Abstract

Despite progressive development of antibiotics and surgical approaches, outcomes in patients with intracranial abscess remained far from ideal. This paper aimed to describe challenges in the diagnosis and treatment of intracranial abscess in developing countries based on study from a tertiary hospital in Indonesia. A retrospective study was performed and included patients who were suspected of intracranial abscess at a tertiary referral hospital in Indonesia between January 1, 2012 and January 1, 2020. Clinical, radiological and laboratory characteristics were analyzed. Two-hundred-and-seventy-seven patients were initially enrolled. Age (p = 0.001), GCS at admission (p = 0.009), comorbidity with hypoalbuminemia (p = 0.003) and hydrocephalus (p = 0.009) are significantly associated with patients’ GCS when discharged.  Results from culture and resistance tests could only be retrieved from 66 patients, with 33 (50%) obtained negative results, while the other 33 (50%) results were positive, consisted of 20 (60.6%) were of gram-negative bacteria and 13 (39.4%) were of gram-positive bacteria. Treatments that are directed to hypoalbuminemia and hydrocephalus should also be prioritized when any of these comorbidities exist.

Abstract

Background: Indonesia still has a high prevalence of hypertension and diabetes mellitus. On the other hand, essential health services were disrupted during the COVID-19 pandemic. Hence, the quality of service still needs to be prioritized. The government also obliges the government-owned primary healthcare facilities (Pusat Kesehatan Masyarakat, Puskesmas) to conduct customer satisfaction surveys to evaluate service quality. This study was conducted to analyze patients’ satisfaction with the chronic disease management program (Program Pengelolaan Penyakit Kronis, Prolanis) provided by Puskesmas in Bogor City. Methods: Quantitative research with a survey approach using the CSI-29 questionnaire was conducted from October to November 2021 on 104 Prolanis participants from 6 Puskesmas in Bogor City, West Java, Indonesia. The variables were 9 dimensions of satisfaction based on the CSI-29 questionnaire. Data was transformed using the Rasch model and analyzed using the Importance-Performance Matrix (IPM).Results: Of the 104 participants, there were 78 (75%) were satisfied with the Prolanis services provided by the Puskesmas. The IPM analysis showed that the dimensions of requirements, procedures, costs, type of service products, and attitudes of health workers had low expectations and perceived performance level. Conclusions: Most of the Prolanis participants were satisfied with the Prolanis services provided by the Puskesmas in Bogor City.

Abstract

Adding to their relatively high number, people with disabilities in Indonesia also face various challenges. Given such high prevalence, cadres have an important role in promoting disability inclusion. It is crucial to measure the level of knowledge, attitudes, and behaviors of these cadres regarding disability inclusion, for example, by using an instrument. To test the validity and reliability of such an instrument, statistical analysis methods like the Rasch Model Analysis are applicable due to their advantages. This study evaluates the construct validity of the Hilfi & Kaylia Disability Inclusiveness Perception instrument among health cadres. The study used a cross-sectional design using secondary data from the Disability Inclusion Capacity instrument administered to 709 cadres residing in Bandung City, Indonesia, from July to December 2024. Construct validity was analyzed using the Rasch model. Data from 626 participants who met the criteria showed that the average outfit mean-square (MnSq) for the 25-item version of the instrument met Rasch model expectations (0.92). However, item J1 was identified as a misfitting item (outfit MnSq > 1.4 and outfit Z-standard (ZSTD) > 2) across all three versions of the instrument. The explained variance was found to be 44.1%. Reliability and separation index results were excellent, with Cronbach’s alpha at 0.96 and separation index above 2. However, category response functioning and targeting did not fully meet Rasch model expectations. Nonetheless, it can be concluded that the Hilfi & Kaylia Disability Inclusiveness Perception Instrument has acceptable construct validity with the sample of health cadres in Bandung City, Indonesia.

Abstract

Health accessibility refers to the availability of health care services accessible to the community as required. However, the convenience of accessing such services vary throughout regions due to geography. Hence differences in geographic accessibility can be an obstacle to accessing health care. This study characterized the influence of geographic accessibility on primary health care (PHC) in Karawang District. A cross-sectional study was conducted in November 2019 in five sub-districts of Karawang District. Respondents were interviewed using questionnaires to collect geographic (mileage and travel time from respondents’ house to nearest PHC) and transportation (mode of transportation and transportation cost) data. In total, the study involved 513 randomly selected households, of which 11% had far to travel to the PHC, 22% had a long traveling time, and 23% had expensive transportation costs, with motorcycles being the most common means of transport. Therefore, PHCs in several sub-districts in Karawang District are less accessible due to geographic barriers.

Abstract

Aging contributes to functional decline and heightened psychological distress that affects the older adults’ quality of life. The World Health Organization (WHO) recommends the 10-item Kessler Psychological Distress Scale (K-10) for assessing psychological distress, however the Classical Test Theory (CTT)-based evaluation method has limitations. Item Response Theory (IRT), specifically the Rasch model, offers a more accurate method for measuring the instrument’ validity and reliability. This study examined the construct validity of the K-10 Indonesian version in older adults with disabilities. A cross-sectional method and secondary data from 321 elderly individuals (≥60 years) with disabilities in Bandung City, Indonesia, were used between August and February 2025. The Instrument’s construct validity of was analyzed by performing the Rasch model. A total of 315 responses were eligible for analysis. The K-10 showed an overall fit to the Rasch model (mean outfit MnSq = 1.03±0.27). Item C10 (“feel worthless”) was identified as misfitting (outfit MnSq >1.4; ZSTD >2), although no factor bias was detected. The explained variance reached 54.10%. Reliability indicators were strong, with a Cronbach’s alpha of 0.89 and a person separation index above 2. However, category functioning and targeting did not fully meet Rasch expectations. The correlation between K-10 and WHODAS 2.0 showed a moderate positive relationship, though it did not reach the criterion for convergent validity (r >0.6).  The Indonesian version of the K-10 has acceptable construct validity in a sample of older adults with disabilities in Bandung City, Indonesia.
