Abstract

Introduction: Monkeypox, a zoonotic viral disease endemic to Central and West Africa, has recently gained global attention due to outbreaks beyond its traditional range. Understanding the genomic diversity of monkeypox virus (MPXV) isolates, particularly those from Sudan, is crucial for effective public health surveillance and vaccine development. Methods: In this study, we employed comparative genomics to analyze MPXV isolates from Sudan and other endemic regions. We utilized publicly available genomic sequences from NCBI's GenBank database. Phylogenetic analysis, sequence alignment, and identification of single nucleotide polymorphisms (SNPs) were performed to assess genetic diversity and identify potential geographic or temporal clustering. Results: Our analysis revealed distinct genetic lineages of MPXV circulating in Sudan compared to other endemic regions. Specific SNPs and genomic signatures were identified that could serve as potential markers for differentiating Sudanese isolates. Notably, some Sudanese isolates exhibited closer genetic relatedness to isolates from recent outbreaks outside of Africa, suggesting potential transmission pathways. Conclusion: This study provides valuable insights into the genomic diversity of MPXV in Sudan and other endemic regions. The identification of Sudan-specific genomic signatures highlights the importance of considering regional variations in public health surveillance and vaccine development efforts. Continued genomic surveillance is essential for monitoring the evolution of MPXV and detecting potential emerging variants with altered virulence or transmissibility.

Abstract

Introduction: Gender dysphoria among adolescents is increasingly recognized globally, yet research on the lived experiences of transgender youth in Indonesia remains limited. This study aimed to explore the unique challenges, coping mechanisms, and support needs of Indonesian adolescents navigating gender dysphoria within a sociocultural context characterized by traditional gender norms and limited access to specialized healthcare. Methods: In-depth, semi-structured interviews were conducted with 15 adolescents (aged 13-19) who identified as experiencing gender dysphoria. Participants were recruited through LGBTQ+ youth organizations and community centers in Jakarta and Surabaya. Data were analyzed using thematic analysis to identify key themes and patterns within their narratives. Results: Four major themes emerged: (1) Early Recognition and Internal Conflict: Participants described an early awareness of their gender identity incongruence, often accompanied by feelings of confusion, shame, and fear due to societal expectations and potential rejection. (2) Navigating Disclosure and Social Support: Coming out to family and peers was a significant challenge, with varied responses ranging from acceptance to rejection. Support networks, primarily found within online communities and LGBTQ+ organizations, played a crucial role in fostering resilience. (3) Barriers to Accessing Healthcare: Participants reported significant obstacles in accessing gender-affirming care, including limited availability of specialized healthcare providers, financial constraints, and lack of family support. (4) Resilience and Hope for the Future: Despite the challenges, participants expressed resilience, hope, and a strong desire to live authentically. They emphasized the importance of self-acceptance, peer support, and advocacy for greater understanding and acceptance of gender diversity in Indonesia. Conclusion: This study provides valuable insights into the experiences of gender dysphoric adolescents in Indonesia. Findings highlight the urgent need for increased awareness, improved access to gender-affirming care, and supportive interventions to promote the well-being of this vulnerable population.

Abstract

Introduction: Sudden sensorineural hearing loss (SSNHL) is an otologic emergency characterized by an unexplained, rapid loss of hearing, usually in one ear. Identifying prognostic factors for hearing recovery is crucial for guiding treatment strategies and patient counseling. This study aimed to determine the prognostic factors associated with hearing recovery in a cohort of SSNHL patients in Singapore. Methods: A retrospective cohort study was conducted involving 150 patients diagnosed with SSNHL at three tertiary referral hospitals in Singapore between January 2019 and December 2022. Data on demographics, clinical characteristics (degree and configuration of hearing loss, presence of vertigo and tinnitus), treatment modalities (corticosteroids, hyperbaric oxygen therapy), and hearing outcomes at 1, 3, and 6 months following diagnosis were collected. Hearing recovery was defined as an improvement of at least 15 dB in pure-tone average (PTA) across frequencies 0.5, 1, 2, and 4 kHz. Multivariate logistic regression analysis was performed to identify independent predictors of hearing recovery. Results: The mean age of the participants was 47.5 ± 12.8 years. Complete hearing recovery was observed in 48% of patients at 6 months. Multivariate analysis revealed that younger age (OR = 0.95, 95% CI: 0.92-0.98, p=0.003), absence of vertigo (OR = 3.15, 95% CI: 1.52-6.53, p=0.002), and prompt initiation of corticosteroid therapy (OR = 2.87, 95% CI: 1.38-5.97, p=0.005) were independent predictors of favorable hearing recovery. The degree of initial hearing loss and the presence of tinnitus were not significantly associated with hearing outcomes. Conclusion: Younger age, absence of vertigo, and early initiation of corticosteroid therapy are associated with better hearing outcomes in Singapore patients with SSNHL. These findings underscore the importance of prompt diagnosis and treatment to maximize the chances of hearing recovery.

Abstract

Platelet-rich plasma (PRP) has emerged as a promising therapeutic modality for various medical applications, particularly in regenerative medicine and wound healing. This is largely attributed to its rich concentration of growth factors (GFs) that play pivotal roles in tissue repair and regeneration. However, the inherent limitations of PRP, such as the variable GF concentrations and short-lived release kinetics, have spurred the development of advanced modalities to enhance its therapeutic efficacy. Activated growth factor (AGF) represents one such advancement, aiming to optimize the release and bioavailability of GFs from platelets. This comprehensive review delves into the biological mechanisms underlying AGF, its preparation methodologies, preclinical and clinical evidence supporting its use, and its potential applications in treating degenerative and traumatic conditions. Furthermore, it explores the advantages of AGF over conventional PRP and discusses future directions for research and clinical translation.
