Abstract

This study is entitled Review of Medical Information Release Services and Filling in Raharja Service Claim Files for Traffic Accident Victims at Al Ihsan Hospital, Bandung Regency, West Java Province. The purpose of this study is to determine the Medical Information Release Service and Filling in Jasa Raharja Claim Files for Traffic Accident Victims at Al Ihsan Hospital, Bandung Regency, West Java Province. The method used in this study is: using a descriptive method with a qualitative approach. The subject of this study is the Medical Information Release Service and Filling in the Raharja Service Claim File at Al-Ihsan Hospital, Bandung Regency, West Java Province. The object of this study is the standard operating procedure for releasing medical information and Jasa Raharja claim forms for traffic accident victims who claim Jasa Raharja insurance at RSUD AlIhsan Bandung Regency. Data collection techniques by observation, interview and documentation. Research results: medical information release services at Al-Ihsan Hospital are in accordance with SOPs. There is no written procedure for filling Jasa Raharja's claim file but the implementation is in accordance with the SOP.

Abstract

The results of a preliminary study at the Bandung Regional General Hospital Kiwari in the March – December 2022 Service found 78 duplicated medical records unsustainable medical record Data and inefficient resources are prone to duplication of medical records so that it can hinder the service process in the filling section. The purposeof this study is to determine the factors that cause duplication of medical records in order to support the quality of Service in the filling of hospitals in Bandung Kiwari.The research method used is a descriptive method with a qualitative approach. Data collection techniques with observation, literature studies and documentation. Instruments in the study using interview guidelines, recording equipment, stationery, Cheklist sheet.The results ofthe implementation of medical record storage using centralization and numbering of medical records using the unit system (unit numbering system) but for the implementation process is not good enough. Factors causing duplication of medical records in hospitals in Bandung Kiwari is the lack of communication between officers, limited inventory of medical records document folder and medical records of patients who were not found. The effort made by the hospital is to increase the accuracy of the officers through the training of checking back on the expedition SIMRS, and if the medical record is not found then a temporary file is made. It is recommended that communication be improved for coordination and improving the knowledge of employees whose qualifications are not suitable through formal and non-formal education.

Abstract

Based on the results of a preliminary study conducted by the author in July 2024 at Cikalongwetan Regional Hospital, from 50 outpatient medical record files, the author analyzed the medical resume forms in the medical record documents and found that 50 files or 100% of the medical resume forms were not filled in completely. This study aims to determine the effect of the completeness of outpatient medical resumes on the quality of medical records at Cikalongwetan Regional Hospital. The research method used by the author is qualitative research. The results of the research show that the implementation of filling out patient medical resumes at Cikalongwetan Regional Hospital has not been carried out according to the regulations by the relevant officers because there is no SOP (Standard Operational Procedure) which regulates filling out and also the flow of medical resumes so that The patient's medical resume is not filled in 100%. Parts of the form that are not filled in completely include the person in charge, discharge date, admission diagnosis, main diagnosis, diagnosis code, doctor's name, doctor's signature, conditions at the time of discharge and how to go home. The author suggests that it is best to create an SOP regarding the implementation of filling out medical resumes, conducting outreach to DPJP to fill in diagnoses, and actions on patient medical resumes.

Abstract

Health is a very important basic need for everyone. The results of the preliminary study carried out, took a sample of 10 files of informed consent forms from general surgery patients in January. Of the 10 files, there were 7 incomplete informed consent forms (70%). This study aims to determine the completeness of the informed consent form in general surgery cases to support service quality. The research method used is a descriptive method with a quantitative approach. The population in this study was the informed consent form for surgical operations in November-January 2023, namely 351. The samples taken from the population were 78 samples. The sampling technique is incidental sampling. The data collection techniques used were observation, interviews and documentation. The results of the research carried out show that of the 78 samples of informed consent sheets in November-January 2023, namely for completeness in filling out informed consent on patient identification variables, 77 files (98%). Important reporting variables amounted to 64 files (43%). The authentication and identification variables of the person in charge amounted to 66 files (85%). Meanwhile, the good and correct recording variable was 76 files (98%). The completeness results of filling out the informed consent form in the patient's medical record file for surgical operations in November-January 2023 are not 100% complete
