Abstract

Kata kunci: hipertensi; pemberdayaan masyarakat; penyakit tidak menular ABSTRACT Non-communicable diseases (NCD) are the major causes of mortality in the world, including in Indonesia. One of the most prevalent type of NCD is hypertension. Karanganyar is a district in Central Java province with high incidence of hypertension and its prevalence continues to rise over years. Both central and local governments have established "Gerakan Masyarakat Hidup Sehat (Germas)", a community-based movement to promote healthy lifestyle, to control NCD. "Yayasan Usaha Umat Karanganyar (YUUK)", a community organization located in Tasikmadu, Karanganyar, provides education and training for locals but their main focus has not reached health programs. To optimize the locals' health through empowerment of community organizations, we conducted education and counseling about hypertension as well as training on using digital sphygmomanometer and oxymeter. The activities aimed to increase YUUK's roles in the prevention and early detection of hypertension. The activities involved teenagers living in the catchment area of Al Walidah mosque, Tasikmadu, Karanganyar. The activites resulted in an increased understanding about hypertension and skills to monitor cardiovascular health by using a digital sphygmomanometer,  Evaluation was performed by contacting the YUUK manager who is responsible for reporting the use of 2 units of digital sphygmomanometers and 1 unit of oxymeter that were given during the occasion so that the equipment can be used by locals. We also continuosly monitor the annual prevalence of hypertension in Karanganyar district. Keywords:  hypertension; community empowerment; non-communicable diseases

Abstract

Maximal oxygen uptake is a measure of cardiorespiratory capacity that shows human body fitness. Resting heart rate shows the ability of the heart to pump blood throughout the body thus resting heart rate related to the maximum oxygen volume. The young adult is an important phase of human in determining the productivity of the level of life. Maximum oxygen volume can be related to resting heart rate, but this has not been studied in detail regarding the relationship between maximal oxygen volume and resting heart rate in young adults. The purpose of this study was to determine the relationship between maximal oxygen volume and resting heart rate of young adults. This research uses cross sectional method. The number of research samples was 38 students from the Faculty of Medical Education, Sebelas Maret University (UNS). The dependent variable tested in this study was resting pulse rate and the independent variable was maximal oxygen volume. The research was conducted at the Physiology Laboratory of Sebelas Maret University, Surakarta. Resting heart rate was calculated by sleeping position in the supine position for 15 minutes then measured by pulse oximetry and maximal oxygen volume was calculated by Astrand-Rhyming Step Test. The results showed that the data were normally distributed so that the Pearson correlation test could be performed. The results of the correlation test showed that the value of p = 0.000 (p < 0.05) which indicated there was a significant correlation between the two variables tested and the Pearson correlation value of - 0.612 which indicated a negative correlation with a strong correlation strength. This study concludes that there is a significant correlation between maximal oxygen volume and heart rate of young adults with a negative correlation.

Abstract

Cardiovascular fitness, quantified as maximal oxygen consumption (VO₂Max), is a significant indicator of exercise tolerance and cardiovascular health. Sedentary behavior and aging are determinants that contribute to endothelial dysfunction, arterial stiffness, and reduced cardiac output, leading to a decline in VO₂Max. Cocoa Flavanols (CFs), rich in (−)-epicatechin, have been found to enhance endothelial function, nitric oxide bioavailability, and reduce blood pressure and hence might be beneficial for cardiovascular adaptation and VO₂Max improvement. This scoping review explores the potential of CFs to enhance cardiovascular function and exercise tolerance. Method: Two randomized controlled trials were employed in this scoping review. The first trial examined the effect of 30-day CF supplementation (1000 mg/day) on cardiorespiratory fitness in healthy elderly adults. The second trial examined the effect of 7-day CF supplementation (400 mg/day) on pulmonary oxygen uptake kinetics and exercise tolerance in middle-age sedentary adults. Results: In older adults, CF supplementation improved VO₂peak, oxygen pulse, exercise capacity, and endothelial function and lowered blood pressure in middle-aged sedentary adults, CFs improved VO₂ kinetics during moderate-intensity exercise, indicating enhanced oxygen delivery, but not exercise tolerance during severe-intensity exercise. Such results highlight CFs' positive effects on vascular health and efficiency of moderate-intensity exercise. Conclusion: Cocoa flavanols may enhance cardiovascular adaptation and exercise performance by improving endothelial function, reducing arterial stiffness, and optimizing oxygen uptake during exercise. Further studies are required to evaluate their effect on maximal exercise performance and long-term cardiovascular outcomes.

Abstract

Monocyte-derived macrophages regulate various aspects of inflammation. Phorbol 12-myristate 13-acetate (PMA)-induced THP-1 monocytes are widely used as monocyte-derived macrophage models suitable for studying the pathophysiology of inflammation and developing therapies. However, there is insufficient data concerning the baseline expression of pro-inflammatory receptors and the associated cytokine levels in THP-1-derived macrophage-like cells compared with their monocyte precursors. In this study, PMA-treated THP-1 cells were employed as models for monocyte-derived macrophage-like cells (M0). The mRNA levels of Toll-like receptors (TLR)4, receptor for advanced glycation end-product (RAGE), and TLR7 in THP-1 cells, both with and without PMA treatment, were evaluated using quantitative real-time polymerase chain reaction (RT-qPCR). ELISA was performed to quantify IL-6, TNF-α, and IL-18 levels in the THP-1 culture supernatant. The mRNA expression of TLR4, RAGE, and TLR7 was increased twofold in PMA-induced THP-1-derived macrophage-like cells compared to monocytes. IL-6 (p = 0.0277) and TNF-α (p = 0.0105) levels were significantly elevated in the culture supernatants of THP-1-derived macrophages. However, IL-18 levels did not show a significant increase. The upregulation of TLR4, RAGE, and TLR7 transcription was accompanied by elevated secretion of IL-6 and TNF-α during PMA-induced differentiation of THP-1-derived macrophage-like cells (M0), highlighting the polarization state and the specific role of macrophages in producing pro-inflammatory cytokines. This study provides baseline data on pro-inflammatory receptor expression and cytokine production in the THP-1-derived macrophage-like cells model for inflammatory research.
