Abstract

Aim: To estimate the incidence of VTE in Indonesian patients undergoing major orthopedic surgery and not receiving thromboprophylaxis.Methods: This was an open clinical study of consecutive Indonesian patients undergoing major orthopedic surgery, conducted in 3 centers in Jakarta. Bilateral venography was performed between days 5 and 8 after surgery to detect the asymptomatic and to confi rm the symptomatic VTE. These patients were followed up to one month after surgery.Results: A total of 17 eligible patients were studied, which a median age of 69 years and 76.5% were females. Sixteen out of the 17 patients (94.1%) underwent hip fracture surgery (HFS). The median time from injury to surgery was 23 days (range 2 to 197 days), the median duration of surgery was 90 minutes (range 60 to 255 minutes), and the median duration of immobilization was 3 days (range 1 to 44 days). Thirteen out of the 17 patients were willing to undergo contrast venography. A symptomatic VTE was found in 9 patients (69.2%) at hospital discharge. Symptomatic VTE was found in 3 patients (23.1%), all corresponding to clinical signs of DVT and none with clinical sign of PE. These patients were treated initially with a low molecular weight heparin, followed by warfarin. Sudden death did not occur up to hospital discharge. From hospital discharge until 1-month follow-up, there were no additional cases of symptomatic VTE. No sudden death, bleeding complication, nor re-hospitalization was found in the present study.Conclusion: The incidence of asymptomatic (69.2%) and symptomatic (23.1%) VTE after major orthopedic surgery without thromboprophylaxis in Indonesian patients (SMART and AIDA), and still higher than the results of the Western studies. A larger study is required to establish the true incidence, and more importantly, that the use of thromboprophylaxis in these patients is warranted. Abstract

Background: The purposes of this study were (1) to describe the characteristics of patients underwent TKR during COVID-19 Pandemic and (2) to compare the demographic of patients underwent TKR before and after COVID-19 Pandemic in St. Carolus Hospital, Jakarta, Indonesia.Methods: Data were extracted from January 2016 to December 2020 from the annual reports of arthroplasty registry (single surgeon). Patient demographic data were collected (Gender, age, comorbidity). Post-operative characteristics were defined as: (1) Length of hospitalization (days); (2) Complication rate (%); (3) Revision event (%). The complication rate included: (1) Deep vein thrombosis; (2) Prosthetic joint infection; (3) Periprosthetic fracture. The above parameters were compared before and after the COVID-19 Pandemic.Result: A total of 730 primary TKR procedures (12.3 cases per month) were performed from January 2016 to February 2020, meanwhile there were 54 primary TKR (6 cases per month) done in 9 months after COVID-19 Pandemic from March 2020 to December 2020. The lowest case monthly after the COVID-19 Pandemic was found on April 2020 (1 case per month), on the other hand, the highest case monthly was found on December 2020 (14 cases per month). The complication rate decreased from the Pre COVID-19 Pandemic (8.6% to 1.8%).Conclusion: The number of cases was reduced during Indonesia large-scale social restriction (40 %) and returned to the Pre-Pandemic period on December 2020.Level of Evidence : Descriptive study, level III

Abstract

Background: The outcome for TKA (total knee arthroplasty) upon whether the posterior cruciate ligament is preserved with cruciate retaining (CR) prostheses or sacrificed with posterior stabilized (PS) prostheses are still debated between studies. Materials and Methods: We included a total of 144 knees operated with cemented fixed bearing primary CR or PS TKAs. Independent t-tests were conducted for the outcomes and possible confounding variables between groups where relevant, with analyses using Chi-squared tests for nominal data. Results: Operation on patients with age of lower than 65 years predicts increased intraoperative bleeding volume (p = 0.037), pre-operative range of motion (ROM) of less than 90 degrees was a predictor for better improved post-operative ROM (p 0.001) and PS prostheses is superior to CR in terms of ROM improvement (p = 0.04), however with both groups achieving similar maximum ROM (p = 0.308). Conclusion: Improvement of ROM is increased by the use of PS prostheses compared to CS prostheses, with pre-operative ROM as a possible confounding factor and the maximum ROM achieved for both prostheses being similar. Lower age of the operation is also related to increased intraoperative bleeding
