Abstract

Background: Chronic osteomyelitis remains difficult to treat and has a high rate of relapse after apparently succesful treatment. Cephalosporin has been greatly used for the antibiotic intervention for patient with osteomyelitis. The objective of this study was to explore bacterial profile and its microbial resistance pattern of each generation of cephalosporin from patients with chronic osteomyelitis at Dr. Hasan Sadikin General Hospital.Methods: A descriptive study was conducted to 39 medical records from patients with chronic osteomyelitis at Dr. Hasan Sadikin General Hospital that met both inclusion and exclusion criteria. The bacterial pattern and antimicrobial susceptibility test results were obtained from both medical record and laboratory database for five years, during the period of Januari 2009 to December 2014. Data of bacterial species, type of Gram staining, and numbers of resistance isolates to antiobiotic were taken from the collected data and analyzed in form of frequency tabulation and percentages.Results: As high as 43−52% of Staphylococcus aureus isolates were resistant to all generation of cephalosporin. Coagulase-negative Staphylococci and Streptococcus spp. showed resistance to cephalosporin as high as 29% and 67%, respectively. This study also discovered that 5 of 23 Staphylococcus aureus isolates were methicillin-resistant Staphylococcus aureus (MRSA). Klebsiella pneumoniae showed a leading number of resistance pattern for both third and fourth generation of cephalosporin (77%). Enterobacter cloacae showed highly resistance for both first and third generation of cephalosporin, between 57−86%.Conclusions: There is the high resistance of all bacteria to cephalosporin among chronic osteomyelitis patients. Abstract

Abstract: The correct positioning of the femoral tunnel and endobutton position determine the success of ACL reconstruction (ACLR). Improper femoral tunnel placement most common errors in failed ACLR. However, plain post-operative imaging in the early state is not routinely performed. EndoButton position evaluation by XRay was made because this technique was cheap and reliable in Indonesia. There is no consensus on the ideal positioning of EndoButton in compare with functional outcome after ACLR.  The study aimed to evaluate anatomic positions of tunnel and fixation devices, and also provide a baseline examination for future controls regarding tunnel placement and fixation devices, and also evaluation functional outcome after ACL procedure and also evaluation of functional outcome after ACLR with IKDC score. Between February 2019 to July 2019, 20 patients aged 17 to 59 years, with post ACL reconstruction were involved in this study. We are evaluating sex, age, affected the side of the knee, complication after an operation, related activity with injury, and evaluation postoperatively with femoral angle measurement Xray AP and lateral view. We also evaluated and correlate post ACRL and functional outcomes with the International Knee Documentation Committee (IKDC) score. The mean age of patients was 28,38 ± 9,3 years, (87,15% men and 12,5% women). There is no complication after the operation in all patients. Nine patients (56,3%) affected on the left side, and seven patients (43,8%) affected right-sided. All patients are performed single-bundle augmentation techniques for ACL reconstruction. Five (31,3%) patient has history fall and eleven (68,8%) patients caused by sports activity. All patient has Femoral angle measurement from knee AP view x-rays more than 17o, with minimum 39o and maximum 54o, with mean 46,63 ± 4,2. From lateral x rays, we interpreted nine patients (45%) interpreted as shallow and high, and 55% others interpreted as deep and low (optimal). The mean of IKDC score pre-op was 58,1±4,6 and means IKDC postoperatively 6 months was 84,68 ± 5,1. There was a significant difference in functional outcome between optimal endobutton and not optimal endobutton position group with p-value < 0,05. Early post-operative imaging (knee x-rays) provides useful information regarding tunnel placement and fixation devices. It also serves as a baseline examination for future reference. In this study, a single bundle ACL reconstruction shows a satisfying outcome. Keywords: Anterior cruciate ligament; knee; postoperative reconstruction; radiograph.

Abstract

The incidence of obesity in Indonesia has increased from 14.8% in 2013 to approximately 21.8% in 2018. There is no published data regarding assessments on functional outcomes and complication after Anterior Cruciate Ligament (ACL) reconstruction in patients with different Body Mass Index (BMI) in Indonesia. The aim of this study was to describe the distribution of ACL injury and compare the functional outcomes after ACL reconstruction using hamstring tendon autograft in patients with different BMI categories.This was a retrospective study in 2 groups of patients with different BMI categories: normal BMI group (18.5 to 24.99) and high BMI group (≥25), who had undergone surgeries for ACL injury in Dr. Hasan Sadikin General Hospital, Bandung, Indonesia, during the period of 2017 to 2018. Data were collected from medical records and direct examination on  functional outcome measurement using Lysholm score.This study comprised of 71 patients who were followed for a minimum of 6 months. Of 71 patients, 43 were in the normal BMI group and 28 were in the high BMI group. No significant differences in postoperative functional outcome between the groups when assessed using Lysholm score. In normal BMI group 95% were graded as excellent to good while5% were in the fair functional category. Meanwhile, in the high BMI group, 86% were graded as excellent to good and 14% were graded as having fair functional results. ACL reconstruction with Hamstring tendon autograft results in a good functional score and a high succes rate in both BMI groups. High BMI does not adversely affect functional outcomes as both BMI groups gain equal benefits. Rekonstruksi Anterior Cruciate Ligament Menggunakan Autograft Tendon HamstringInsidensi obesitas di Indonesia meningkat dari 14.8% pada tahun 2013 menjadi 21.8% pada tahun 2018. Tidak ada publikasi data yang menilai hasil akhir fungsional dan komplikasi setelah operasi rekonstruksi Anterior Cruciate Ligament (ACL) pada pasien dengan indeks massa tubuh (IMT) yang berbeda di Indonesia. Tujuan dari penelitian ini membandingkan hasil akhir fungsional menggunakan skor Lysholm setelah rekonstruksi ACL pada pasien dengan IMT yang berbeda. Penelitian ini bersifat retrospektif yang dibagi menjadi 2 kelompok penelitian dengan IMT yang berbeda; yaitu IMT normal (18.5–24.99) dan IMT yang tinggi (≥25), yang dilakukan operasi cedera ACL di Rumah Sakit Hasan Sadikin periode tahun 2017-2018. Penelitian ini terdiri dari 71 pasien dan dilakukan pengamatan selama minimal 6 bulan dengan jumlah pasien kelompok IMT normal sebesar 43 pasien, sedangkan 28 pasien pada kelompok IMT yang tinggi. Tidak ada perbedaan signifikan diantara kedua kelompok pada hasil akhir fungsional pasca operasi ACL. Kelompok IMT normal terdiri dari 95% dinilai sebagai excellent sampai good dan 5% dinilai sebagai fair, sedangkan untuk kelompok IMT yang tinggi terdiri dari 86% dinilai sebagai excellent sampai good dan 14% dinilai sebagai fair hasil fungsional. Simpulan, operasi rekonstruksi ACL memiliki skor akhir fungsional yang bagus dan angka kesuksesan yang tinggi pada kedua kelompok IMT.

Abstract

Fiksasi Disrupsi pada distal radial ulnar joint pada fraktur radius ulna dengan endobutton masih jarang dilakukan baik di Indonesia maupun diluar negeri, dari tahun 2015 sampai dengan 2016 hanya ada 1 pasien yang dilakukan fiksasi dengan endobutton di rumah sakit Hasan Sadikin. Penggunaan Endobutton pada kasus disrupsi dari distal radioulnar mempunyai keuntungan karena pasien dapat memulai latihan gerak lebih awal sehingga pasien dapat memulai aktifitasnya .Tujuan penelitian ini untuk mengetahui kemampuan fungs pasien dalam melaksanakan kegiatan sehari-hari setelah dilakukan operasi dengan endobutton, kemampuan diukur dengan mengunakan skor DASH.Laki-laki umur 13 tahun datang dengan keluhan nyeri dan deformitas pada pergelangan tangan kiri. Dari pemeriksaan fisik didapatkan adanya nyeri pada daerah pergelangan tangan kiri. Pada pemeriksaan xray di temukan adanya diskontinuitas dari tulang radius dan ulna disertai adanya pelebaran dari ligamen radioulnar . Pasien di lakukan operasi dengan reduksi terbuka dan fiksasi interna mengunakan plate dan screw dan dilakukan fiksasi dengan endobutton untuk robekan pada distal ligament radioulnar. Kemudian dilakukan follow up selama satu tahun. Skor Dash diukur umtuk melihat keterbatasan fungsi dalam melakukan kegiatan, pada bulan pertama skor DASH 49,2 kemudian menurun pada bulan ketiga menjadi 35, dan turun lagi pada bulan ke enam menjadi 20, pada bulan ke Sembilan menjadi 3,3 dan pada bulan ke 12 menjadi 0Kesimpulan dari follow up selama satu tahun didapatkan hasil klinis dan fungsional yang sangat baik, tidak ada lagi nyeri saat mengangkat beban,  pergerakan dari sendi yang sangat baik, dan pasien sangat senang dengan hasil operasi. Kedepannya perlu dilakukan penelitian yang lebih banyak mengenai pemakaian endobutton pada kasus dirupsi dari sendi distal radioulnar.Kata kunci : robekan distal radioulnarjoint, endobutton, fiksasi

Abstract

Pendahuluan: Dislokasi patella dan cedera yang berhubungan dengan medial patellofemoral ligament (MPFL) merupakan cedera lutut yang paling umum pada masa remaja yang  memberikan kontribusi 53 hingga 60% dari gaya tarik medial terhadap pergeseran ke arah lateral. Tujuan dari penelitian ini adalah untuk menggambarkan luaran setelah operasi dilaksanakan pada center kami. 1 Metodologi: Penelitian analitik observasional pada ‘pasien cedera MPFL antara Januari 2015 dan Juli 2019 dengan jumlah 31 pasien. Pencarian database indeks medis/bedah di Rumah Sakit Hasan Sadikin Bandung dilakukan dengan menggunakan kata kunci Cedera Ligamentum Patellofemoral Medial dan ruptur Ligamentum Patellofemoral Medial dengan hasil data berupa catatan klinis dan operatif, serta studi pencitraan. Hasil klinis dievaluasi menggunakan Skor Kujala untuk menilai rasa sakit, kualitas hidup, dan kekambuhan.Hasil: 31 pasien memenuhi kriteria inklusi dan menjalani operasi selama 2 tahun. Menurut jenis kelamin terdapat 11 pasien pria (35,48%) dan 20 wanita pasien (64,52%). Usia rata-rata adalah 26,87 tahun (kisaran 15-42 tahun). Mekanisme cedera pada 23 pasien adalah cedera olahraga (74,2%) dan 8 pasien cedera non-olahraga. Terdapat 25 pasien (80,64%) yang menjalani operasi MPFL dengan retinacular lateral release, dan 6 pasien (19,36%) menjalani operasi MPFL tanpa lateral retinacular release. 22 pasien (70,96%) menjalani rekonstruksi MPLF dengan endo-button dan graft, dan 9 pasien (29,04%) menjalani perbaikan MPFL dengan jahitan primer. Terdapat peningkatan median skor Kujala sebelum dan setelah operasi (setelah 1 tahun), dari 63,90 (kisaran 45-75) menjadi 88,19 (kisaran 80-100) dan dianalisis dengan pair t-test dengan hasil signifikan secara statistik (p <0,05).Kesimpulan: Tatalaksana dislokasi patella menggunakan rekonstruksi MPFL menunjukkan luaran klinis yang lebih baik yaitu sangat mengurangi rasa sakit, tingkat kekambuhan, dan membantu meningkatkan kualitas hidup.

Abstract

Posterior tibial slope (PTS) has been proposed as a potential risk factor for ACL injury; however, studies that examined this relationship have provided inconclusive and sometimes contradictory results. Further characterization of this relationship may enable the medical community to identify individuals at greater risk for ACL injury and possibly characterize an anatomic target during surgical reconstruction. Medical records of 20 patients who underwent MRI of the knee and athroscopy procedure between August 2016 and March 2017 were reviewed. Patients were separated into 2 groups: a study group of those subjects who had undergone surgery for ACL injury (n =10) and a control group of patients diagnosed without ACL Injury (n = 10). Demographic data were collected, and MRI images from both groups were analyzed using protactor android imaging software to obtain PTS measurements. Data then analyzed using SPSS v20. Data analysis demonstrated that the ACL injury group had significantly greater values for PTS (14.4 SD ± 3.5) compared with controls (10.1 SD ± 2.6). After stepwise elimination of non significant variables, the final t test independent determined that PTS (p value 0.08; p < 0.1) had statistically significant relationships with ACL injury. INW ratio, age, and sex were not demonstrated to be significant predictors of ACL injury in this final model. This study showed a relationship between increased PTS and ACL injury, which corroborates the findings of previously published studies. INW ratio, sex, and age showed no significant relationship with ACL injury.

Abstract

Kebocoran cairan dari terowongan atau portal merupakan masalah umum selama artroskopi lutut, yang berpotensi menyebabkan kekeruhan, gangguan visualisasi, dan waktu operasi yang lebih lama. Polytetrafluoroethylene (PTFE) adalah bahan yang dikenal karena sifatnya yang hidrofobik, tidak reaktif, tahan panas, dan tidak menyebabkan iritasi, sehingga banyak digunakan dalam aplikasi medis. Penelitian ini bertujuan untuk mengevaluasi efektivitas PTFE sebagai bahan penyumbat portal dan terowongan selama prosedur artroskopi. Penelitian eksperimental ini dilakukan pada 30 pasien yang didiagnosis mengalami cedera ligamen anterior cruciate (ACL), dan dijadwalkan menjalani artroskopi lutut di Departemen Ortopedi dan Traumatologi Rumah Sakit Hasan Sadikin (RSHS) Bandung, pada tahun 2024. Pasien dibagi menjadi dua kelompok: 15 orang menjalani operasi tanpa sumbat PTFE (Kelompok A) dan 15 orang dengan sumbat PTFE (Kelompok B). Usia rata-rata pasien adalah 24 tahun, dengan mayoritas berjenis kelamin laki-laki. Rata-rata penggunaan larutan NaCl pada Kelompok A dibandingkan Kelompok B adalah 13,3 vs. 7,767 liter. NaCl yang dikumpulkan melalui mesin hisap adalah 8,633 vs. 5,333 liter, dan NaCl yang ditemukan di lantai adalah 4,7 vs. 2,467 liter. Uji normalitas menunjukkan bahwa data terdistribusi normal (p > 0,05). Uji T tidak berpasangan menunjukkan perbedaan yang signifikan antara kelompok (p < 0,05). Penggunaan sumbat PTFE secara signifikan mengurangi kebocoran cairan dan meningkatkan efisiensi prosedur bedah. Keuntungan tambahan termasuk distensi sendi yang lebih baik, pengurangan kebutuhan akan torniket, dan efek tamponade terhadap perdarahan. Sumbat PTFE menawarkan alternatif yang hemat biaya dibandingkan dengan kanula silikon atau artroskop yang tidak memiliki fitur anti-balik cairan.
