Abstract

Completeness of medical record documents becomes important because it describes the practice of quality services. RSUD dr. R. Koesma Tuban have problems with completeness of medical record documentation. Incompleteness medical record documentation of outpatient at RSUD dr. R. Koesma Tuban was rated at 87, 67%. The object of this study was to identify the incompleteness of medical record documentation of outpatient based on Victor Vroom expectancy theory motivation at RSUD dr. R. Koesma Tuban. The present study conducts a cross-sectional, descriptive research during May 2021 through to use questionnaires and observation on medical record documentation of outpatient. The respondents of this study were personnel who were filling the outpatient medical record documentation. These people were 5 doctors, 5 refractions, and 6 nurses. The result of the study showed that Filling of the medical record documentation at RSUD dr. R. Koesma Tuban was low. Motivation has driving factor to the incompleteness of outpatient medical record documentation at RSUD dr. R. Koesma Tuban. Suggestions given to monitoring the performance to complete the medical record documentation of outpatient. Keywords : hospital, incompleteness, medical record, motivation, outpatient, victor vroom

Abstract

The community health center (Puskesmas) is one of the many stakeholders involved in the implementation of the national health insurance program, which aims to provide health protection to participants with the goal of maintaining health and safeguarding to meet basic health needs. Patient satisfaction is influenced by the quality of health services. This research aims to explore between the quality of health services and patient satisfaction among National Health Insurance participants at the Wire Community Health Center. This type of research is quantitative with a cross-sectional approach at the Wire Community Health Center. The sample in this study consists of 108 individuals selected using probability sampling techniques with simple random sampling method. Data collection was conducted using a questionnaire that covered aspects of healthcare service quality. The results of the study, after being analyzed using Spearman with a significance level of a = < 0.05, showed a value of r = 0.000 < 0.05, with a correlation coefficient of r = 0.048, which means the strength of the relationship between the two variables is categorized as strong. Based on the analysis of the data, it can be concluded that there is a relationship between the quality of health services and patient satisfaction among national health insurance participants at the Wire Community Health Center. The quality of service at the Wire Community Health Center is good, and the majority of national health insurance patients are satisfied with the services provided.

Abstract

Hospital Health Promotion plays an important role in improving public health literacy and supporting preventive healthcare services. However, health promotion activities in several hospitals in Tuban are still predominantly conducted using conventional methods, resulting in limited effectiveness and accessibility of health information. This community service program aimed to optimize hospital health promotion through the utilization of digital educational media. The program was implemented using a participatory and educational approach involving healthcare workers, hospital health promotion staff, and hospital management. The activities consisted of preliminary assessment, development of digital educational media, training and mentoring sessions, implementation of digital health promotion, and evaluation. The digital media developed included educational videos, digital posters, social media content, and QR code–based educational materials. The results showed an improvement in participants’ knowledge and skills regarding the utilization of digital media for health promotion activities. Healthcare workers were able to develop and use digital educational content effectively within hospital services. Patients and visitors also demonstrated positive responses toward the use of digital educational media because the information was considered more attractive, interactive, and easier to access compared to conventional methods. In addition, digital media contributed to improving the consistency and efficiency of health education delivery in hospitals. In conclusion, optimizing hospital health promotion through digital educational media can enhance healthcare workers’ competencies, improve public access to health information, and support the improvement of healthcare service quality. Continuous training and institutional support are necessary to ensure the sustainability of digital-based health promotion programs in hospitals.
