Abstract

Erythema Multiforme is polymorphous recurrent eruptions on the skin and oral mucosa. This case is describing a progression of a case of Erythema Multiforme in the oral mucosa of a 33-year-old man suggesting of herpes viral involvement. In the absence of identified inducing drugs, past medical history of clinical manifestation on Herpes Simplex Viral (HSV) but positive serum antibody against HSV-1 is suggesting of Herpes Associated Erythema Multiforme. This became the basis of the patient management. Several episodes of recurrences and relapses occurred during our treatment period in oral isolated sites which were later progressed with skin eruption of bullae. The later clinical presentation challenges the on going working diagnosis. It also necessitated a skin biopsy to rule out another possibility of bullous lesion diseases. The latter blood test showed seroconverted of HSV-2 antibody accompanying previous detected HSV-1 antibody. Both biopsy and peripheral blood sera test consistent with Herpes Simplex Virus involvement. The patient is on a scheduled antiviral prophylaxis. This case showed that Herpes Associated Erythema Multiforme (HAEM) does present initially as an isolated oral lesions and a serological test could be used to monitor seroconverting of HSV.

Abstract

Kesehatan gigi dan mulut merupakan hal yang peting dan perlu diperhatikan. Perilaku menjaga kesehatan gigi dan mulut didukung oleh pengetahuan. Hingga saat ini, pengeratuah mengenai kesehatan gigi dan mulut masih minim terutama, pada masyarakat wilayah RW 005, Kelurahan Tanjung Duren Selatan. Berdasarkan permasalahn tersebut, maka akan diadakan Pengabdian Kesehatan Masyarakat di wilayah RW 005, Tanjung Duren Selatan. Tujuan: PkM ini bertujuan untuk meningkatkan kesadaran dan pengetahuan masyarakat di wilayah RW 005, Kelurahan Tanjung Duren Selatan. Metode yang digunakan adalah metode observasional deskriptif. Hasil kegiatan PkM ini, setelah dilakukan penyuluhan dan pengetahuan masyarakat RW 005, Kelurahan Tanjung Duren Selatan mengalami peningkatan. Kesimpulannya adalah pengetahuan masyarakat RW 005 mengenai karies dan penyebabnya, frekuensi menyikat gigi, dan cara mencegah karies mengalami peningkatan pasca penyuluhan kesehatan gigi dan mulut.

Abstract

Background: Oral Allergy Syndrome (OAS) is a localized IgE-mediated food allergy triggered by certain fruits and vegetables in patients with atopic history. This case report describes two siblings with Oral Allergy Syndrome (OAS) presenting as allergic angioedema triggered by specific food allergens.  Case Presentation:  Case 1 involves a 25-year-old female who developed lip swelling, a burning sensation, numbness, multiple oral ulcers on the lower lip, and surrounding redness 30 minutes after consuming mint sauce in an Indian dish. Case 2 details a 21-year-old male who experienced immediate lip and soft palate swelling with numbness after eating Chinese food suspected to contain nuts. Discussion: Both siblings share a history of childhood atopy, predisposing them to IgE-mediated reactions. Their symptoms were characteristic of localized allergic responses in the oral mucosa, consistent with OAS. Case Management: Management involved a combined therapeutic approach: topical corticosteroids were applied to the affected oral areas to reduce local inflammation and promote healing of the mucosal lesions, while oral corticosteroids provided systemic anti-inflammatory effects. Additionally, cetirizine, an H1 receptor antagonist, was administered to block histamine-mediated symptoms such as swelling and itching. Patients were also advised to avoid mint-containing and peanut-containing foods as predisposing factors. Conclusion: This report highlights not only the clinical presentation and successful management of OAS in individuals with familial atopy but also underscores the potential genetic predisposition contributing to food-induced allergic reactions.
