Abstract

Diabetes melitus adalah penyakit metabolik kronis yang ditandai dengan peningkatan kadar glukosa darah yang dapat menyebabkan kerusakan pada jantung, pembuluh darah, mata, ginjal, dan saraf. Tujuan penelitian ini untuk menerapkan asuhan keperawatan dan penerapan evidence based nursing Buerger-Allen exercise pada pasien diabetes melitus. Asuhan keperawatan ini dilakukan pada perempuan, umur 59 tahun dengan diagnosis diabetes melitus. Dari pengkajian diperoleh empat masalah keperawatan yaitu perfusi perifer tidak efektif, nyeri akut, risiko kekurangan volume cairan, risiko defisit nutrisi. Implementasi yang dilakukan untuk masalah keperawatan perfusi perifer tidak efektif salah satunya adalah melakukan Buerger-Allen exercise, hasil evaluasi menunjukkan terjadi peningkatan ankle brachial index (ABI) di kaki kanan dari 0,81 menjadi 0,992, kaki kiri 0,83 menjadi 1,007 dan perbaikan pada sistem vaskularisasi perifer dari kerusakan ringan menjadi normal. Perawat dapat mengajarkan teknik Buerger- Allen exercise pada pasien dan keluarga untuk meningkatkan ABI dan perbaikan pada sistem vaskularisasi perifer

Abstract

ABSTRACT Background: Type 2 Diabetes Mellitus (T2DM) accounts for around 90% of all diabetes cases and is at risk of experiencing acute complications such as hypoglycemia, DKA, and HHNS. The three main factors that influence self-management of T2DM patients are knowledge, disease duration, and self-efficacy. High self-efficacy plays an important role in patient compliance with self-management. Improving self-efficacy can be done through health education, one of which is through the Diabetes Self Management Education (DSME) program. The purpose of this study was to determine the effect of education on self-efficacy in T2DM patients. Method: using the literature review method, the search method using Google Scholar, after searching and looking at the year of publication, title suitability and quality assessment, six selected articles were obtained, Research Results: shows that educational interventions are effective in increasing self-efficacy. Conclusion: education provided through various educational methods such as videos, e-modules, group discussions, and digital media has been shown to increase patient confidence in managing the disease independently. Abstract

Introduction: Self-efficacy is a key factor influencing diabetes self-management, with higher self-efficacy linked to better adherence to self-care and reduced complications. The Diabetic Management Self-Efficacy Scale (DMSES), a 15-item Likert questionnaire, assesses self-efficacy in diet, exercise, blood glucose monitoring, therapy, and foot care. This study aimed to evaluate the validity and reliability of the Indonesian version of DMSES for use among patients with diabetes mellitus. Methods: This cross-sectional study recruited 30 adults with diabetes mellitus, with 26 participants completing the questionnaire. The study was conducted in three stages: (1) translation, (2) content validation, and (3) validity and reliability testing. Forward–backward translation was applied to ensure linguistic and cultural accuracy. Content validity was assessed by a panel of five experts who rated item relevance, and the content validity index (CVI) was calculated. Item validity was examined by correlating individual item scores with total scores, while internal consistency reliability was evaluated using Cronbach’s alpha. Results: During translation, four items were modified to improve cultural and language clarity. The content validity assessment yielded a CVI of 1.00, indicating excellent agreement among experts. Item analysis demonstrated moderate to strong correlations (r = 0.550–0.867), with all correlation values exceeding the critical r table value (0.388), confirming item validity. The internal consistency reliability test produced a Cronbach’s alpha of 0.934, signifying excellent reliability across all items. Conclusion: The Indonesian version of the DMSES demonstrates strong content validity and high reliability, making it a valid and reliable instrument for measuring self-efficacy in patients with diabetes mellitus. These findings support the use of DMSES in future research and clinical practice in Indonesia to assess and strengthen diabetes self-management behaviors.

Abstract

Background: Diabetes mellitus (DM) is a chronic metabolic disease that is at risk of causing various complications including microvascular and macrovascular complications. It therefore needs to be controlled through the implementation of diabetes self-management on a regular daily basis. The factor that plays a role in influencing diabetes self-management is self-efficacy, therefore it is necessary to make efforts to increase self-efficacy through an educational program. Purpose: The purpose of this study was to design and validate educational materials to improve self-efficacy in patients with type 2 diabetes mellitus. Methods: The method used consists of two phases, namely 1) design; and 2) validation of educational materials. The design of the educational materials followed the recommendations provided by Hoffman while the content followed the guidelines of the Association of Diabetes Educators. Validation was conducted including validation by experts and validation by representatives of the DM patient population using validity instruments. Results: The educational materials compiled consisted of six units, namely self-efficacy; diet (meal planning); exercise; therapy, independent blood sugar monitoring; and foot care. The results of validation by experts obtained an approval rate of 87.65%, while the results of validation from population representatives reached 99.04% approval. Conclusions: The educational materials compiled or designed have met the provisions of valid or feasible to be used as educational materials for type 2 DM patients. Abstract

Diabetes mellitus, a metabolic disorder, is a chronic disease distinguished by hyperglycemia (above-normal blood sugar levels). Case studies are conducted to gain practical experience in administering foot exercises and providing nursing care to clients with diabetes mellitus. The descriptive method was used, along with a case study approach and the use of evidence-based practices. Nursing care was provided to a 55-year-old female patient with diabetes and ketosis. The results of the study obtained four nursing problems, namely ineffective airway clearance, unstable blood glucose levels, risk of nutritional deficits, risk of ineffective peripheral perfusion. The implementation was carried out, namely doing diabetic foot exercises for 15-20 minutes with a frequency of 1x/day for three days to reduce tingling in patients. The evaluation results showed that the tingling sensation was significantly reduced on the third day. In conclusion, diabetic foot exercise can be applied to diabetes mellitus patients who experience impaired peripheral blood flow. Hopefully the results of this study can provide information about the application of diabetic foot exercises to patients and their families to reduce tingling in the feet.
