Abstract

ABSTRACT Background: NSAIDs are drugs that are often used in pharmacological therapy due to their effectiveness as anti-inflammatory, antipyretic, and analgesic. In addition to providing benefits, NSAIDs have damaging effects due to their mechanism of action. The use of NSAIDs must be appropriate. Aims of this study was to evaluate the rationality of NSAID use in outpatients at Puskesmas Putri Ayu, Jambi City in 2023. Methods: This is a descriptive study, conducted at Puskesmas Putri Ayu Jambi City with a total sample size of 225. Results: from 225 samples, the distribution of patients who received NSAID therapy based on gender was mostly female, 75.6%, based on age, 42.7% people aged 46-55 years, based on employment status, 73.3% people did not work, and based on comorbidities, there were no comorbidities, 92% people. Based on the diagnosis, NSAIDs were given to musculoskeletal cases in 52% cases. The most commonly prescribed NSAID was diclofenac sodium. Conclusion: The rationality of the use of NSAIDs for outpatients at Puskesmas Putri Ayu Jambi City in January-March 2023 was 96.4% appropriate indications, and 96.9% appropriate doses and intervals. Keywords: NSAID, Rationality, Outpatients ABSTRAK Latar Belakang: NSAID adalah obat yang sering digunakan dalam terapi farmakologis karena efektivitasnya sebagai antiinflamasi, antipiretik, dan analgesik. Selain memberikan manfaat, OAINS juga memiliki efek yang merugikan karena mekanisme kerjanya. Penggunaan NSAID harus tepat. Tujuan dari penelitian ini adalah untuk mengevaluasi kerasionalan penggunaan OAINS pada pasien rawat jalan di Puskesmas Putri Ayu Kota Jambi tahun 2023. Metode: Penelitian ini merupakan penelitian deskriptif, yang dilakukan di Puskesmas Putri Ayu Kota Jambi dengan jumlah sampel sebanyak 225. Hasil: Dari 225 sampel, distribusi pasien yang mendapatkan terapi OAINS berdasarkan jenis kelamin paling banyak adalah perempuan yaitu 75,6%, berdasarkan usia paling banyak adalah 46-55 tahun yaitu 42,7%, berdasarkan status pekerjaan paling banyak adalah tidak bekerja yaitu 73,3%, dan berdasarkan penyakit penyerta paling banyak adalah tidak ada penyakit penyerta yaitu 92%. Berdasarkan diagnosis, NSAID diresepkan pada 52% kasus muskuloskeletal. NSAID yang paling sering diresepkan adalah natrium diklofenak. Kesimpulan: Penggunaan OAINS yang rasional pada pasien rawat jalan di Puskesmas Putri Ayu Kota Jambi pada bulan Januari-Maret 2023 adalah 96,4% tepat indikasi, dan 96,9% tepat dosis dan interval. Abstract

Background: Hypertension is a serious global health problem and a leading cause of morbidity and mortality worldwide. Dietary patterns, including high-fat intake and obesity, are modifiable risk factors strongly associated with hypertension. Objective: This study aims to analyze the relationship between fat intake and obesity with hypertension among men aged 35–54 years at Simpang IV Sipin Public Health Center. Methods: This study used an analytical observational design with a case-control approach. A total of 100 participants were included, consisting of 50 hypertensive cases and 50 controls. Fat intake was assessed using a 2×24-hour food recall and processed with NutriSurvey, while obesity was determined by BMI ≥ 25 kg/m². Data were analyzed using Chi-square and logistic regression. Results: A significant relationship was found between fat intake and hypertension (p < 0.001; AOR = 49.8, 95% CI: 8.35–297.11). There was no significant relationship between obesity and hypertension after multivariate analysis (p = 0.070; AOR = 4.47, 95% CI: 0.89–22.58). Conclusion: High fat intake is a dominant risk factor for hypertension in this population, while obesity showed no significant association after adjustment.

Abstract

Hypertension is a chronic disease with a high global prevalence and a major risk factor for cardiovascular complications. Angiotensin-Converting Enzyme Inhibitors (ACE-Is) are widely recommended as first-line antihypertensive therapy, making evaluation of their rational use essential to ensure treatment safety and effectiveness. This descriptive observational study employed a retrospective design using medical records of hypertensive outpatients at Puskesmas Simpang IV Sipin, Jambi, from January 2023 to December 2024. A total of 100 eligible patients were included. The evaluation assessed the appropriateness of indication, dosage, frequency, duration of therapy, and potential drug interactions based on World Health Organization (WHO) criteria and the JNC 8 guidelines. Most patients were aged 60 years or older and were diagnosed with essential hypertension or hypertensive heart disease. All ACE-I prescriptions were appropriate for their clinical indications (100%), while dosage, frequency, and treatment duration fully complied with guideline recommendations. Most identified drug interactions were synergistic, whereas antagonistic (11%) and potentiation (25%) interactions were generally mild to moderate and clinically manageable. Overall, ACE-I prescribing was considered rational. Continuous monitoring remains important, particularly for patients at risk of clinically significant drug interactions to optimize therapeutic outcomes and patient safety.

Abstract

Apolipoprotein A-5 (APOA5) is a hepatic regulator of triglyceride-rich lipoprotein metabolism, and the promoter polymorphism rs662799 is its most extensively investigated variant. This narrative review synthesises current evidence on the molecular consequences of rs662799 and its relationship with lipid profile, central obesity, and metabolic syndrome, with particular attention to its relevance for Asian and Indonesian populations. Literature published between 2011 and 2026 was retrieved from PubMed, ScienceDirect, SpringerLink, Google Scholar, and Garuda, and thirty-nine sources met the selection criteria. The minor allele reduces promoter transcriptional activity, lowers APOA5 concentration, and impairs lipoprotein lipase-mediated triglyceride clearance, resulting in higher plasma triglyceride and lower high-density lipoprotein cholesterol. Meta-analyses report a 1.3- to 1.5-fold increase in metabolic syndrome risk among minor allele carriers, whereas associations with central obesity are inconsistent and frequently conditional on energy intake, sex, or degree of adiposity. Minor allele frequency is substantially higher in East Asian than in European populations, indicating population-specific clinical implications. Evidence relating this variant to lipid-lowering pharmacotherapy response remains scarce and warrants investigation, particularly in Indonesia where population data are still limited.
