Abstract

Waiting time for patient discharge is a key indicator in determining the quality of hospital services. In an effort to improve waiting time for patient discharge, some hospitals recruit insurance relation doctors to help check discharge paperwork, especially for patients with insurance coverage. This study aims to see the role of insurance relation doctors in reducing the waiting time for inpatient discharge. This study is an observational study with a cross-sectional study design. Waiting time data was taken through the hospital system and analyzed univariately and bivariately using the independent t-statistical test. Univariate data showed a decrease in the waiting time for inpatient discharge by 5% after the insurance relation doctor joined. However, the statistical test results showed that there was no significant relationship (CI 95% p>0.05) between the joining of insurance relation doctors and the decrease in the waiting time for inpatient discharge. The role of insurance relations doctors has yet to have a significant effect in reducing waiting time for discharge, possibly due to the short duration of the intervention, the involvement of many parties, and the workload of insurance relations doctors. Further research is needed regarding the details of the inpatient discharge process and the productivity of insurance relations doctors in carrying out their roles in the hospital.

Abstract

Background: Remote public health centers (PHCs) have the highest proportion of PHCs without doctors, with Southeast Sulawesi Province ranking fifth in this regard. PHCs serve as the main gateway to health services in rural and remote areas. Despite its high fiscal capacity and provision of support for medical education and incentives, North Konawe District has not succeeded in retaining doctors in its public health centers. Aims: This study aims to analyze factors that influence the retention of doctors in remote PHCs in the North Konawe District. Methods: This qualitative study employed a case study design, involving in-depth interviews with 14 informants and a review of six documents. Results: Factors affecting doctor retention in the North Konawe District include individual factors, work factors, living environment factors, and health system factors. Conclusion: Doctors in remote PHCs in North Konawe District are Ministry of Health placement doctors working on temporary contracts. Strategies to improve the retention of doctors in remote PHCs include improving living conditions in remote areas, supporting job opportunities for doctors' spouses, recruiting doctors early in their careers, implementing government disincentive policies, building PHCs with official housing, requiring mandatory service for recipients of medical education scholarships, and developing doctor attendance information systems. Keywords: Doctor, PHC, remote area, retention.

Abstract

Adequacy of doctors through the deployment of public health center doctors to achieve equity in health services for the community. The deployment of public health center doctors has been implemented through various policies but there are still public health centers that do not have a doctor. This article aims to determine the strategies, challenges, and successes in the distribution of public health center doctors. Document analysis of legal documents was approached using the READ method to determine the distribution strategy of doctors. A Systematic Literature Review (SLR) was conducted using the PRISMA protocol from two online database search engines, where 25 articles were found that met the research objectives, inclusion, and exclusion criteria. The challenges of the distribution of public health center doctors are due to deployment for remote and very remote areas. Factors that challenge the distribution of doctors are individual factors, work factors, living environment factors, and health system factors. Recruitment and deployment of public health center doctors in remote areas need to be done comprehensively with crossector.

Abstract

The quality of health services at the health center is greatly influenced by health equipment. Efforts to maintain health equipment so that it functions properly and is suitable for use include carrying out maintenance including calibration of medical devices. In 2022, the calibration achievement of health centers is still very low compared to the achievement of hospital calibration (health centers 34.1% and hospitals 70.2%). One of the factors causing the low calibration implementation is the lack of budget availability. In 2023, the government provided budget support for health center calibration through the Health Sector Nonphysical Special Allocation Fund. This study aims to determine the achievement of the implementation of medical device calibration at health centers in Indonesia in 2022 and 2023 and the effect of the Nonphysical Special Allocation Fund for the health sector in 2023 on improving the implementation of calibration at health centers. This study is a quantitative study with a cross-sectional design. Data collection used secondary data with a total sampling of 10,374 health centers. Data analysis techniques were carried out univariate and bivariate using SPSS.  Univariate analysis showed an increase in the proportion of health centers that calibrated medical devices by 22.6% from 34.1% in 2022 to 56.7% in 2023. Similarly, the proportion of calibrated medical devices increased by 3.5% from 6.6% in 2022 to 11.6% in 2023. Bivariate analysis using the T-test obtained a p-value = 0.008 and the Chi-Square test obtained a p-value = 0.002, indicating an association between the Nonphysical Special Allocation Fund for the Health Sector and the increase in the calibration achievement of puskesmas in 2023.

Abstract

The Hospital Infection Prevention and Control Program (HIPCP) is an effort to reduce the risk of Healthcare-Associated Infections (HAIs), including hand hygiene compliance. WHO issued the Multimodal Hand Hygiene Improvement Strategy as one of the strategies to tackle low hand hygiene compliance. Dr. M. Goenawan Partowidigdo Pulmonary Hospital (RSPG) has implemented hand hygiene regulations referring to the current policies, but it has not met the target over the last three years. This study aims to analyze the implementation of the regulation of hand hygiene compliance at RSPG according to the WHO Multimodal Hand Hygiene Improvement Strategy, using an analytical descriptive qualitative approach with a case study method. The analysis uses the combination of the theory of George Edward III, Weaver, and the WHO Multimodal Hand Hygiene Improvement Strategy. WHO Multimodal Hand Hygiene Improvement Strategy assessment was conducted by scoring the Hand Hygiene Self-Assessment Framework (HHSAF). The results showed the lowest percentage of HHSAF scores on the bureaucratic structure variable, but the most important variable was the resource variable, namely human resources, related to activeness and behavior issues. These issues were obtained from in-depth interviews developed from structured questions on HHSAF. Hand Hygiene of RSPG is at an intermediate level. Researchers recommend behavioral enforcement with appreciative inquiry in addition to reward and punishment as an effort to improve compliance with the implementation of hand hygiene regulations at RSPG.

Abstract

Evaluation of Medical Check Up (MCU) services with the implementation of information systems in hospitals is very important to identify the effectiveness and efficiency of the system used in the health services process. This study aims to evaluate various aspects of the implementation of information systems in MCU services, including the speed and efficiency of services, data integration, user satisfaction (patients and medical personnel), and the level of patient data security. The method used in this study is a qualitative approach with in-dept interview, direct observatio, and document analysis. Interview were conducted involving 30 informants, namely 4 general practitioners, 1 spesialist doctor, 5 health staff, 5 Medical Check Up nurses, 5 management, 5 administrative officer, and 5 participants who have undergone MCU. The results of the study indicate that the implementation of information systems in MCU serviceshas accelerated the administration process and increased the accuracy of patient data management. However, several technical obstacles, such as registration problems and suboptimal system integration., are still obstaclesin the implementation of services. In addition, although patients are satisfied with the ease of access to informationthrough the online portal, some of them feel that they do not fully understand the use ofthe system. Based on these findings, it is recommended to improve staff training, create integration between information systems with laboratories, radiology and strengthen the security of patient medical data. This research is expected to provide insight into development and improvement of information systems in MCU services in Permata Cibubur hospital to achieve more efficient, safe and quality services.
