT. HPP found that 33 basic risks consisting of acceptable categories of 3 risks (9.09%), priority 3 categories of 8 risks (24.24%), substantial categories (priority 2) of 8 risks (24, 24%), priority 1 category was 5 risks (15.15%) and very high category was 9 risks (27.27%). After controlling for the existing risk, there is a reduction in risk, namely acceptable category as much as 3 risks (9.09%), priority category as much as 8 risks (24.24%), substantial category (priority 2) as much as 8 risks (24.24%), priority 1 category 5 risks (15.15%) and very high category 9 risks (27.27%).Keywords— basic risk, existing risk, HIRA. Abstrak— Penelitian ini memiliki tujuan untuk mengetahui nilai risiko potensi bahaya kerja dan level risiko potensi bahaya kerja serta mengetahui potensi bahaya kerja dominan yang dapat menyebabkan terjadinya kecelakaan kerja di PT. HPP yang bergerak di bidang jasa konsultasi maupun pelaksanaan di bidang peralatan hydraulic alat berat (heavy equipment). Penelitian ini menggunakan pendekatan dengan metode HIRA (Hazard Identification and Risk Assesment) untuk menghitung nilai-nilai risiko sebelum ada pengendalian (basic risk) sampai setelah dilakukan pengendalian terhadap risiko (existing risk). Hasil penelitian dari kegiatan proses kerja pada fasilitas system hydraulic di peralatan berat (heavy equipment)  PT. HPP ini didapatkan 33 basic risk  yang terdiri dari kategori dapat diterima (acceptable) sebanyak 3 risiko (9,09%), kategori priority 3 sebanyak 8 risiko (24,24%), kategori substantial (priority 2) sebanyak 8 risiko (24,24%), kategori priority 1 sebanyak 5 risiko (15,15%) dan kategori very high  sebanyak 9 risiko (27,27%). Setelah dilakukan pengendalian terhadap risiko (existing risk) didapatkan penurunan resiko yaitu kategori yang dapat diterima (acceptable) sebanyak 3 risiko (9,09%), kategori priority 3 sebanyak 8 risiko (24,24%), kategori substantial (priority 2) sebanyak 8 risiko (24,24%), kategori priority 1 sebanyak 5 risiko (15,15%) dan kategori very high  sebanyak 9 risiko (27,27%).Kata kunci— HIRA, risiko dasar, risiko yang ada.

Abstract

Abstract— PT. ABC is one of the national steel industry companies that has a steel production capacity of three million tons more steel per year and the crane is one of the main equipment used at PT. ABC. Crane has a function as a lifting equipment used to move an object or item from one place to another and the type of crane used at PT. ABC is a type of overhead crane. The use of overhead cranes has various potential hazards both during the routine and non-routine lifting process so that the use of overhead cranes has a very high risk of work accidents. From accident data on overhead crane operations at PT. ABC, there were 27 accident cases from 2016 - 2018 that showed the high level of work accidents that occurred. The SWIFT (Structural What If Analysis) method is a technique for identifying hazards by asking what if (what if) keywords. The purpose of this study was to determine the dangers and risks of overhead crane operations at PT. ABC and provide recommendations for improvements to reduce the level of risk of work accidents. Analysis of the worksheet using the SWIFT (structural what if analysis) method produces 18 potential hazards in overhead crane operations and based on RRN (Risk Rating Number) calculations, there are 9 main priority hazards, 5 medium priority hazards, 2 low priority hazards and 2 priority hazards the lowest. Improvement recommendations are made based on the level of risk with the highest priority to prevent and reduce the risk of work accident in overhead crane operations.Keywords— Overhead crane, Potential hazard, Risk Rating Number, SWIFT. Abstrak— PT. ABC merupakan salah satu perusahaan industri baja nasional yang mempunyai kapasitas produksi baja sebesar tiga juta ton lebih baja per tahunnya dan crane merupakan salah satu peralatan utama yang digunakan di PT. ABC. Crane memiliki fungsi sebagai pesawat angkat angkut yang digunakan untuk memindahkan suatu benda atau barang dari tempat yang satu ke tempat lainnya dan jenis crane yang digunakan di PT. ABC adalah jenis overhead crane. Penggunaan overhead crane mempunyai berbagai potensi bahaya baik itu saat proses pengangkatan yang bersifat rutin maupun non rutin sehingga penggunaan overhead crane memiliki risiko kecelakaan kerja yang sangat tinggi. Dari data kecelakaan pada pengoperasian overhead crane di PT. ABC terdapat 27 kasus kecelakaan dari tahun 2016 - 2018 yang menunjukkan masih tingginya tingkat kecelakaan kerja yang terjadi. Metode SWIFT (Structural What If Analysis) adalah suatu teknik untuk mengidentifikasi bahaya dengan pendekatan bertanya menggunakan kata kunci what if (bagaimana jika). Tujuan penelitian ini untuk mengetahui bahaya dan risiko pada pekerjaan pengoperasian overhead crane di PT. ABC serta memberikan rekomendasi perbaikan untuk mengurangi tingkat risiko kecelakaan kerja. Analisa lembar kerja menggunakan Metode SWIFT (structural what if analysis) menghasilkan 18 potensi bahaya pada pekerjaan pengoperasian overhead crane dan berdasarkan perhitungan RRN (Risk Rating Number), terdapat 9 bahaya prioritas utama, 5 bahaya prioritas menengah, 2 bahaya prioritas rendah dan 2 bahaya prioritas paling rendah. Rekomendasi perbaikan dibuat berdasarkan tingkat risiko dengan prioritas utama untuk mencegah dan mengurangi tingkat risiko kecelakan kerja pada pengoperasian overhead crane.Kata kunci— Overhead crane, Potensi bahaya, Risk Rating Number, SWIFT.

Abstract

Abstract— PT. XYZ is one of the large and multinational companies engaged in steel production in Indonesia and in the first half of 2019 PT. XYZ faces a challenge that is the many employees in the sinter plant do not implement the 5R culture. Sinter plant is the main supplier plant for the blast furnace plant at PT. XYZ which functions to combine a raw material mixture at high temperatures to produce a product known as sinter ore. This research was conducted at PT. XYZ. The data obtained was then processed and analyzed using the SPSS version 26 program. Based on the results of data processing and analysis, it was found that all indicators used in this study had good validity and reliability. The results of data processing and analysis in this study also found that 5R culture (X1) and employee performance (X2) had an influence on the work environment (Y).Keywords— 5R Culture, Employee Performance, Work Environment. Abstrak— PT. XYZ adalah salah satu perusahaan besar dan multinasional yang bergerak di bidang produksi baja di Indonesia dan pada semester pertama tahun 2019 PT. XYZ menghadapi tantangan yaitu banyaknya karyawan di sinter plant yang tidak melaksanakan budaya 5R. Sinter plant merupakan plant pemasok utama untuk blast furnace plant di PT. XYZ yang berfungsi untuk menggabungkan suatu raw material mixture pada temperatur tinggi sehingga menghasilkan produk yang dikenal dengan nama sinter ore. Penelitian ini dilakukan di sinter plant PT. XYZ. Data yang didapat kemudian diolah dan dianalisis menggunakan program SPSS versi 26. Berdasarkan hasil pengolahan data dan analisa tersebut didapat bahwa  semua indikator-indikator yang digunakan dalam penelitian ini memiliki keabsahan (validitas) dan keandalan (reliabilitas) yang baik. Hasil pengolahan data dan analisa dalam penelitian ini juga mendapatkan bahwa budaya 5R (X1) dan kinerja karyawan (X2) mempunyai pengaruh terhadap lingkungan kerja (Y).Kata kunci— Budaya 5R, Kinerja Karyawan, Lingkungan Kerja.Â
