Multigravida pregnancy (frequent pregnancies) increases the risk of complications. This is because the mother's body becomes more vulnerable and the uterus can experience fatigue. Continuity of Care is a midwife's effort to improve continuous midwifery services in the community. The author was able to provide continuous midwifery care (Continuity of Care) to Mrs. N, 36 years old, G4P3A0, at Mutia Kasih Sani, S. Keb. The method used was descriptive qualitative with a case study approach. The instruments used were documentation studies, literature studies, observations, interviews. Results of Mrs. N's Pregnancy Care Mrs. N, 36 years old, G4P3A0, UK 35 weeks, complained of a tight stomach and leg cramps. She was given health education and recommendations for pregnancy exercises to overcome complaints. Labor Process (September 7, 2025): Contractions began at 13.00 WIB (5 cm dilation). Stage I: counterpressure therapy for pain. At 17.30 WIB: complete dilation, normal delivery care 60 steps. The baby was born at 5:45 PM WIB, crying spontaneously. Stage III: placenta complete. Stage IV: uterine contractions strong, PPV normal. Neonatal Visit: The baby adapted well, suckled smoothly, breast milk flowed normally. Taught baby massage techniques for stimulation. Postpartum Period: Day 1 postpartum, stitch pain (normal for wound healing). Further monitoring without additional complaints. The mother adapted well, independently caring for the baby and daily activities. Taught oxytocin massage to facilitate breast milk flow. Family Planning Care: No complaints, not yet using a birth control method but planning an IUD. Provided education on contraceptive options. Overall, care went smoothly without significant complications, mother and baby adapted optimally. Abstrak Kehamilan multigravida (sering hamil) meningkatkan risiko komplikasi. Hal tersebut dikarenakan tubuh ibu menjadi lebih rentan dan rahim bisa mengalami kelelahan. Contiunity of Care adalah upaya bidan untuk meningkatkan pelayanan kebidanan berkelanjutan di masyarakat. Penulis mampu memberikan asuhan kebidanan berkesinambungan (Continuity Of Care) Pada Ny. N Umur 36 tahun G4P3A0 di Mutia Kasih Sani, S. Keb. Metode yang digunakan diskriptif kualitatif dengan pendekatan case study. Intrumen yang digunakan adalah studi dokumentasi, studi pustaka, obsevasi, wawancara. Hasil Asuhan Kehamilan Ny. N Ny. N, 36 tahun, G4P3A0, UK 35 minggu, mengeluh perut kencang dan kaki kram. Diberikan penkes serta anjuran senam hamil untuk mengatasi keluhan. Proses Persalinan (7 September 2025): Mulai kontraksi jam 13.00 WIB (pembukaan 5 cm). Kala I: terapi counterpressure untuk nyeri. Jam 17.30 WIB: pembukaan lengkap, asuhan persalinan normal 60 langkah. Bayi lahir jam 17.45 WIB, menangis spontan. Kala III: plasenta lengkap. Kala IV: uterus kontraksi keras, PPV normal. Kunjungan Neonatal: Bayi adaptasi baik, menyusu lancar, ASI keluar normal. Diajarkan teknik pijat bayi untuk stimulasi. Masa Nifas: Hari 1 postpartum, nyeri jahitan (normal pada penyembuhan luka). Pemantauan selanjutnya tanpa keluhan tambahan. Ibu adaptasi baik, mandiri merawat bayi dan aktivitas harian. Diajarkan pijat oksitosin untuk lancarkan ASI. Asuhan KB: Tidak ada keluhan, belum pakai metode KB tapi rencana IUD. Diberikan edukasi pilihan kontrasepsi. Secara keseluruhan, asuhan berjalan lancar tanpa komplikasi signifikan, ibu dan bayi adaptasi optimal.
