Abstract

Rasa haus merupakan gejala umum pada pasien Congestive Heart Failure (CHF) yang mendapatkan terapi diuretic dan pembatasan cairan. Ketidakpatuhan pasien dalam pembatasan cairan akibat rasa haus dapat menyebabkan hipervolemia, salah satu penyebab rawat inap. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian stroberi beku terhadap skala haus pada pasien CHF. Metode yang digunakan adalah quasi eksperimen dengan desain pre-test and post-test control group design. Jumlah total responden sebanyak 36 orang yang terbagi dalam dua kelompok yaitu kelompok intervensi 18 orang dan kelompok kontrol 18 orang, diambil menggunakan tehnik stratified random sampling berdasarkan klasifikasi NYHA pada responden. Responden pada kelompok intervensi diberikan stroberi beku (@20 gram) sedangkan untuk kelompok kontrol diberikan es batu (@5 ml) sebanyak lima kali sehari selama tiga hari. Data dianalisis menggunakan analisis deskriptif, uji-t independent dan uji-t berpasangan. Hasil penelitian menunjukkan bahwa rata-rata skor intensitas haus pada kelompok intervensi dan kontrol masing-masing adalah 4,44 dan 2,35. Uji t independen perbedaan rata-rata intensitas haus sebelum dan sesudah tes antara kelompok intervensi dan kontrol adalah p=0,000. Berdasarkan analisis uji t berpasangan, skor intensitas haus antara pre-test dan post-test adalah p<0,001, dari intensitas haus sedang hingga rendah. Skor intensitas haus antara sebelum dan sesudah prosedur pada kelompok intervensi dan kontrol berbeda secara signifikan. Analisis data diperoleh p-value sebesar 0,000. Rata-rata delta intensitas haus pada kelompok intervensi lebih tinggi dibandingkan kelompok kontrol. Terdapat perbedaan rasa haus antara kelompok intervensi dan control (p-value =0.006). Hasil penelitian menunjukkan bahwa konsumsi stroberi beku secara signifikan mengurangi rasa haus pada pasien CHF.

Abstract

Based on Riskesdas data in 2018, the prevalence of heart disease in Indonesia is around 1,017,290 population. Congestive heart failure is a condition in which the heart is unable to pump blood throughout the body properly. Signs of symptoms that usually appear are dyspnea and edema, where edema is a buildup of fluid or hypervolemia in several organs of the body. If not treated immediately can cause complications in the form of cardiogenic shock, thrombolytic episodes and pericardial effusion or cardiac tamponade. One of the non-pharmacological management that can be done to overcome hypervolemia is a contrast bath. Contrast bath is a treatment by soaking the feet in warm water, followed by cold water alternately. The temperature of the warm water used is 36,6 – 43,3 °C and the temperature of cold water is 10 - 20 °C. This case study aims to illustrate nursing care with the application of contrast bath in congestive heart failure patients who have hypervolemia. This type of case study method used a descriptive method with 1 patient with inclusion criteria for congestive heart failure patients who have lower extremity edema. The assessment showed that the patient felt tight and there was edema of the lower extremities, so that the nursing diagnosis that was established was hypervolemia with the implementation of the application of contrast bath carried out 3 times a day for 2 consecutive days with a time of 15 minutes once an action. The results of the case study of the application of contrast bath showed that there was a decrease in the degree of edema in both lower extremities which was originally at a degree of +3, to a degree of +2 after the intervention. It can be concluded that this contrast bath can overcome hypervolemia in congestive heart failure patients, the author also recommended to patients and families to perform this intervention independently.

Abstract

The prevalence of stroke at Dustira Hospital is 124 people. Stroke is a sudden neurological function deficit caused by a disturbance in cerebral blood circulation. One of its signs and symptoms is swallowing disorder. If swallowing exercises are not performed, complications such as aspiration pneumonia may occur. Swallowing exercises are a type of therapy designed to improve swallowing ability by focusing on strengthening muscles and enhancing the coordination of nerves and muscles involved in the swallowing process. This case study uses a descriptive method to illustrate nursing care for stroke patients experiencing swallowing disorders. The instruments used include the standard operating procedure for swallowing exercises and an observation sheet before and after the exercises. The assessment results showed that the patient had difficulty moving the tongue to the right, left, and upward, impaired swallowing reflexes, slurred speech, and frequent choking while eating or drinking, leading to the nursing diagnosis of swallowing disorder. Swallowing exercises were conducted three times a day for seven days, with each session lasting 30 minutes. The evaluation of this case showed that the swallowing disorder initially had a swallowing reflex score of 3 (moderate), which improved to 4 (fair improvement) after the intervention. It is concluded that swallowing exercises can improve the swallowing reflex in stroke patients with swallowing disorders. It is hoped that these exercises can be applied by hospitals, patients, and families independently.
