Abstract

This research explores the potential use of DCM4CHEE as an open-source Picture Archiving and Communication System (PACS) that is highly flexible for development in the current era. Despite being open source, DCM4CHEE has proven to be effective and reliable in healthcare settings, particularly in radiology department. Primaya Hospital Semarang has successfully implemented DCM4CHEE as an integrated archive system directly connected to CT Scan and Conventional X-Ray modalities. Direct observations were conducted over a significant period at the radiology installation of Primaya Hospital Semarang, which utilizes DCM4CHEE. This provided direct insights into how the system is implemented and used in practical situations. Furthermore, in-depth interviews were conducted with radiographers to gain their perspective on the experience of using DCM4CHEE. The research results indicate that the use of DCM4CHEE contributes positively in various aspects, including user-friendly operation, adequate utility, workflow improvement, and cost savings in healthcare services. The implementation of this software not only aids in the efficient management of radiological data but also reduces dependence on traditional filming systems, supporting the transition towards a paperless workflow. In conclusion, DCM4CHEE is reliable and highly beneficial in the context of healthcare services. Its adaptability allows seamless integration with Hospital Information Systems (HIS) and Radiology Information Systems (RIS), enhancing overall interoperability and efficiency. This research provides a foundation for recommending the use of DCM4CHEE as a customizable PACS solution that can positively contribute to improving the quality of radiology services in various healthcare department.

Abstract

Background: One aspect of digital transformation in education today is online learning or commonly referred to as E-learning. Clinical Education Management System or CLEMAGS, is an application system used for clinical education management in the field of web-based Diagnostic Imaging. The development of Clinical Learning Management System (CLEMAGS), which was built with the help of Orthanc Platform, is to create a more inventive and interactive learning model for students of Postgraduate Diagnostic Imaging study program of Poltekkes Kemenkes Semarang. Designing and developing the PACS Server Post Processing model in CLEMAGS to support activities in the Diagnostic Imaging Postgraduate Program of the Poltekkes Kemenkes Semarang and testing the effectiveness of the PACS Server Post Processing model in CLEMAGS in the Diagnostic Imaging Postgraduate Program of the Poltekkes Kemenkes Semarang.Methods: This type of research is Research and Development (RD) which uses the Rapid Application Development (RAD) research approach method. Analysis of differences in pre-test and post-test data using paired sample t-test. While analyzing the increase in effectiveness test results using the N-Gain Test.Results: The paired sample t-test results are sig. value of 0.003 (p-value 0.05), which means that there is a significant difference between the pre-test data and the post-test implementation of the PACS Server Post Processing Model in CLEMAGS. Overall, the average N-Gain score is 0.89 and is included in the “High” understanding improvement category. The percentage of N-Gain obtained was 89.94% and included in the effective category.Conclusions: The PACS Server Post Processing model in CLEMAGS is effective in improving user performance in managing medical imaging data. The TRIP-ID system is a medical image-based learning solution that can be practically applied in health education institutions. Further development can be directed toward the integration of artificial intelligence (AI) for automatic interpretation, as well as expanding implementation in various other educational institutions.

Abstract

Gadget addiction is one of the negative effects of excessive gadget use. Currently, there are many studies investigating internet addiction using a neuroimaging approach using diagnostic modalities. One of the modalities that can be used is Magnetic Resonance Imaging (MRI). One of the developments in MRI is the Magnetic Resonance Spectroscopy Imaging or MRS examination. MRS is a radiological examination that uses MRS supporting software in the MRI modality which can show correlations of anatomical and physiological metabolic or biochemical information present in the body, both in normal and abnormal conditions. The purpose of this study was to analyze the results of metabolite values in adolescents with suspected gadget addiction to prevent and support therapeutic research using MRS. The method in this study is descriptive qualitative research used to determine the results of metabolite values in adolescents with suspected gadget addiction to prevent and support therapeutic research using MRS with the single voxel. The sample used is teenagers with suspected gadget addiction. The result of the MRS examination can show the value of metabolites in the adolescent's body. The results of the MRS are in the form of graphs of metabolite values. Normal metabolite values will produce Hunter's Angle. The graph of teenagers with suspected gadget addiction does not form a Hunter's Angle. The values of NAA (5,49), Cho (3,63), Cr (3,65) and Glx (1.06) in adolescents with suspected gadget addiction decreased from the normal values of NAA (10-15), Cho (8), Cr (7) and Glx value (5-10).

Abstract

Magnetic Resonance Imaging (MRI) in pediatric patients presents significant challenges due to the difficulty children often experience in remaining still and cooperative during the procedure. Monitored Anesthesia Care (MAC) is frequently employed to address these challenges, ensuring both patient comfort and optimal imaging quality.This study aims to provide a detailed overview of the procedural protocol and potential adverse effects associated with the use of MAC in pediatric MRI examinations. A qualitative descriptive analysis was conducted based on a clinical case from the radiology department of a tertiary care hospital in Indonesia, supported by relevant literature review. Key procedural steps include pre-procedure assessment, laboratory testing, intravenous line placement, sedation administered and monitored by an anesthesiologist, continuous vital signs monitoring, airway management, and post-anesthesia recovery. Potential complications identified include allergic reactions to gadolinium-based contrast agents, local injection site bruising, hypothermia, airway obstruction, laryngospasm, post-sedation agitation, confusion, respiratory depression, and heightened anxiety due to the confined MRI environment. The report highlights the necessity of strict adherence to safety protocols, comprehensive patient preparation, and the availability of emergency equipment and trained personnel to mitigate risks. In conclusion, the use of MAC in pediatric MRI is a safe and effective strategy when performed with proper planning and multidisciplinary collaboration, enhancing diagnostic accuracy and patient safety.
