Abstract

This study aims to identify the effectiveness of nurses in carrying out discharge planning for stroke patients and their families based on the components of Swanson's Theory of Caring. The research method used is a systematic review through databases and electronic websites, namely Scopus, Sciencedirect, ProQuest, Research Gate, PubMed and Google Scholar. The research results showed that of the 15 research articles there were articles that reported an increase in knowledge of stroke patients and their families after being given discharge planning so that they could improve care after being at home. Conclusion: Discharge planning based on Swanson's Caring Theory involves five components, namely maintaining belief, knowing, being with, doing for, enabling. There is effectiveness of nurses in implementing caring-based discharge planning for patients with stroke. Keywords: Caring, Patient Discharge Planning, Stroke

Abstract

Adequate nutrition is very important for the body, especially during the growth and development of children. Stunting is a condition of impaired growth in children under five years of age due to chronic malnutrition. This study aims to determine the effect of providing additional animal protein through complementary feeding (MP-ASI) and education based on visual media on the anthropometric parameter of waist circumference in children with stunting. This study used a quasi-experimental method with a pre-test and post-test design. The study involved a sample of 9 respondents who were children with stunting in the working area of the Jayengan Public Health Center, Surakarta City. The sample criteria included children with a waist circumference not appropriate for their age. Total sampling was used as the sampling technique, meaning all eligible individuals from the population were included in the study. The implications of this research indicate that the combination of providing additional animal protein through complementary feeding (MP-ASI) and visual media-based education is an effective strategy for improving the nutritional status of children with stunting. Specifically, this combined intervention significantly increased the children's waist circumference after the program. This demonstrates that the method successfully improved a crucial anthropometric parameter related to the nutritional status of stunted children. The practical implication is that this approach can be implemented by health workers in public health centers (puskesmas) or community health posts (posyandu) as a nutritional program model to accelerate stunting reduction, particularly in areas with high stunting prevalence and limited access to information.

Abstract

Background: One of the vital signs monitoring systems currently being developed in medical-surgical wards is the Early Warning Score (EWS). However, its implementation still needs to be evaluated, given the frequent occurrence of code blues and even deaths in the ward due to delays in recognizing deteriorating patient conditions. Purpose: To determine and analyze the quality of documentation regarding the implementation of EWS monitoring in patient setting. Method: A retrospective survey of patient medical records, supported by an assessment tool. Medical records were collected based on inclusion criteria, namely medical records from both medical and surgical wards. Results: The completeness of the EWS form was still quite low at 44%, with monitoring frequency accuracy at 12%, and total score calculation accuracy at 3.8%. Several important parameters that were incomplete and needed improvement included blood sugar (65%), which was the component with the lowest achievement; level of consciousness (84%), blood pressure (89%), temperature (90%), total score (89%), and escalation scale (88%), which was still suboptimal. Conclusion: The significant gap between the implementation of EWS monitoring and the accuracy of its documentation indicates that nurses' consistent documentation of EWS monitoring needs to be improved. This condition increases the risk of clinical deterioration, delayed intervention, serious complications, and even death. Inaccurate documentation also hinders interprofessional communication and increases the potential for clinical errors.
