Abstract

Good quantity and quality of bones are keys to the success of prosthodontic treatment. The quantity of bones is showed by the height of mandibular residual ridge, whereas the quality of bones is indicated by the bone density.The aim of this study is to investigate the relationship between the age, the bone density, and the height of the mandibular residual ridge. The observation is carried out to six edentulous menopausal women. The height of mandibular residual ridge and the mandibular bone density are measured by using CBCT-3D (Cone Beam Computed Tomography-3 Dimension). Pearson analysis of bivariate correlation is used to determine the relation between variables. As a result, there is a strong correlation between the increased of the age and the decrease of mandibular bone density (r=-0.922 with α=0.009). The study also shows a significant correlation between the decrease of the height of mandibular residual ridge and the decrease of the mandibular bone density (r=0.815 with α=0.048) and between the decrease of the height of mandibular residual ridge and the increase of the age (r=-0.89 with α=0.017).To summarize, the decrease of the height of mandibular residual ridge in this study correlates with the decrease of the mandibular bone density and the age.

Abstract

Introduction: Infants with complete cleft lip and palate (CLP) exhibit asymmetrical characteristic of nasal deformation. Pre-surgical treatment by making presurgical nasalveolar molding (PNAM) with a nasal stent aims to assist the sucking and swallowing function of infants with CLP as well as passively moving separated alveolar segments and correcting nasal deformities in order to obtain more functional and aesthetically satisfactory surgical results. Objective: This study aimed to analyze the effect of PNAM insertion with a nasal stent on the correction of nasal symmetry in infants with complete unilateral cleft lip and palate. Methods: This research is a quasi experimental with one-group pre-treatment and post-treatment design. Five unilateral complete CLP infants were subjected to anthropometric measurements of alar displacement, columella height, nostril width nostril height and nostril axis inclination to obtain anthropometric ratios on the cleft and non-cleft sides with three-dimensional surface scanning before (T1) and after (T2) insertion of PNAM with nasal stent. Results: Analysis of the students' t-test showed a significant (p-value 0.05) between T1 and T2 in four anthropometric-ratio. Insertion of PNAM with nasal stent has resulted symmetrical nasal morphology of infants with complete unilateral CLP pre labioplasty with an increased ratio of columella height, nostril width, nostril height and nostril axis inclination between the cleft and non-cleft sides. Conclusion: Installation of PNAM with nasal stent can correct pre-labioplasty nasal symmetry.

Abstract

Background: Midline facial cleft is extremely rare congenital malformation. It may be associated with other syndromes, including West syndrome (WS), a severe form of epilepsy in infancy. Objective: This report presents modified Presurgical Nasoalveolar Molding (PNAM) treatment of midline facial cleft in WS patient. Case Report: An infant girl born with midline facial cleft, characterized by flat nose due to complete absence of nasal cartilage, columella, and premaxilla. She was also diagnosed with WS, hydrocephalus, and cerebral palsy. PNAM treatment was performed, consisting of feeding plate and extraoral nasal hook. The feeding plate prevented tongue protrusion which could interfere the airway during multiple spasms and trained her to place the tongue in its normal position. The extraoral strapping was retracted across the upper lip and feeding plate was grinded selectively to approximate the lip and alveolar segment concurrently. An elastic band was paired on nasal hook and attached to the forehead to elevate nasal dome and maintain the airway during retraction. After 1 year follow-up, alveolar gap was reduced by 4,5mm, lip segment moved closer to the midline, and bodyweight increased sufficiently. Conclusion: Modified PNAM can help infant with WS breath, increase nutritional intake and mold the alveolar tissue properly. Keywords: midline facial cleft, West syndrome, modified PNAM

Abstract

Treatment for the patient with cleft lip and palate (CLP) requires an interdisciplinary team concerned with improving the patient’s quality of life. This case evaluates the influence of comprehensive therapy on unilateral CLP complete dextra within a 4-year follow-up. A  4-days-old female patient visited a government hospital in Bandung after being referred to a pediatric clinic in the same hospital. The patient was born at 3.2 kg weight with no underlying medical conditions or syndromes. Intraoral examination revealed cleft soft and hard palate and separated alveolar segment. Extraoral examination revealed cleft lip unilateral dextra and shortened columella. The patient underwent compre-hensive therapy including presurgical NAM (PNAM), labioplasty, nasoalveolar molding (NAM), veloplasty, speech therapy, and palatoplasty. It is concluded that early treatment of PNAM reduces the severity of the orofacial deformi-ty, induced the reposition of soft & hard tissue and there’s an improvement in intelligibility and production of pharyng-eal sounds and velopharyngeal closure when using intravelar veloplasty in palatal repair along with NAM. In the fourth year of life, the patient has capable on communicates with intelligibility pronounce with relative adequate arch.

Abstract

ABSTRAKPendahuluan: Hilangnya struktur anatomi mandibula dapat disebabkan oleh karena malformasi kongenital, trauma, serta reseksi dengan menghilangkan jaringan tumor. Tumor ameloblastoma seringkali melibatkan sejumlah tulang pada mandibula dan metastasis dapat terjadi sehingga harus dilakukan reseksi. Defek yang mempengaruhi fungsi bicara, pengunyahan serta penelanan merupakan konsekuensi dari dilakukannya reseksi, sehingga pembuatan obturator menjadi penting untuk dilaksanakan. Laporan kasus ini bertujuan untuk memaparkan tatalaksana rehabilitasi fungsi dan estetika pada pasien post hemimandibulektomi dengan obturator definitif. Laporan kasus: Seorang laki-laki berusia 19 tahun datang ke Departemen Prostodonsia Rumah Sakit Hasan Sadikin Bandung untuk dibuatkan protesa pasca reseksi. Pasien telah menjalani operasi hemimandibulektomi disertai rekonstruksi plat dengan riwayat medis terdiagnosa ameloblastoma tipe plexiform ar. mandibula sinistra. Pasien merasa fungsi bicara, penelanan serta penampilannya terganggu karena setiap pergerakan membuka mulut, rahang mengalami deviasi dan kesulitan untuk mengunyah makanan yang halus sekalipun. Secara klinis pada bagian defek yang dipasang plat tidak ada rasa sakit, inflamasi serta infeksi sehingga dilakukan penatalaksanaan obturator definitif dengan pertimbangan agar dapat memulihkan fungsi pengunyahan dengan segera dan untuk memperoleh stabilitas oklusi. Pasien dibuatkan obturator definitif mandibula kerangka logam dengan metode pencetakan alter cast. Simpulan: Penatalaksanaan rehabilitasi dengan obturator definitif mandibula dapat memulihkan fungsi bicara, pengunyahan dan penelanan serta meningkatkan kepercayaan diri pasien.Kata kunciobturator definitif mandibula, pencetakan alter cast, hemimandibulektomi, ameloblastoma Aesthetic and functional rehabilitation on post hemimandi-bulectomy patient with obturator definitiveABSTRACTIntroduction: Losing part of the mandible structure might be caused by congenital malformations, trauma, and surgical resection by removing a tumor. It is frequently found that ameloblastoma involves several bones of the mandible and metastasis might occur resulting in mandible resection. As a consequence, the defects would occur and influence some functions including speech and swallowing so the fabrication of an obturator was necessarily needed. Case report: A 19 years old male came to the Department of Prosthodontics Rumah Sakit Hasan Sadikin Bandung required a prosthesis after surgical resection. The patient has been diagnosed with ameloblastoma plexiform type ar. mandibula sinistra and underwent hemimandibulectomy including plate reconstruction inserted. There are some impairments in speaking, swallowing also reduced visual performance because of mandibular deviation and chewing difficulty, even soft food most of the time. Mandibula definitive frame obturator was then delivered to the patient with the alter cast impression method. Conclusion: The rehabilitation with the mandible definitive obturator gained some function including speech, chewing, and swallowing and there’s an improvement in the patient’s self-esteem.Keywordsmandible definitive obturator, alter cast impression, hemimandibulectomy, ameloblastoma
