Abstract

PT. XYZ is a manufacturing company that produces urea fertilizer. The quality of urea fertilizer is influenced by three main parameters, namely H2O content, Biuret, and color. Based on known secondary data in the form of product defect history, in September 2023 product defects will be 12.9%. This research was conducted using the Six Sigma method with the aim of improving overall quality so as to prevent losses in company productivity. The research results show that in the defect category the H2O parameter has a DPMO value of 225834.7 with a sigma value of 2.25, for the biuret parameter it has a sigma value of 2.83 with a DPMO of 91216.7, and for the color parameter it is known that the DPMO value is 562700 or a sigma value of 1.34. then a root cause analysis is carried out using a fishbone diagram to obtain suggestions for improvement. Suggestions for improving Urea Fertilizer products for CTQ H2O include providing training/certification, coordinating with the Utilities section regarding steam, determining the right sampling point, and optimizing vacuum performance. For CTQ Biuret, namely providing training/certification, changing the tonnage in the urea process, paying attention to residence time and controlling the valve to work properly. For CTQ Color, namely providing training/certification, routinely cleaning dye spray filters, determining the right sampling point and routine pump maintenance.

Abstract

PT. XYZ is a manufacturing company that specializes in making food and soft drinks. According to the company, there have been no reports of incidents during the production process. However, based on empirical findings obtained from observations in the maintenance section, especially in the field of assembly work, many near misses or near misses occur during the implementation stage. Efforts that need to be made to overcome this are by identifying activities that are at risk of causing work accidents. In this research, work accidents were identified using the HIRA method, which then based on the results of the HIRA, improvements were then carried out based on priorities using the FMEA methodology. In the grinding machine assembly process, it is known that there are six work activities including material selection, grinding, welding, painting, assembly, and machine trial. The HIRA identification results show that there are 8 activities that have a medium risk of work accidents, for which FMEA analysis will be carried out. The results of the FMEA analysis show that the motor selection activity for the grinding drive has the highest RPN value so the control in the work is carried out using the help of a chain block, ensuring the chain block is in prime condition, using complete personal protective equipment, carrying out work according to SOP, and using chain block according to capacity.

Abstract

PT.XYZ merupakan perusahaan yang bergerak dalam bidang manufacturing dengan salah satu produknya yaitu leaf spring. Berdasarkan data history perusahaan pada periode 1 Oktober 2022 hingga 31 Maret 2023 diketahui persentase terjadinya defect produk leaf spring tersebut sebesar 0.79%-1.10%, sedangkan standar perusahaan maksimal 0.50%. Penelitian ini dilakukan untuk menganalisis penyebab tingkat cacat pada proses produksi. Six sigma menjadi metode yang digunakan bersama untuk mengendalikan kualitas produk. Dari hasil analisis yang dilakukan, terdapat empat jenis cacat pada produksi yaitu cacat material, visualĀ  not good, dan grinding surface not good. Hasil pada tahap measure diketahui bahwa tingkat sigma perusahaan sebesar 4.24 yang berarti sudah berada diatas nilai rata-rata perusahaan Indonesia. Dari hasil perhitungan yang disajikan pada diagam pareto diketahui bahwa cacat visual not good memiliki nilai kumulatif yang paling tinggi yaitu 41.6%. Penyebab terjadinya defect yang didapat dari brainstorming dikarenakan area kerja kurang nyaman, penyimpanan bekas kaleng cat tercampur dengan material lain, raw material terkena oli, pengerjaan tidak memerhatikan SOP, skill pekerja tidak merata, dan kondisi mesin yang tidak prima. Pembersihan setiap proses produksi mulai dari bahan mentah hingga produk jadi, penekanan pada SOP, penjadwalan pemeliharaan preventif pada setiap mesin, dan pelatihan pekerja secara berkala merupakan solusi yang disarankan untuk mengatasi cacat tersebut.
