Abstract

Background. The morbidity and mortality of patients with acute coronary syndrome (ACS) are very high so it is important to do risk factor stratification. Several studies found that plateletcrit plays an important role in predicting mortality in STEMI patients and that it had a significant correlation as a predictor of a coronary slow flow phenomenon correlated with a worse cardiovascular outcome. The aging process is associated with altered platelet activity and a higher rate of vascular disease. The effect of aging on thrombotic function is not fully understood yet, so further research is needed. It is hypothesized that high plateletcrit increases the risk of major cardiovascular events (MACE) in elderly acute coronary syndrome (ACS) patients treated in intensive cardiac care unit (ICCU).Objective. To determine the role of plateletcrit as a risk factor for major cardiovascular events in elderly acute coronary syndrome patients.Method. We collected samples of elderly patients who experienced ACS in the period January 2016 - December 2019, recorded plateletcrit data and whether there were MACE incidents or not. The data were processed in SPSS to find a cut off of plateletcrit values which are a risk factor for the occurrence of MACE in elderly patients at Dr. Sardjito Hospital.Result. A total of 174 study subjects consisted of 58 MACE and 116 non-MACE groups, the results showed that platelecrit ≥0.35 increased the risk of MACE with a p value of 0.046 and OR 5.49.Conclusion. Plateletcrit is statistically significant to the incidence of MACE in elderly coronary syndrome patients who are treated in intensive cardiac are units.

Abstract

Non-ST Elevation Myocardial Infarction (NSTEMI) merupakan bagian dari sindrom koroner akut (SKA), yang sering kali ditandai dengan nyeri dada tipikal dan atipikal. Manifestasi atipikal dapat menyulitkan diagnosis dini, terutama di fasilitas kesehatan dengan keterbatasan sumber daya. Penatalaksanaan yang cepat dan tepat diperlukan untuk mencegah komplikasi fatal. Dilaporkan seorang pria 53 tahun dengan keluhan nyeri dada menjalar ke punggung dan lengan kiri, disertai keringat dingin. Gejala awal berupa nyeri atipikal yang berubah menjadi tipikal. Pasien memiliki riwayat hipertensi tidak terkontrol, kebiasaan merokok, obesitas (IMT 30,3), dan pola makan tidak sehat. Diagnosis NSTEMI ditegakkan berdasarkan gejala klinis dan hasil EKG (T-inversi pada lead V1-V4). Penatalaksanaan meliputi terapi farmakologis sesuai panduan, namun keterbatasan fasilitas menghambat tindakan invasif seperti angiografi koroner. Manifestasi atipikal sering ditemukan pada pasien dengan faktor risiko seperti diabetes, usia lanjut, dan wanita. Penanganan NSTEMI melibatkan kombinasi terapi farmakologis, evaluasi risiko menggunakan skoring (TIMI, GRACE), dan strategi invasif bila tersedia. Diagnosis dini sangat penting meskipun dengan keterbatasan fasilitas diagnostik. Kesimpulan: Kasus ini menyoroti pentingnya deteksi dini dan penanganan NSTEMI terutama di fasilitas dengan sumber daya terbatas. Edukasi pasien dan keluarga tentang kontrol faktor risiko serta kepatuhan terapi menjadi kunci pencegahan komplikasi berulang.

Abstract

Hospitals must develop relevant business strategies in order to compete. In determining a strategy, internal and external analysis is needed so that the strategy used is appropriate. Based on a survey by the Provincial Health Service, several criteria for type C hospitals were not met, so the Buton Regency Regional Hospital was threatened with being downgraded. This research aims to determine the appropriate management strategy for District Hospitals in HR management, hospital facilities, and financial management to maintain type C class status. This qualitative study uses an in-depth interview method with purposive sampling. The sample in this study was the regional secretary as the representative of the regional government as the owner of the hospital, the hospital director, the head of finance, the head of personnel, the head of hospital facilities, general practitioners, and specialist doctors of all divisions in the hospital, the head nurse of each treatment room. The research using SWOT analysis results shows that Buton District Hospital is in a position of growth and development and, based on the internal-external matrix, is in a stable position. This condition is good because Buton District Hospital is in a favorable position to take advantage of existing opportunities and strengths. The implications of the research results are for the management of Buton District Hospital to be used as suggestions or references for consideration in making hospital strategies to maintain the status of a type C hospital. Providing input to the head of the Regional Government regarding the great need for financial assistance to meet the requirements of the Minister of Health Regulation regarding the requirements for a type C hospital that Buton District Hospital has not met.
