Abstract

Binahong plants has been widely used for treatment many diseases such as diabetic mellitus. Binahong leaves have been studied for secondary metabolic compounds including saponins, flavonoids, steroids, and terpenoids. Objective of the study was to performed in vivo antihyperglycemic test of binahong leaves ethyl acetate extract on blood sugar levels in male mice (Mus musculus). Binahong leaves were macerated with ethyl acetate solvent and antihyperglycemic test using glucose 40 mg/20 gBW tolerance method. In the study, number of mice used were 25 mice and divided into 5 treatment groups namely negative control, positive control, ethyl acetate extract of binahong leaves dose of 100 mg/KgBW, 200 mg /KgBW, and 400 mg/KgBW. The results of the study were ethyl acetate extract of binahong leaves contained secondary metabolites of alkaloids, flavonoids, saponins, and phenols. Data analysis was performed using Statistical Product and Service (SPSS) with the One-Way ANOVA test and post hoc Tukey. The results showed that there was a significant difference (p <0.05) which stated that binahong leaves ethyl acetate extract had good effect in lowering blood glucose levels with an effective dose 100 mg/KgBW. Abstract

Prescriptions for patients of the Health Social Security Organizing Agency (BPJS) in hospitals in accordance with the National Formulary (FORNAS) guidelines regulate drugs that must be given to patients according to complaints. The benefits of FORNAS optimize the quality of pharmaceutical services, especially to BPJS Kesehatan social security patients. Hospital X Bekasi is a type C private hospital located in Bekasi, West Java as an Advanced Referral Health Facility (FKRTL) prescribing services for chronic diseases with outpatient polyclinic services with a 30-day limit according to restriction requirements or indications Many prescriptions are not in accordance with FORNAS and drugs that have not been registered in FORNAS so that their administration is based on the policies of the Medical Committee and the hospital director at the expense of the hospital. The aim of the study was to determine the percentage of prescribing conformity that was not in accordance with FORNAS at Hospital X Bekasi from January to March 2021. Non-experimental research method with a descriptive approach conducted retrospectively on a population of 6,891 specialist polyclinic outpatient prescription sheets from January to March 2021. The number of samples based on the calculation of the slovin formula is 400 prescription sheets. Inclusion criteria were outpatient specialist polyclinic prescriptions with BPJS Health coverage and exclusion criteria were outpatient prescription sheets for non-chronic diseases. The suitability of drug prescription to FORNAS at X Bekasi Hospital was 84,5 % of the total prescriptions. Prescription concordance in specialist clinics was highest at 100% cardiac clinic, 97.5% urology clinic, 95% pulmonary clinic, 91.25% internal medicine clinic and lowest at 38.75% neurology clinic.. The most common drug found not in accordance with FORNAS was Mecobalamin 500 mg in urology polyclinic 100%, internal medicine polyclinic 42.86%, neurology polyclinic 28.57% and lung polyclinic 25%. The mismatch of prescription writing based on FORNAS causes an inefficient reduction in INA-CBGs costs affecting long LOS and increasing the total real cost of patients. Suggestions need to socialize to doctors to write prescriptions in accordance with FORNAS and conduct periodic evaluations of prescribing discrepancies

Abstract

Resep elektronik merupakan permintaan obat dari dokter untuk pasien dengan memanfaatkan teknologi perangkat lunak ke farmasi. Pelayanan resep memiliki Standar Pelayanan Minimal (SPM) Rumah Sakit yaitu waktu tunggu obat non racikan ≤ 30 menit dan obat racikan ≤ 60 menit. SPM waktu tunggu di Rumah Sakit X Kota Tangerang untuk obat non racikan ≤ 8 menit dan obat racikan ≤ 30 menit. Tujuan penelitian untuk mengevaluasi waktu tunggu obat resep elektronik dan faktor yang mempengaruhinya. Metode penelitian deskriptif secara prospektif dengan pengambilan data sampel menggunakan teknik random sampling pada resep elektronik pasien rawat jalan periode April – Mei 2022. Total populasi resep Januari 2022 adalah 750 lembar dan perhitungan sampel menggunakan rumus slovin berjumlah 261 lembar resep.   Hasil penelitian waktu tunggu obat non racikan berjumlah 213 lembar resep dengan waktu ≤ 8 menit adalah 87 lembar resep (40,85 %), sedangkan obat racikan berjumlah 48 lembar resep dengan waktu ≤30 menit adalah 19 lembar resep (39,58%). Faktor yang menyebabkan tidak tercapainya SPM waktu tunggu obat adalah tahap pengambilan obat non racikan (rata-rata 3,11 menit).

Abstract

Self-medication or self-treatment is one way to treat diseases experienced without a doctor's prescription. Limited public knowledge about drugs can cause medication errors from drug selection, drug dosage and how to use drugs. Drug abuse and inappropriate drug use are often because the public does not get education or pharmacists give wrong drug recommendations. This study aims to evaluate the implementation of self-medication services by TTK at Pharmacy X, Tangerang City and the drug recommendations given. Quantitative research method with a prospective descriptive approach. Data were obtained through direct observation using a form on self-medication services by 6 TTK for 30 working days from February 1 to March 2, 2021. The sample consisted of 105 consumers who bought drugs without a doctor's prescription and the data was analyzed descriptively using Microsoft Excel with percentage calculations. The results of the study showed that consumer characteristics were dominated by men 55.24% and adults aged 26-45 years 46.67%. The implementation of self-medication that was not carried out 100% was the method of using drugs 55.23% and storing drugs 0%. The most common complaint was a dry cough in 20 cases (19.05%) accompanied by flu in 10 cases, colds in 6 cases, itchy throat in 3 cases and stomach ulcers in 1 caseInconsistency of drug recommendations occurred in complaints of dry cough, namely drugs containing guaifenesin indicated for phlegmy cough in 9 cases and phlegmy cough containing dextromethorphan HBr in 1 case which is an antitussive that can inhibit the cough reflex to expel phlegm. This shows a lack of understanding by TTK regarding the composition of drugs according to complaints and drug indications. TTK self-medication services need to be improved, especially aspects of drug storage education and drug recommendations based on complaints to minimize medication errors and improve the quality of pharmaceutical services at the community level.
