The Role of Systemic Inflammation in COPD Severity: Insights from FEV1 and hs-CRP Julike, Katarina; Mery Silvia Harahap

Abstract

Chronic obstructive pulmonary disease (COPD) is a complex inflammatory disease characterized by progressive airflow limitation. While pulmonary inflammation is a hallmark, the role of systemic inflammation in COPD severity remains an area of active investigation. This study aimed to explore the relationship between lung function, assessed by Forced Expiratory Volume in 1 second (FEV1), and systemic inflammation, measured by high-sensitivity C-reactive protein (hs-CRP), in patients with stable COPD. A cross-sectional study was conducted involving 51 patients with stable COPD. Lung function was assessed using spirometry, and hs-CRP levels were measured using the Architect tool. The relationship between FEV1 and hs-CRP was analyzed using Spearman's rank correlation test. The study population consisted predominantly of older males (mean age 64.05 ± 8.05 years) with moderate to severe airflow limitation (mean FEV1 33.65 ± 15.78%). All patients had hs-CRP levels within the normal range (<10 mg/L), with a median of 0.34 mg/L. A significant negative correlation was observed between FEV1 and hs-CRP (r = -0.260, p = 0.032), indicating that patients with worse lung function tended to have higher levels of systemic inflammation. The findings suggest that even in stable COPD, systemic inflammation, as reflected by hs-CRP, is associated with lung function impairment. This highlights the potential role of systemic inflammation in COPD progression and underscores the need for further research to elucidate the complex interplay between pulmonary and systemic inflammation in this disease.

Abstract

This study aims to analyze factors related to the implementation of patient safety culture at UPTD Puskesmas Deli Tua. An analytical survey with a cross-sectional approach was conducted involving 75 respondents. Data were collected using structured questionnaires covering leadership roles, staff arrangement, incident reporting, and communication. The analysis of chi square revealed significant relationships between patient safety culture and leadership roles (p=0.002; PR=2.227), staff arrangement (p=0.004; PR=2.041), incident reporting (p=0.01; PR=1.834), and communication (p=0.007; PR=1.929). These findings highlight the importance of these four factors in fostering a supportive environment for patient safety. This study recommends strengthening leadership, optimizing staff arrangement, improving incident reporting systems, and enhancing effective communication to better implement patient safety culture. AbstrakPenelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan penerapan budaya keselamatan pasien di UPTD Puskesmas Deli Tua. Penelitian menggunakan desain survei analitik dengan pendekatan cross-sectional dan melibatkan 75 responden. Data dikumpulkan menggunakan kuesioner terstruktur yang mencakup peran kepemimpinan, penyusunan staf, pelaporan insiden, dan komunikasi. Hasil analisis chi square menunjukkan bahwa peran kepemimpinan (p=0,002; PR=2,227), penyusunan staf (p=0,004; PR=2,041), pelaporan insiden (p=0,01; PR=1,834), dan komunikasi (p=0,007; PR=1,929) memiliki hubungan signifikan dengan penerapan budaya keselamatan pasien. Hasil ini menunjukkan bahwa keempat faktor tersebut berkontribusi penting dalam menciptakan lingkungan kerja yang mendukung keselamatan pasien. Penelitian ini merekomendasikan penguatan kepemimpinan, optimalisasi penyusunan staf, peningkatan sistem pelaporan insiden, dan komunikasi efektif untuk mendukung penerapan budaya keselamatan pasien yang lebih baik.

Abstract

Demam berdarah adalah penyakit yang disebabkan oleh virus dengue dan ditularkan ke manusia melalui gigitan nyamuk Aedes aegypti. Hati merupakan tempat replikasi virus dengue yang utama dan akan menjadi keterlibatan hati yang fatal. Kerusakan pada hati dapat dinilai salah satu keberadaannya dengan pemeriksaan enzim transaminase yaitu SGOT (Serum Glutamic Oxaloacetic Transaminase) dan SGPT (Serum Glutamic Pyruvic Transaminase), enzim tersebut dikeluarkan bila terjadi kerusakan jaringan dan sel hati. Penelitian ini bertujuan untuk mengetahui gambaran SGOT dan SGPT pada pasien demam berdarah di Rumah Sakit Columbia Asia Medan. Jenis penelitian observasional analitik dengan menggunakan pendekatan cross sectional yang dilakukan untuk melihat hubungan antara variabel satu dengan variabel yang lain. Pengambilan sampel dilakukan dengan teknik random sampling. Jumlah sampel dalam penelitian ini adalah 20 responden. Data dianalisis menggunakan uji paired t test dengan α 0,05. Hasil penelitian menggunakan analisis bivariat menunjukkan bahwa terdapat pengaruh antara faktor-faktor yang berhubungan dengan mutu pelayanan kesehatan yaitu dari 20 responden terdapat 16 responden (75%) hasil pemeriksaan SGOT dan SGPT kategori tinggi terhadap pasien demam berdarah kategori tinggi, sementara 4 responden (20%) hasil pemeriksaan SGPT terhadap pasien demam berdarah kategori normal. Kesimpulan: Gambaran SGOT dan SGPT pada Pasien Demam Berdarah di Rumah Sakit Columbia Asia Medan adalah signifikan

Abstract

In general, people use plant parts which include roots, bark, leaves, flowers or seeds as medicinal ingredients. Chinese ketepeng (Cassia alata L.) is one of the plants that is often used by the community as traditional medicine. This Chinese ketepeng plant is known as the wax tree, wax bush and ringworm plant. Chinese ketepeng leaf is one part that is often used by the community to treat skin infections caused by fungi, such as tinea versicolor, ringworm, thrush, water fleas and others. Tannins are organic compounds that are very complex and consist of phenolic compounds found in various plants. Tannins are considered as a group of compounds that attract applications in terms of potency and environmental prediction. The analytical method used is qualitative using FeCl3 reagent, gelatin and Folin Ciocalteu, while quantitative testing is using UV-Vis spectrophotometry with a wavelength of 725 nm with gallic acid as a comparison raw material. The results of the qualitative analysis of the positive samples contained tannins. The quantitative results of the ethanol extract of Chinese ketepeng leaves were 8.71 mg/g. Based on the results of research that has been carried out, tannin compounds in the ethanol extract of the leaves of the Chinese Ketepeng (Cassia Alata L.) can be determined using UV-Vis spectrophotometry using gallic acid as a standard.
