Abstract

Chronic kidney disease is still a problem faced by the world of health in Indonesia. The high prevalence of chronic kidney disease has increased every year. The high number of cases of chronic kidney disease causes higher efforts to seek treatment for chronic kidney disease which is influenced by perceptions of disease and perceptions of the health service system. This study aims to determine the illness perception and perception of the service system in chronic kidney patients with hemodialysis at one of the district hospitals in Yogyakarta Province (hospital X). Method: The research method was analytic observational with a cross-sectional approach which was carried out at the hemodialysis polyclinic at hospital X in January-February 2023. The sampling technique used Convenience sampling. Data collection was carried out by direct interviews with respondents using a questionnaire. Data were analyzed using the Kolmogorov-Smirnov test and the Chi-Square test. Result and discussion: The research results obtained a total of 102 respondents. The percentage of respondents with a good illness perception was 66.67% and a good perception of the health service system was 90.19%. Based on the results of the study, it can be concluded that perceptions of disease and perceptions of the health service system for chronic kidney patients with hemodialysis at Hospital X tend to be good. Conclusion: The results of statistical tests showed that there was no relationship between gender, age, duration of chronic kidney disease, education level, and family income on perceptions of disease and perceptions of the health service system at Hospital X, Special Region of Yogyakarta.

Abstract

Kesehatan yang baik muncul dari tingkat kepatuhan seseorang dalam mengkonsumsi obat. Kepatuhan seorang pasien dalam mengkonsumsi obat memiliki pengaruh besar terhadap kesembuhan pasien. Kepatuhan pasien minum obat antidiabetik sangat menentukan keberhasilan pasien dalam pengobatan diabetes mellitus. Tujuan dari penelitian ini adalah untuk mengetahui tingkat kepatuhan minum obat dan sikap pasien terhadap penyakit diabetes mellitus di Klinik Utama Bersama Pasir Pengaraian Rokan Hulu Riau. Metode penelitian bersifat deskriptif dengan menggunakan kuesioner sebagai tolak ukur kepatuhan pada pasien diabetes mellitus. Hasil penelitian menunjukkan tingkat kepatuhan minum obat pada pasien level tinggi sebanyak 7 orang (22,6%), level sedang 5 orang (16,1%) dan level rendah 19 orang (61,3%). Tingkat sikap responden terhadap pengobatan diabetes mellitus sebanyak 13 responden (42%) baik dan 18 responden (58%) kurang baik. Berdasarkan hasil penelitian menunjukan bahwa tingkat kepatuhan minum obat dan sikap pasien terhadap pengobatan diabetes mellitus di Klinik Utama Bersama Pasir Pengaraian Rokan Hulu Riau masih rendah dan kurang baik.

Abstract

The rapid escalation of healthcare demand in Indonesia has exacerbated systemic inefficiencies, characterized by high out-of-pocket (OOP) expenditures and hospital congestion. This study evaluates the effectiveness of telemedicine in mitigating these burdens within the Indonesian healthcare ecosystem. Utilizing a quantitative cross-sectional secondary analysis, the research synthesized datasets from 2023-2024, including the National Socio-Economic Survey (Susenas, n=345,000 households), Satu Data JKN portal (n=248 million participants), and SIRANAP metrics. A Cost-Effectiveness Analysis (CEA) framework was employed to calculate direct non-medical cost savings and labor productivity gains based on Sakernas hourly wage data. Results demonstrate that telemedicine integration facilitated a 66.7% reduction in total OOP expenditure per encounter, with a significant difference observed between digital and physical consultations (p < .001). Furthermore, a 20.7% national decrease in physical hospital queuing was recorded, resulting in a mean time-saving of 4.1 hours per patient (t(150) = 4.32; p < .001). These findings indicate that digital health adoption enhances allocative efficiency within referral facilities. Implications suggest that while digital platforms improve service throughput, efficacy correlates with regional internet penetration. It is concluded that telemedicine reduces financial friction and hospital load, though its impact is non-uniform across the archipelago. Future policy should focus on infrastructural equity in remote regions to ensure the sustainability of digital health access.

Abstract

Prevalensi penyakit diabetes melitus terus mengalami peningkatan dan menuntut pendekatan promotif dan preventif berbasis potensi lokal. Salah satu upaya yang dapat dilakukan adalah melalui edukasi pemanfaatan tanaman herbal lokal kepada masyarakat dalam kegiatan pengabdian kepada masyarakat. Kegiatan ini bertujuan meningkatkan pengetahuan dan keterampilan masyarakat dalam memanfaatkan tanaman herbal lokal sebagai minuman fungsional pendukung pengendalian kadar gula darah. Herbal yang digunakan meliputi jahe (Zingiber officinale), kayu manis (Cinnamomum burmannii), daun kelor (Moringa oleifera), dan ketumbar (Coriandrum sativum). Metode pelaksanaan berupa penyuluhan edukatif dan demonstrasi pembuatan minuman fungsional wedang herbal. Hasil menunjukkan semangat peserta yang tinggi, pemahaman dan kemampuan dalam membuat kembali minuman fungsional wedang herbal. Secara ilmiah, keempat tanaman ini mengandung senyawa bioaktif seperti flavonoid, polifenol, gingerol, cinnamaldehyde, dan isothiocyanate yang berpotensi meningkatkan sensitivitas insulin dan membantu menurunkan kadar glukosa darah. Pemanfaatan herbal lokal ini dapat menjadi strategi pemberdayaan masyarakat dalam mendukung pengendalian faktor risiko diabetes secara mandiri dan berkelanjutan.
