Abstract

ABSTRAKHIV adalah suatu virus RNA. Virus ini memiliki target sasaran sistem, dan tetap berada dalam tubuh host seumur hidup. Pasien HIV dapat menjadi Acquired Immunodeficiency Syndrome (AIDS) dan memicuterjadinya infeksi oportunistik (IO). Tujuan penelitian adalah mengetahui pengaruh kadar CD4, Viral Load, dan NLR terhadap IO pasien HIV/AIDS. Penelitian ini menggunakan metode observasional analitik dengan pendekatan cross-sectional. Pengambilan data melalui rekam medis pasien HIV/AIDS di Poliklinik VCT admisi bulan September hingga November 2021. Besar sampel penelitian sebanyak 60 pasien, lalu data diuji dengan menggunakan Mann Whitney, Chi Square, dan Regresi Logistik Berganda. Hasil penelitian menyatakan bahwa sebanyak 24 (40%) subyek mengalami IO. Berdasarkan Uji Mann Whitney, kadar sel CD4 memiliki nilai p=0.00 (p0.05) berhubungan secara statistik terhadap IO, serta berdasarkan Uji Regresi Logistik Berganda didapatkan kadar sel CD4 memiliki nilai OR= 0.985 dan Nagelkerke R Square=0.492 artinya pengaruh kadar sel CD4 sebesar 49% terhadap kejadian IO. Berdasarkan Uji Mann Whitney, kadar NLR memiliki nilai p=0.126 (p0.05) artinya tidak berhubungan terhadap IO. Uji Mann Whitney, kadar Viral Load memiliki nilai p=0.039 (p0.05), didapatkan perbedaan yang signifikan. Kesimpulan: Kejadian CD4 berpengaruh terhadap kejadian Infeksi Oportunistik pada pasien HIV, sedangkan viral load dan NLR tidak berpengaruh. Kata Kunci: HIV/AIDS, IO, CD4, Viral Load, NLR  ABSTRACT HIV is a RNA virus from Retroviridae family and Lentivirinae subfamily. The immune system is the main target of the virus. The virus remains in the body for a lifetime. Patients HIV can develop Acquired Immunodeficiency Syndrom (AIDS) and triggers the occurence of Oppotunistic Infections (OI). The purpose of this study was to determine the effect of CD4, Viral Load, and NLR levels on the incidence of OI. Method: This study is an analytic observational study with a cross-sectional approach. Data collection was carried out through the medical records of HIV/AIDS patients that were admitted at VCT Polyclinic from September 2021 to November 2021. Sample consisted of 60 patients and the data analysis methods used Mann Whitney Test, Chi-Square Test, and the Multiple Logistics Regression Test. Result: 24 patients with HIV experienced OI (40%). Based on Mann Whitney Test, the CD4 levels had p-value=0.00 (p0.05) it was statistically related to the incidence of OI, and Multiple Logistics Regression Test, it was found that the CD4 levels had an OR = 0.985 and Nagelkerke R Square = 0.492, which means CD4 levels on the incidence of OI as much as 49%. Based on the Mann Whitney test, the NLR level has a p-value of 0.126 (p0.05) meaning there was not any statistical relation with OI. Based on the Mann-Whitney test, the Viral Load level has a p-value of 0.039 (p0.05). Conclusion: There was a correlation between CD4 level and the occurrence of OI in HIV patients, but viral load and NLR not. Keywords: HIV/AIDS, OI, CD4, Viral Load, NLR

Abstract

ABSTRACT COVID-19 is an infectious respiratory disease caused by infection SARS-CoV-2. Abnormal kidney function is associated with increased morbidity and mortality in COVID-19 patients. D-dimer levels, serum creatinine, and ALC can be used as markers of inflammation to determine the severity of COVID-19 comorbid chronic kidney disease (CKD). However, there are not many studies on this subject. This study aims to determine the impact of D-dimer, serum creatinine, and Absolute Lymphocyte Count (ALC) on the severity of COVID-19 comorbid CKD. This study uses an analytic observational method with a cross sectional approach from medical record data. The study population included COVID-19 patients with comorbid CKD at RS X Surakarta, who was examined for D-dimer levels, serum creatinine, and absolute lymphocyte counts, using purposive sampling and sample consisted of 53 patients. Bivariate analysis using Spearman’s Rank test obtained p value of D-dimer, serum creatinine, absolute lymphocyte counts 0,002; 0,042; 0,455. Multivariate analysis using multiple logistics regression showed that D-dimer OR=28,521 and serum creatinine OR=5,650, with a Nagelkerke R Square value of 0,338. There is an impact of D-dimer and serum creatinine levels on the severity of COVID-19 patients with CKD. ABSTRAK COVID-19 adalah penyakit pernapasan menular akibat infeksi virus SARS-CoV-2. Penyakit ginjal merupakan salah satu komorbid terbanyak pasien COVID-19, fungsi ginjal yang tidak normal berhubungan dengan peningkatan angka morbiditas dan mortalitas pasien COVID-19. Penelitian ini bertujuan untuk mengetahui pengaruh antara D-dimer, kreatinin serum, dan jumlah limfosit absolut terhadap derajat keparahan COVID-19 dengan komorbid chronic kidney disease (CKD). Penelitian ini menggunakan metode observasional analitik dengan pendekatan cross sectional dari data rekam medis. Populasi penelitian meliputi pasien COVID-19 dengan komorbid CKD di RS X Surakarta yang dilakukan pemeriksaan kadar D-dimer, kreatinin serum, dan jumlah limfosit absolut dengan teknik sampling purposive sampling didapatkan sebanyak 53 sampel. Analisis bivariat menggunakan uji korelasi Rank Spearman dan didapatkan nilai p D-dimer, kreatinin serum, jumlah limfosit absolut dengan keparahan COVID-19 masing-masing 0,002; 0,042; 0,455. Analisis multivariat menggunakan uji regresi logistik berganda dan didapatkan nilai OR D-dimer dan kreatinin serum masing-masing 28,521 dan 5,650 dengan nilai Nagelkerke R Square 0,338. Kesimpulan penelitian ini bahwa Kadar D-dimer dan kreatinin serum berpengaruh terhadap derajat keparahan pasien COVID-19 dengan komorbid CKD.

Abstract

Durian is mainly distributed in Southeast Asia and is already famous among Indonesian society. In addition, the nutritional content and unique flavor of the pulp, part of durian fruit such as the seed, peel, roots, and leaves also have many benefits. Durian seeds are known as inedible parts and should disposed of as food waste. However, these are a good source of bioactive compounds. Durian seeds contain bioactive compounds that are antioxidants, including phenolics, flavonoids, and alkaloids. Flavonoids are phenolic compounds which have good benefits for human health. This research aims to analyze the potential antioxidant activity and flavonoid content of durian seed waste. The extraction used ethanol 70%, the antioxidant activity test used a scavenging test to 2,2-diphenyl-1-pycryl-hydrazil (DPPH) radical, and total flavonoid content was determined by the UV-Vis spectrophotometry method. Based on the results, durian seed extract has antioxidant activity of 99.70 µg/ml and total flavonoid content of 23,24±0,39 mgQE/g. Durian seed extract has relatively strong antioxidant activity and contains flavonoids as a bioactive compound. This information is useful for researchers to find bioactive compounds that may be new nutraceuticals for antioxidants, anti-inflammatory, anti-diabetic, and therapeutic functional foods.

Abstract

People living with HIV/AIDS (PLWHA) are vulnerable to anxiety related to their illness, which can adversely affect inflammatory markers such as the Neutrophil-to-Lymphocyte Ratio (NLR) and overall quality of life (QOL). This study aimed to examine the effect of Self-Resignation Exercise as a complementary and holistic intervention on NLR and quality of life among HIV patients experiencing anxiety disorders. A randomized controlled trial with a pretest–posttest control group design was conducted in the Infection Subsection of Dr. Moewardi Hospital, Surakarta, starting in January 2024. Participants were HIV patients with anxiety disorders who had stable physical conditions and received routine treatment, and they were randomly assigned to a control group (n = 20) and an intervention group (n = 20). Anxiety levels were assessed using the Hamilton Rating Scale for Anxiety (HRSA), quality of life was measured with the WHOQOL-BREF, and inflammatory status was evaluated through blood tests to determine NLR. The intervention group received Self-Resignation Exercise in addition to standard care, while the control group received standard care alone. The results demonstrated a significant reduction in anxiety levels and a significant improvement in quality of life in the intervention group compared to the control group, with all measured outcomes showing p-values < 0.001. In conclusion, Self-Resignation Exercise is effective in reducing anxiety and improving quality of life in people living with HIV/AIDS, indicating its potential as a complementary approach to enhance both psychological well-being and overall health status.

Abstract

Human Immunodeficiency Virus (HIV) infection remains a major global health problem and may affect the quality of life of people living with HIV. Quality of life can be assessed using the World Health Organization Quality of Life-Human Immunodeficiency Virus BREF (WHOQOL-HIV BREF) questionnaire. This study aimed to analyze the association of viral load, CD4 count, neutrophil-to-lymphocyte ratio (NLR), serum glutamic pyruvic transaminase (SGPT), and estimated glomerular filtration rate (eGFR) with quality of life among HIV patients. This observational analytic study used a cross-sectional design and was conducted at the Voluntary Counseling and Testing Clinic of Dr. Moewardi Hospital, Surakarta, Central Java, Indonesia. A total of 100 HIV patients undergoing antiretroviral therapy were included based on predefined inclusion and exclusion criteria. The independent variables were viral load, CD4 count, NLR, SGPT, and eGFR, while the dependent variable was quality of life assessed using the WHOQOL-HIV BREF questionnaire. Clinical and laboratory data were obtained from medical records. Data were analyzed using univariate analysis, Spearman correlation test, and ordinal logistic regression with SPSS version 29.0. Higher viral load, lower CD4 count, and elevated NLR were significantly associated with poorer quality of life. In contrast, SGPT and eGFR were not significantly associated with quality of life. In the multivariate analysis, CD4 count showed the strongest association with quality-of-life category. These findings suggest that viral load, CD4 count, and NLR are associated with quality of life among HIV patients. The integration of virological, immunological, and inflammatory markers may complement clinical assessment of quality of life in people living with HIV, particularly in the Indonesian clinical setting. Further longitudinal studies are needed to clarify their predictive value.

Abstract

Objective: The number of HIV cases in Indonesia reached a peak in 2019 with 50,282 cases dispersed over 407 of 507 districts and cities (or 80%) of the country's provinces. Central Java is the province after DKI Jakarta and East Java which has a mortality of 12.41 and an HIV/AIDS prevalence of 22% of all cases. High mortality in HIV patients influenced by very low CD4 count ≤ 50 cells/mm3. Identification of clinical characteristics, risk factors and causes of death is very important to carry out optimal management of HIV/AIDS for delaying the progression of infection and saving lives. Design and method: We identified three patients with newly diagnosed HIV between January to April, 2023. We provide demographic data, clinical characteristics, laboratory examination, chest x-ray imaging and final outcome. Results: We presented 3 newly diagnosed HIV patients. They are still young < 40 years old, the majority have BMI < 18.5. Major opportunistic infections are oral candidiasis (100%), Pruritic Popular Eruption (PPE), pulmonary infection due to Tuberculosis, Pneumocystis pneumonia (PCP) and bacterial pneumonia. Hematological alteration dominated by anemia. Increased of transaminase enzyme and hyponatremia are also common. All patients had CD4 count < 20 cells/mm3, only one patient survived. Causes of death were pulmonary infection mainly due to clinical PCP. Conclusion: PCP is one of the fatal opportunistic infections in HIV. It is necessary to be diagnosed as early as possible so that patients can start Anti-Retro Viral (ARV) at an early stage to improve prognosis.

Abstract

Introduction: Malaria and dengue are endemic in Indonesia, and therefore, may result in the possibility of co-infection. Urban demographic expansion, deforestation, and agricultural settlements in peri-urban areas are known causes of the increase in the probability of concurrent infection of these two diseases. It is reasonable to envisage that the occurrence of concurrent infections would not be rare, mainly during outbreaks of dengue in the rainy season. However, due to non-systematic investigation of both diseases, only a few cases of malaria and dengue co-infection have been reported. These three patients demonstrated co-infection manifestations with good outcomes and prognoses. Case Illustrations: Two cases of co-infection of malaria vivax and dengue fever in male patients aged 35 and 43 years. The difference between these two cases is that one patient had experienced a previous episode of malaria with a fever lasting less than 5 days. Meanwhile, one patient who had never suffered from malaria experienced a relatively longer fever episode, up to 14 days. Thrombocytopenia in these two patients was still above 50,000/µL and rose quickly after anti-malarial administration. The third case was a 47-year-old woman with co-infection of malaria falciparum and dengue. This woman lived in Papua and had been infected with malaria before. This third case showed platelet numbers reaching levels as low as 38,000/µL and hypotension without signs of shock in the critical phase of dengue. The three patients did not show complications from other organs due to malaria or dengue and responded well to antimalarials with good outcomes. These patients were treated with a combination of DHP and primaquine, along with supportive care for fever and fluid management. Conclusion: Whenever co-infection is confirmed, we recommend careful monitoring for bleeding and hepatic complications, which may result in a higher chance of severity. Careful and intense monitoring of fluid requirements is necessary to avoid complications of fluid overload.
