Abstract

BPJS Health experienced various challenges in achieving Universal Health Coverage / Jaminan Kesehatan Nasional (JKN), including a referral system. According to the 2018 National Health Account, curative spending costs reached 69.1%. The Case Mix Index (CMI) is a benchmark for hospital performance abroad, but Indonesian hospitals have not used it. In 2019, BPJS Health tested CMI in 30 hospitals. The research uses sample data from 2019-2020 based on province, hospital type and ownership. The methods used are anova test, independent T, linear regression and multiple linear regression. The research results show a decrease in cases in 2019-2020. The highest CMI was in region 1, hospital A, and the private sector. The beta coefficient value for the proportion of male gender, elderly age, PBI APBD participants, and class 1 is greater than for CMI. CMI inpatients in 2019 is related to region, type of hospital, hospital ownership, proportion of children, proportion of productive, proportion of elderly, proportion of BP, proportion of PBI APBN, proportion of PBI APBD, proportion of PBPU, and proportion of class 1. CMI of inpatients in 2020 related to region, type of hospital, hospital ownership, proportion of men, proportion of women, proportion of children, proportion of productive, proportion of elderly, proportion of BP, proportion of PBI APBN, proportion of class 1, and proportion of class 3. It is hoped that the government will accelerate the distribution of services health and BPJS Health monitor hospital CMI.

Abstract

Health is a basic need of every individual which is an important part in improving the quality of human life. The quality of health facility services is one of the main indicators in assessing the success of a country's health care system. Good services not only contribute to improving the health status of the community, but also reflect the effectiveness of the management system and health policy as a whole. Good service quality will have a direct impact on increasing public trust and the effectiveness of patient handling. This study used a quantitative approach with descriptive analytic research. The number of samples determined based on the Slovin formula with a margin of error of 5% amounted to 100 respondents. Based on the results obtained, the variables of officer attitude, availability of medical personnel, and infrastructure have a significant influence on improving service quality (p < 0.05). The variable waiting time has an influence, but is not statistically significant (p> 0.05). Thus, improving service quality at the Karang Anyer Health Center can focus on developing the attitude and competence of officers, equitable distribution of medical personnel, and providing more patient-friendly service facilities.

Abstract

Background: Changes in lifestyle and consumption patterns of Indonesian adolescents in recent years have shown an increasing trend towards a diet high in calories, fat, sugar, and salt and low in fiber. This condition has led to an increase in the rate of adolescent obesity, which is a major risk factor for degenerative diseases in adulthood. Based on the 2023 Riskesdas, the prevalence of adolescent obesity in Indonesia reached 18.7%, up from 13.5% in 2018. One of the effective promotive-preventive efforts is through balanced nutrition education in the school environment. Objective: This activity aims to improve the knowledge, attitudes, and behavior of adolescents about the principles of balanced nutrition and encourage the implementation of healthy eating patterns to prevent obesity and degenerative diseases. Methods: The activity was carried out through the stages of preparing educational materials, implementing interactive education, demonstrating the contents of my plate, and evaluating using pre-test and post-test. Results: The balanced nutrition education activity held at SMP Swasta 102114244 Nusantara Lubuk Pakam successfully attracted the attention and interest of all students. The participants' enthusiasm was evident from the start of the activity, from the presentation of the material, the question and answer session, to the simulation of preparing a balanced nutrition menu based on the “Isi Piringku” guidelines. This is shown by the results of an increase in knowledge of 41.12% with a p value of <0.001. Conclusion: Balanced nutrition education is proven to be effective in increasing knowledge and forming healthy eating habits in adolescents. This program should be implemented sustainably in schools to prevent obesity and degenerative diseases

Abstract

Dysmenorrheaiis a frequent menstrual discomfort among adolescent girls that may greatly disrupt both school performance and everyday activities. Eating patterns, especially junk food consumption and calcium intake, may play a role in the occurrence of dysmenorrhea. This research sought to examine the association between these dietary factors and the incidence of dysmenorrhea among first-year nutrition students at Poltekkes Kemenkes Riau. A quantitative, cross-sectional_design was_employed involving_54 respondents selected through total sampling. Data were collected using a Food Frequency Questionnaire (FFQ), Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ), and a menstrual pain scale questionnaire. The results revealed that the majority of respondents frequently consumed junk food (57.4%) and had inadequate calcium intake (53.5%). Chi-square analysis showed a significant association between_junk_food consumption (p = 0.037) and calcium intake (p = 0.037) with the incidence of dysmenorrhea. The findings suggest that high junk food consumption and low calcium intake may increase the risk of dysmenorrhea. Therefore, nutrition education promoting healthy eating habits is essential to prevent menstrual disorders among young women.

Abstract

Chronic kidney failure is a progressive and irreversible decline in kidney function, and it is one of the catastrophic diseases with high financial burden in Indonesia. The increase in chronic kidney failure cases is influenced by the prevalence of hypertension, diabetes mellitus, obesity, as well as changes in community lifestyle. This study aims to identify the characteristics of chronic kidney failure patients undergoing hemodialysis in North Sumatra Province from 2015 to 2020 based on gender, age, type of National Health Insurance membership, and average service claim costs. The study uses a descriptive quantitative approach with a non-experimental cross-sectional design. The data used are secondary data of a 1% sample from BPJS Kesehatan for 2015–2020, which has obtained data usage permission from the BPJS Kesehatan Central Office. The variables analyzed include gender, age, membership segmentation, diagnosis, and claim verification costs.Keywords: chronic kidney disease, hemodialysis, BPJS Kesehatan, claim cost, patient characteristics. The analysis was carried out descriptively by taking into account individual weights to reflect the population. The study results showed that the proportion of chronic kidney failure patients undergoing hemodialysis was mostly male, totaling 66.17 percent. Based on age, the adult group (19–59 years) dominated with 67.79 percent, followed by the elderly proportion at 28.07 percent. The largest proportion by type of participation came from the PBPU group at 62.05 percent, followed by PPU at 17.29 percent. The average cost of chronic kidney failure claims with hemodialysis was Rp816.713,9 with a standard deviation of Rp209.344,8, a minimum claim of Rp95.900, and a maximum of Rp4.219.800. Chronic kidney failure patients in North Sumatra were dominated by the productive age group, males, and PBPU participants, with a fairly high financial burden on the health insurance system. These findings indicate the need to strengthen promotive and preventive efforts against risk factors for chronic diseases to curb the increase in cases and the burden of health care costs
