Abstract

ABSTRACT Introduction: Anterior implant placement is always challenging, especially when it is caused by trauma. Treatment preference is not only because of bone sufficiency but also aesthetic considerations. The complications aggravated by trauma and temporomandibular joint disorder need particular emphasis. The aim of this case report is to describe a new insight into dental implant treatment for trauma-induced edentulism and associated temporomandibular joint disorders. Case report: A 24-year-old woman had a motor vehicle accident. Her four anterior mandibular teeth were extracted through fractures. The patient also complained of clicking in both jaws during mouth opening. A bone graft and modification provisoris-stabilization splint were applied before definitive treatment. Furthermore, a 4-unit Porcelain Fused to Metal (PFM) implant-supported-fixed partial denture was done to replace the missing teeth. Conclusion: New insight into dental implant treatment for trauma-induced edentulism and associated temporomandibular joint disorders with replacement of several anterior mandibular missing teeth due to trauma with temporomandibular disorders could be treated with dental implant modification splint that functions as a temporary restoration as well, which was an alternative treatment option. The patient was satisfied with the result of the treatment, especially in the aesthetic aspect.KEYWORDSdental implant, trauma, temporomandibular joint disorders, splint

Abstract

Pendahuluan: Bruxism adalah aktivitas parafungsional yang sering dikaitkan dengan kebiasaan clenching, gnashing, dan grinding antar gigi dan dilakukan pada saat tersadar ataupun tertidur. Kebiasaan bruxism jika dilakukan terus menerus dapat menyebabkan terjadinya pemendekan dimensi vertikal dan penambahan lebar bigonial mandibula. Tujuan penelitian ini adalah untuk menganalisis mengenai proporsi wajah antara penderita bruxism dengan non bruxism. Metode: Penelitian ini dilakukan dengan metode cross sectional pada 40 orang yang merupakan pasien RSGM Unpad, terdiri dari 20 orang bruxism (perempuan 16 orang dan laki-laki 4 orang) dan 20 orang non bruxism (perempuan 16 orang dan laki-laki 4 orang). Data diperoleh dengan memotret wajah pasien bruxism dan non bruxism di Instalasi Prostodonsia RSGM Unpad. Hasil foto dianalisis menggunakan aplikasi Photoshop dan dihitung menggunakan metode horizontal thirds dan vertical fifths. Uji yang dilakukan pada penelitian ini adalah uji statistik parametrik t-test independent  (uji t).  Hasil: Analisis foto wajah menunjukkan bahwa penderita bruxism lebih banyak pada wanita dengan tinggi sepertiga wajah bagian bawah, lebih pendek, yaitu rata-rata 3,11 cm sedangkan non bruxism rata-rata 3,52 cm dan signifikan secara statistik. Simpulan: Proporsi sepertiga wajah bagian bawah penderita bruxism lebih pendek dibandingkan dengan non bruxism, ditinjau dari hasil fotografi.Differences in facial proportions between bruxism and non bruxism patients: a cross sectional study Introduction: Bruxism is a parafunctional activity commonly associated with clenching, gnashing, and grinding of teeth, occurring both during wakefulness and sleep. Persistent bruxism can lead to a reduction in vertical dimension and an increase in mandibular bigonial width. This study aims to compare facial proportions of individuals with bruxism to those without. Methods: This cross-sectional study was conducted on 40 patients at RSGM Unpad, comprising 20 bruxism patients (16 females and 4 males) and 20 non-bruxism patients (16 females and 4 males). Data were collected by photographing the faces of both bruxism and non-bruxism patients at the Prosthodontics Department of RSGM Unpad. The photographs were analyzed using Photoshop and measured according to the horizontal thirds and vertical fifths methods. The statistical analysis performed in this study was a parametric test using the independent t-test. Results: Facial photo analysis revealed that bruxism was more prevalent among females. The lower third of the face in bruxism patients was significantly shorter, averaging 3.11 cm, compared to 3.52 cm in non-bruxism patients, a statistically significant difference. Conclusions: Photographic analysis revealed that the lower third of the face in bruxism patients is significantly shorter than in non-bruxism individuals

Abstract

Introduction: Loss of an eyeball due to enucleation or phthisis bulbi results in functional deficiency and facial deformities. This condition can cause significant psychological distress, especially if it results from unexpected infections or trauma. Case Report: A 30-year-old male patient presented to the Prosthodontic Clinic at Universitas Padjadjaran's Dental Hospital with complaints of low self-confidence due to his eye condition. The patient lost his left eye in childhood due to trauma and subsequently underwent surgical removal. Examination revealed a healthy conjunctiva with no signs of infection. Meanwhile, the right eye had experienced an infection six years ago but had not been removed. Examination showed a shrunken eyeball with vision loss. Bilateral custom ocular prostheses were fabricated for aesthetic purposes.  Disscusion: Maintaining a healthy socket condition, preserving the ability of eye muscles to blink, normal eyelash positioning, preventing superior or inferior eyelid malposition, and maintaining normal eyelid closure post-enucleation or phthisis bulbi are crucial factors for the creation of an ideal ocular prosthesis. This case report outlines socket rehabilitation in a case of anophthalmic ocular sinistra and phthisis bulbi ocular dextra, focusing on preserving socket shape and support from muscles. Conclusion: Precisely crafted ocular prostheses offer advantages in retention, stability, aesthetics, and comfort for patients with anophthalmic sockets following enucleation and phthisis bulbi.

Abstract

Introduction: The loss of anterior teeth can lead to aesthetic, phonetic, and functional issues. A lithium disilicate adhesive bridge can be a good alternative and a viable compromise in cases of anterior tooth loss. The advantages include minimally invasive preparation, adhesive system, better retention, and aesthetics compared to metal materials. Removable orthodontic treatment is a preliminary option for cases of minor tooth malposition to achieve an ideal dental arch. This case report describes the management sequence for a missing incisor with tooth malposition using removable orthodontic appliances and modified design adhesive bridge. Case report: A 19-year-old female patient visited Dental Hospital Universitas Padjadjaran, Bandung with an issue of missing left central incisor due to traffic accident, and the right central incisor being in a crossbite position. The patient wanted to use a fixed dental prosthesis to improve her appearance but had not yet undergone correction for the misaligned tooth. Discussion: The patient was provided with a temporary removable dental prosthesis with removable orthodontic modifications including the addition of bite risers. Subsequently, a modified design adhesive bridge with lithium disilicate material was planned. Conclusion: The combination of removable orthodontic treatment with a final restoration utilizing a modified design adhesive bridge using lithium disilicate material for the upper anterior teeth can enhance patient satisfaction by addressing aesthetic concerns and improving phonetic function and chewing ability.
