Abstract

HIGHLIGHTS 1. This review focuses on maximizing the effectiveness of "Kartu Skor Poedji Rochyati" (KSPR) in Indonesia's maternal referral system.2. It addresses the lack of information about Indonesia's maternal referral system, offering valuable insights.3. Underlining the high rates of maternal and fetal deaths due to referral errors, the article stresses the urgency of understanding KSPR's function for high-risk pregnant women.   ABSTRACT Maternal Mortality Rate (MMR) represents fatalities during pregnancy, childbirth, and the postpartum period due to complications, not external factors. Timely detection is crucial for preventing maternal deaths, necessitating integrated planning. The "Kartu Skor Poedji Rochjati" (KSPR) emerges as a vital tool for identifying high-risk pregnancies, facilitating urgent referrals to mitigate complications and enhance healthcare. A literature review on maternal mortality, risk assessment tools, and KSPR effectiveness was conducted to address the issues. The findings are expected to highlight KSPR's pivotal role in identifying at-risk pregnant women, enabling prompt referrals, and reducing maternal mortality rates. The case studies and data analysis will enrich our understanding of the tool's impact on maternal healthcare. The apex of public health involves embracing health-conscious lifestyles, reflected in indicators like mortality and morbidity. Proactively identifying high-risk pregnancies using KSPR is a strategic step in averting maternal mortality. Leveraging KSPR allows early intervention, reducing complications and contributing to enhanced maternal health, aligning with the goal of minimizing mortality risks.

Abstract

Objective: to analize how freeze-dried amniotic membrane and human amniotic stem cell seeding affect TGF B and type III collagen expression in suturing a New Zealand rabbit vesicovaginal fistula model. Metode: This experimental study employed New Zealand rabbits and a vesicovaginal fistula model with a post-test only control group design. The rabbits were divided into 3 groups: vesicovaginal fistula suturing alone, suturing with freeze-dried amniotic membrane, and suturing with freeze-dried amnion-seeded stem cells. After 7 days of treatment, specimens near the repaired vesicovaginal fistula were collected for immunohistochemical analysis of TGF B and collagen type III expression. Result: TGF B expression was significantly higher in the freeze-dried amniotic membrane with stem cell seeding group (p=0.001) compared to the freeze-dried amniotic membrane without stem cell seeding group (p=0.017) and the suturing-only group (p=0.049). Additionally, type III collagen expression was significantly elevated in the freeze-dried amnion membrane and stem cell seeding group (p=0.001) compared to the freeze-dried amnion group without stem cell seeding (p=0.09) and the suturing-only group (p=0.026). Conclusion: The expression of TGF B and type III collagen was higher in rabbits with vesicovaginal fistulas treated using freeze-dried amnion and amniotic stem cell seeding compared to those without amniotic stem cell seeding and vesicovaginal fistula suturing alone. Abstract

Latar belakang: Seksio sesarea (SC) merupakan prosedur bedah yang sering dilakukan untuk mengatasi komplikasi obstetri. Namun, nyeri pasca SC menjadi tantangan utama yang dapat memperlambat pemulihan dan meningkatkan risiko nyeri kronis. Manajemen nyeri yang efektif diperlukan untuk mengoptimalkan hasil pascaoperasi. Artikel ini bertujuan merangkum pendekatan terkini manajemen nyeri pasca seksio sesarea secara farmakologis, regional, dan non-farmakologis untuk mendukung pengendalian nyeri yang efektif. Isi: Nyeri pasca SC bersifat nosiseptif dan neuropatik, memerlukan pendekatan multimodal yang mencakup terapi farmakologis dan non-farmakologis. Obat seperti OAINS, opioid, dan anestesi lokal terbukti efektif, sementara teknik regional seperti morfin intratekal memberikan analgesia jangka panjang. Terapi non-farmakologis, termasuk CBT dan TENS, juga berperan dalam mempercepat pemulihan. Kesimpulan Manajemen nyeri pasca SC sangat penting untuk mempercepat pemulihan dan mengurangi risiko nyeri kronis. Pendektakan terapi multimodal efektif mengurangi morbitas pasien paska SC.

Abstract

In order to reduce maternal and infant mortality rates, interventions must focus on improving the quality of antenatal care, particularly at primary care level. One key competency that general practitioners must possess is the ability to perform basic obstetric ultrasound examinations to detect intrauterine foetal abnormalities early on. This community service activity aims to enhance general practitioners' skills in this area using a six-step approach. Implementation involves theoretical education, hands-on training and pre- and post-test evaluations of participants' knowledge and skills. The activity involved 30 general practitioners from the Kampar Regency area. Training results showed an average increase in post-test scores of 31.67% compared to pre-test scores, indicating significant improvements in participants' competencies. The activity also facilitated the distribution of practice- based teaching modules and strengthened collaboration with local community health centres to support maternal and child health services. Basic obstetric ultrasound training has proven to be an effective way of strengthening primary healthcare services and reducing maternal and perinatal mortality and morbidity rates.
