Abstract

Introduction: In the modern healthcare system, blood transfusion is an essential treatment that plays a significant role in the clinical improvement of adult and pediatric patients. Given the physiological and hematological differences between children and adults, careful planning and an individualized approach are essential to maximize its effectiveness and avoid serious side effects. Method:  A thorough review of the literature was performed, utilizing PubMed, Embase, and the Cochrane Library, to gather evidence on pediatric perioperative transfusion practices. The search encompassed studies published between 2013 and 2024, employing a combination of terms and free-text keywords, as well as Boolean operators, to refine the results. Conclusion: Intraoperative blood loss in pediatric patients requires special attention with an approach that includes pre to postoperative management. This approach must be adapted to the anatomical and physiological differences of children compared to adults. Research, development of universal transfusion strategies, and education and training for medical personnel are essential to improve blood management and prevention of complications.

Abstract

Background: Diabetic retinopathy (DR) is a common microvascular complication of diabetes mellitus that remains a leading cause of preventable blindness worldwide. Chronic hyperglycemia leads to retinal microangiopathy, resulting in progressive damage that may culminate in serious complications. As DR advances, especially into the proliferative stage (PDR), patients may experience vitreous hemorrhage (VH), diabetic macular edema (DME), or tractional retinal detachment (TRD). Methods: This study is a literature review of ten scientific articles published between 2015 and 2024, focusing on the complications and management of DR. Results: The literature highlights that the primary treatment modalities for DR include panretinal photocoagulation (PRP), intravitreal anti-VEGF injections, and vitrectomy. PRP is commonly used for high-risk PDR, anti-VEGF for DME or active neovascularization, and vitrectomy for advanced cases with VH or TRD. Management decisions are highly dependent on the stage of DR and the presence of complications. Conclusion: The management of DR should be adapted to the severity and type of complication. Early diagnosis and timely intervention are crucial to prevent vision loss.

Abstract

Pterygium is an abnormal growth of the conjunctiva onto the cornea that often causes astigmatism and impairs vision. Exposure to ultraviolet (UV) rays is a major risk factor. This study aims to analyze changes in astigmatism in pterygium patients who underwent surgery at the JEC Orbita Eye Clinic in Makassar between 2021 and 2023. An analytical observational study with a cohort approach was used to measure changes in astigmatism before and after surgery. The study sample consisted of 26 patients with pterygium who had complete medical records. Data were collected from medical records and analyzed using the Wilcoxon test. The results showed that the majority of patients (53.8%) had mild astigmatism before surgery. After surgery, most patients (53.8%) still had mild astigmatism, with some patients experiencing a significant decrease. The Wilcoxon test showed a significant difference between astigmatism before and after surgery, indicating a significant improvement in most patients. The conclusion of this study shows that pterygium surgery has a significant effect on reducing astigmatism in patients. Although the majority of patients experienced visual improvement after surgery, some patients still showed high astigmatism. The size of the pterygium and the surgical technique play an important role in postoperative results
