Abstract

Inflammation and oxidative stress are risk factors of cardiovascular diseases among chronic kidney disease patients. There is increasing of HsCRP level in stage V CKD patients wih CAPD. N-Acetyl Cysteine(NAC) is a compounds containing thiol with antioxidant and anti-inflammatory effects. This research obejectives was to examine the effect of oral NAC in the decline of HsCRP level in stage V CKD patients with CAPD. This is an experimental research with Randomized Double Blind Control Trial, involving 30 patients CKD with CAPD Dr Moewardi Hospital and complete inclusion and exclusion criteria, divided 15 patients as a control group (placebo) and 15 patients as a treatment group of oral NAC. Examination of data research variables have been performed before and after 8 weeks treatment. The results were the level of HsCRP in control group before treatment and after treatment (15,76±42,79 vs 16,50±30,63). Level of HsCRP in treatment group before treatment and after treatment (29,53±76,52 vs 4,56±12,85) was decrease statistically significant with p 0,05. The N-Acetyl Cysteine orally can decreased the level of HsCRP in stage V CKD patients with CAPD.Keywords : N-Acetyl Cysteine, CAPD, HsCRP

Abstract

Coronavirus Disease 2019 (COVID-19) adalah penyakit virus jenis baru yang memiliki daya penularan tinggi sehingga menyebabkan pandemi di dunia. Penderita HIV adalah penderita yang rentan terkena COVID-19 dan jika sudah terkena COVID-19 akan cenderung lebih parah dibandingkan populasi lainnya. Akibat belum ditemukannya pengobatan definitif, maka langkah terbaik untuk memutus rantai penularan COVID-19 adalah dengan menerapkan protokol kesehatan sebagai upaya pencegahan. Tujuan pengabdian ini adalah untuk menganalisis tingkat kepatuhan penderita HIV dalam menerapkan protokol kesehatan COVID-19. Pengambilan data dilakukan melalui survei kuesioner yang diberikan langsung kepada responden setelah dilakukan penyuluhan kesehatan. Responden adalah penderita HIV yang berjumlah 100 orang. Data dianalisa dengan analisis statistik chi-square (α=0,05) dengan software SPSS versi 21. Pada uji chi square, didapatkan hasil bahwa persepsi keparahan (p=0,012), persepsi hambatan (p=0,001), persepsi manfaat (p=0,031), dan isyarat untuk bertindak (p=0,003) berhubungan secara statistik terhadap kepatuhan protokol kesehatan. Sementara persepsi kerentanan (p=0,164) dan efikasi diri (p=0,120) tidak berhubungan secara statistik terhadap kepatuhan protokol kesehatan. Terdapat hubungan antara persepsi keparahan, persepsi hambatan, persepsi manfaat dan isyarat untuk bertindak terhadap kepatuhan protokol kesehatan. Kata kunci: penyuluhan, covid-19, kepatuhan, HIV ABSTRACT Coronavirus Disease 2019 (COVID-19) is a new type of viral disease that has a high transmission power, causing a pandemic in the world. People with HIV are people who are vulnerable to COVID-19 and if they have been exposed to COVID-19 will tend to be more severe than other populations. As a result of not finding a definitive treatment, the best step to break the chain of transmission of COVID-19 is to implement a health protocol as a prevention effort. The purpose was to analyze the level of adherence of HIV patients in implementing the COVID-19 health protocol. Data were collected through a questionnaire survey which was given directly to respondents after health education was conducted. The respondents were 100 people with HIV. This study uses chi-square statistical analysis (α=0.05) with SPSS version 21 software. In the chi square test, it was found that perceived severity (p=0.012), perceived barriers (p=0.001), perceived benefits (p=0.031), and cues to action (p=0.003) were statistically related to health protocol compliance. Meanwhile, the perception of vulnerability (p=0.164) and self-efficacy (p=0.120) were not statistically related to health protocol compliance.There is a relationship between perceived severity, perceived barriers, perceived benefits and cues to act on adherence to health protocols. Abstract

COVID-19 merupakan masalah kesehatan global yang menghantui berbagai aspek kehidupan. Orang dengan lupus (ODAPUS) merupakan populasi rentan terjadi penularan COVID-19, dengan tingkat kematian lebih tinggi daripada populasi normal. Mitra pengabdian ini adalah Yayasan Tittari Surakarta. Yayasan Tittari merupakan yayasan yang bertujuan mensejahterakan ODAPUS dengan melakukan berbagai macam kegiatan, Analisis masalah bersama mitra menunjukkan bahwa tingkat pengetahuan tentang COVID-19 yang rendah pada ODAPUS menyebabkan ODAPUS lebih rentan terkena COVID-19, ODAPUS menjadi takut kontrol ke rumah sakit dan menjadi tidak disiplin minum obat serta tingkat stress yang tinggi pada ODAPUS. Pengabdian ini bertujuan meningkatkan pengetahuan tentang COVID-19 pada ODAPUS dan menurunkan stress ODAPUS selama pandemik COVID-19. Kegiatan pengabdian meliputi penyuluhan di poliklinik dan webinar awam melalui aplikasi daring, pembuatan buku saku COVID-19 khusus ODAPUS. Pengabdian ini dapat meningkatkan pengetahuan tentang COVID-19 dan menurunkan angka stress pada ODAPUS Yayasan Tittari Surakarta. Kata kunci: Pencegahan, COVID-19, Lupus, ODAPUS, Yayasan Tittari ABSTRACT COVID-19 is a global health problem that haunts various aspects of life. People with lupus (ODAPUS) are a population susceptible to transmission of COVID-19, with a higher mortality rate than the normal population. The Tittari Foundation is a foundation that aims to improve ODAPUS by carrying out various kinds of activities, Problem analysis with partners shows that the low level of knowledge about COVID-19 in ODAPUS makes ODAPUS more susceptible to COVID-19, ODAPUS is afraid to go to the hospital and becomes undisciplined taking medication and high stress levels in ODAPUS. This service aims to increase knowledge about COVID-19 in ODAPUS and reduce stress for ODAPUS during the COVID-19 pandemic. Community service activities include counseling at the clinic and webinars through online applications, making a special COVID-19 pocket book for ODAPUS. Abstract

Background. IL 6  level is increased in stage V CKD patients on CAPD. N-Acetyl Cysteine is a compound containing thiol with antioxidant and anti-inflammatory effects.Objective. To examine the effect of oral NAC on IL 6 level in stage V CKD patients on CAPD.Method. An experimental research with Randomized Double Blind Control Trial,  involving 30 CKD patients on CAPD in Dr Moewardi Hospital. They were divided into control group/placebo (15 patients) and oral NAS treatment group (15 patients). IL 6 level was measured before and after 8 weeks of treatment.Result. Level of IL 6  before and after treatment in control group were (8.13 +7.62  vs 11.16 + 8.32 , p= 0,001), significantly (p<0,001). Level of IL 6 before and after treatment in treatment group were (9.72 + 7.29 vs 6.09 + 3.82, p = 0,002); lowered significantly (p<0,05).Conclusion. Oral NAC lowered the level of IL 6 in stage V CKD patients on CAPD.

Abstract

BACKGROUND: Anthrax is a zoonotic disease caused by Bacillus anthracis, whose endospores stimulate the release of pro-inflammatory cytokines and promote oxidative stress. Propolis, a natural resource that can be found in Indonesia, has been proven to have anti-apoptotic and antioxidant role which might be a potential adjuvant therapy for anthrax treatment. Hence in this study we aimed to investigate effect of propolis as an anti-inflammatory, anti-apoptotic, and antioxidant in anthrax rats model by examining the level of tumor necrosis factor (TNF)-α, caspase-3, and malondialdehyde (MDA), respectively.METHODS: This was an experimental post-test only study with 40 male rats weighed 180-200 g that were induced by anthrax spores injected subcutaneously. The rats were divided into one control positive group and four intervention groups that were administered with 200 mg/kgBW propolis extract for 7 to 14 days. The levels of serum TNF-α, caspase-3, and MDA were measured using enzyme-linked immunosorbent assay (ELISA) and analyzed with bivariate analysis.RESULTS: TNF-α, caspase-3, and MDA level were found lower in anthrax rats model given ethanol extract of propolis than the control group. The lowest concentration of TNF-α value was found in group administered with propolis extract 200 mg/kgBW for 7 days before the anthrax induction (6.136±0.205 pg/mL). The results were similar to the MDA serum and caspase-3 which were the lowest when the propolis was administered 7 days earlier (1.893±0.188 nmol/mL and 2.040±0.067 ng/mL). There was significant difference in the TNF-α, caspase-3, and MDA serum levels (p≤0.001) compared to control group.CONCLUSION: Propolis has anti-apoptotic and antioxidant effects which can be used as complementary therapy in anthrax infection.KEYWORDS: propolis, anthrax, anti-apoptotic, antioxidants

Abstract

People living with HIV/AIDS (PLWHA) are vulnerable to anxiety related to their illness, which can adversely affect inflammatory markers such as the Neutrophil-to-Lymphocyte Ratio (NLR) and overall quality of life (QOL). This study aimed to examine the effect of Self-Resignation Exercise as a complementary and holistic intervention on NLR and quality of life among HIV patients experiencing anxiety disorders. A randomized controlled trial with a pretest–posttest control group design was conducted in the Infection Subsection of Dr. Moewardi Hospital, Surakarta, starting in January 2024. Participants were HIV patients with anxiety disorders who had stable physical conditions and received routine treatment, and they were randomly assigned to a control group (n = 20) and an intervention group (n = 20). Anxiety levels were assessed using the Hamilton Rating Scale for Anxiety (HRSA), quality of life was measured with the WHOQOL-BREF, and inflammatory status was evaluated through blood tests to determine NLR. The intervention group received Self-Resignation Exercise in addition to standard care, while the control group received standard care alone. The results demonstrated a significant reduction in anxiety levels and a significant improvement in quality of life in the intervention group compared to the control group, with all measured outcomes showing p-values < 0.001. In conclusion, Self-Resignation Exercise is effective in reducing anxiety and improving quality of life in people living with HIV/AIDS, indicating its potential as a complementary approach to enhance both psychological well-being and overall health status.

Abstract

The Human Immunodeficiency virus (HIV) presents a major public health challenge due to its association with high mortality and morbidity rates. Infection HIV occurs when the virus attacks the body’s immune system at a cellular level. Without treatment, this can progress to the most advanced stage, Acquired Immune Deficiency Syndrome (AIDS). For people living with HIV (PLWHIV), lifelong treatment with antiretroviral (ARV) medication is necessary. This continuing therapy works to reduce the amount of virus in the blood, lowering the risk of transmission, preventing secondary infections, and improving the patient's general life quality. The success and effectiveness of ARV therapy depends on PLWHIV adherence. Poor adherence to antiretroviral therapy decreases its effectiveness and increases viral replication. Therapy adherence indicates that the patient understands the value of receiving therapy and follows the instructions. Knowledge is an important role in therapy compliance. This cross-sectional study was conducted at the VCT Polyclinic of the Department of Internal Medicine, Dr. Moewardi General Hospital in Surakarta, between April and July 2024. The target population included all patients who received a positive HIV diagnosis. A total of 60 patients from this group were selected to participate using a purposive sampling strategy. Bivariate analysis was then used to investigate the relationship between variables, with the Spearmean Rank statistical test.  The study result showed that 21 patients with poor level of knowledge had low compliance (92.9%), followed by patients with intermediate level of knowledge with moderate compliance 10 (60.0%), and patients with high level of knowledge with high compliance 29 (69.4%). A statistically significant, moderately strong positive relationship (r=0.572, p < 0.001) was found between patient knowledge and adherence to ARV therapy at the VCT polyclinic of Dr. Moewardi Hospital, Surakarta. This suggests that PLWHIV who receive adequately informed patients are more likely to adhere to their treatment plans. Therefore, improving patient compliance requires implementing robust educational programs and conducting further research to uncover other influencing factors.
