Abstract

Idiopathic pulmonary fibrosis (IPF) is a rare chronic respiratory disease characterized by progressive fibrotic changes in lung tissue of unknown origin, resulting in severe decline in lung function and poor prognosis with a median survival of 3 to 5 years. Current pharmacological therapies, including nintedanib and pirfenidone, aim to slow disease progression but are limited by side effects and lack of efficacy in reversing established fibrosis. This literature review explores emerging therapeutic approaches for IPF using data from PubMed, Google Scholar, and ScienceDirect databases. The review highlights mesenchymal stem cell (MSC) therapy, specifically allogeneic bone marrow-derived MSCs, as a promising option. MSC therapy demonstrates superior efficacy in improving forced vital capacity (FVC) by 3.7%, surpassing the effects of nintedanib (3.3%) and pirfenidone (-4.8%), while exhibiting minimal adverse effects. The findings underscore the potential of MSC therapy as a renewable treatment option for IPF, suggesting a paradigm shift towards addressing both disease progression and lung function restoration in affected individuals.

Abstract

Abstrak Kegiatan pengabdian masyarakat ini bertujuan untuk memberikan edukasi kepada petani tebu di Kampung Lentigi, Kota Belud, Sabah, Malaysia, mengenai dua inovasi berbasis sumber daya lokal, yaitu pemanfaatan limbah daun tebu menjadi silase dan penggunaan ekoenzim sebagai insektisida alami. Petani di daerah tersebut menghadapi masalah berupa penumpukan limbah daun tebu yang belum dikelola secara optimal, serta serangan hama seperti kutu buluh putih, tikus, dan monyet ekor panjang yang turut menimbulkan risiko zoonosis. Kegiatan dilaksanakan melalui pendekatan deskriptif kualitatif dengan metode sosialisasi dan demonstrasi langsung dalam satu kali kunjungan. Tim memperkenalkan proses pembuatan silase menggunakan daun tebu cacah, molase, dan EM4 sebagai starter fermentasi, serta pembuatan ekoenzim dari fermentasi gula, kulit bawang merah, dan air sebagai alternatif pengendalian hama ramah lingkungan. Evaluasi dilakukan melalui observasi terhadap keterlibatan aktif dan respons peserta selama kegiatan. Hasil kegiatan menunjukkan respons positif dari para petani, silase dinilai berpotensi mengurangi limbah sekaligus menyediakan pakan ternak alternatif, sementara ekoenzim dipandang sebagai solusi pengendalian hama yang murah, mudah dibuat, dan lebih aman dibandingkan pestisida sintetis. Kegiatan ini diharapkan dapat meningkatkan pengetahuan, kesadaran, dan keterampilan petani dalam mengelola sumber daya lokal secara berkelanjutan, serta mendukung ketahanan pangan dan kesejahteraan masyarakat Kampung Lentigi. Kata kunci: ekoenzim; insektisida alami; limbah daun tebu; penyuluhan; silase Abstract This community service activity aimed to educate sugarcane farmers in Kampung Lentigi, Kota Belud, Sabah, Malaysia, on two locally-based innovations: the utilization of sugarcane leaf waste as silage for animal feed, and the application of eco-enzyme as a natural insecticide. Farmers in this area face major challenges including unmanaged accumulation of sugarcane leaf waste and pest attacks from white mealybugs, rats, and long-tailed macaques, the latter of which also poses zoonotic disease risks. The activity was conducted using a qualitative descriptive approach through a single-visit socialization and hands-on demonstration session. The team introduced silage production using chopped sugarcane leaves, molasses, and EM4 as a fermentation starter, as well as eco-enzyme production from fermented sugar, onion peel, and water as an environmentally friendly pest control alternative. Evaluation was carried out through direct observation of participant engagement and responses during the session. Results demonstrated positive reception from the farmers — silage was perceived as a practical solution for waste reduction and alternative feed provision, while eco-enzyme was regarded as an affordable, easy-to-make, and safer alternative to synthetic pesticides. This program is expected to enhance farmers' knowledge, awareness, and skills in managing local resources sustainably, while contributing to food security and community welfare in Kampung Lentigi. Keywords: eco-enzyme, natural insecticide, outreach, silage, sugarcane waste
