Sustainable Midwifery Care (Continuity Of Care) In The Preconception Period in The Working Area Of The Pagambiran Health Center Padang City Yulizawati, Yulizawati; Aprila, Prety Zinta; Intasir, M. Pemberdi

Abstract

Background: Based on World Health Organization (WHO) data in 2021 that Maternal Mortality Rate (MMR) in the world caused by complications pregnancy and childbirth reached 295,000 (810 per day) people. Meanwhile, The maternal mortality rate in Indonesia recorded in the Indonesian health profile in 2021 is 7,389 mothers in West Sumatra Province. The maternal mortality rate also increased from 125 people in 2020 to 193 people in 2021. This phenomenon is not in accordance with Indonesia's Sustainable Development Goals (SDGs), which states that the maternal mortality rate target is a maximum of 70 deaths per 100,000 live births by 2030. So, the implementation of midwifery Continuity of Care (CoC) is very important as promotional and preventive step to prevent mother complication during pregnancy. Purpose: Providing CoC midwifery care to women during the preconception period. Methods: Implementation of midwifery care in a continuity of care manner with assessment according to midwifery science aims to help women solve the problems they face, with various steps consisting of basic data assessment, data interpretation, potential diagnosis, immediate action, drawing up a comprehensive care plan, implementation, and evaluation, as well as recording through SOAP documentation. S (Subjective) O (Objective) A (Assessment) P(Planning). Results: The condition of the 4 clients was obtained with 2 clients, namely Mrs.V 29 years old with a history of 1 eracs and 1 abortion and Mrs. child and wants to get it back. Apart from that, 2 other clients, namely Mrs.C, 28 years old and Mrs.A, 29 years old, have not been successful in getting pregnant till date. Increasing client knowledge in the form of material discussed with interactive discussions using posters, leaflets and educational videos. Conclusion: Regulation of Health Ministry of Indonesia (PMK 21 of 2021) has regulated the implementation of CoC which aims to reduce morbidity and mortality rates for mothers and newborns. Abstract

Background: World Health Organization (WHO) in 2020 stated that 149.2 million children under 5 years old are affected by stunting. National Statistics Center of Indonesia (BPS) stated that 5.2% of child under 2 years were in underweight condition and 1.2% of them were very underweight. Based on Indonesian Health Ministry Regulation (No. 23 of 2014), efforts to implement balanced nutrition to overcome chronic nutritional problems can be done through exclusive breastfeeding. Besides that, in 2020, West Sumatra province experienced a decrease in the percentage of breastfeeding from (77.6%) to (69.7%), which was caused dominantly by factor decreased of breast milk production and secretion by 32%. WHO states that there are 10 steps for successful exclusive breastfeeding, one of which is breast care, that can be achieved by giving of rolling massage and endorphin massage, which not many people know about.Purpose: The aim of this literature study is to present information about the application of rolling massage and endorphin massage as a way to accelerate and increase breast milk production.Methods: This research  method  was  a  literature  review  study.  Journal  searches  were carried out by applying online database such as Pubmed, Garuda Portal, ScienceDirect and Google scholar. Articles were selected based on inclusion and exclusion criteria.Results: The analysis was carried out on 22 research articles. It was found that Rolling massage and endorphin massage are non-pharmacological techniques for breast care performed on the mother's back, neck and arms to trigger the excretion of the hormones prolactin and oxytocin.Conclusion: The influence of rolling massage and endorphin massage is useful in overcoming breast milk production constraintsKeywords: breastfeeding, breast milk, rolling massage, endorphin massage, breast care

Abstract

During the Covid-19 pandemic, there was a decrease in the availability of Maternal and Child Health (MCH) and contraceptive services, reduced access to services, and increased risk of infection for health workers. coverage of antenatal care, contraception and immunization has decreased significantly. Nagari Kambang Utara also experienced a decrease in the coverage of MCH services during the pandemic. Therefore, to support the recovery of maternal and child health services during the pandemic, health workers need to work together with cadres to reach all communities in order to get the health services they need. Empowering cadres is a step that is needed to help optimize maternal and child health services during the COVID-19 pandemic. The method of activities carried out is by using participatory methods from the target group, namely the health partners and the community of Nagari Kambang Utara. The activity began with conducting a workshop related to the formation of health cadres for the Nagari Kambang Utara. After the workshop was carried out, it was continued with data collection for cadres and training of health cadres in accordance with the work focus, namely cadres of pregnant women, maternity mothers, postpartum mothers, breastfeeding mothers, infant and toddler cadres and family planning cadres. The proposing team and partners agreed to work together to optimize the performance of health cadres by conducting cadre training and evaluation and agreed to carry out cooperation in the next 5 years. The results of this activity are expected to increase the achievement of MCH and family planning services and decrease MMR and IMR.
