Abstract

Sinus maksilaris merupakan struktur anatomi vital yang berada di atas gigi posterior rahang atas dengan variasi perluasan anatomis yang beragam. Kedekatan akar gigi terhadap sinus maksilaris dapat berkaitan dengan komplikasi yang tidak diinginkan yang menyertai tindakan klinis. Radiograf panoramik dapat digunakan sebagai pemeriksaan penunjang untuk menganalisis kedekatan struktur anatomi tersebut. Penelitian ini bertujuan untuk mengetahui hubungan jenis gigi molar kedua kanan dan kiri serta jenis kelamin terhadap keterdekatan akar mesiobukal dengan sinus maksilaris. Sampel penelitian berjumlah 164 sampel radiograf panoramik digital (81 pria dan 81 wanita) berusia 20-40 tahun yang sesuai kriteria inklusi dan ekslusi. Sampel diperoleh secara retrospektif dari Instalasi Radiologi Dentomaksilofasial RSGM UGM Prof. Soedomo. Pengamatan radiografis hubungan akar mesiobukal gigi molar kedua terhadap dasar sinus maksilaris dikategorikan menjadi tiga tipe, yaitu tipe 0 (akar tidak berkontak dengan dasar sinus), tipe 1 (akar berkontak dengan tepi kortikal sinus), dan tipe 3 (akar menembus kedalam rongga sinus). Hasil uji korelasi dengan Coefficient Contingency menunjukkan tidak terdapat hubungan antara molar kedua sisi kanan dan kiri terhadap tipe keterdekatan akar gigi terhadap sinus (p > 0,05) serta tidak terdapat hubungan antara jenis kelamin terhadap tipe keterdekatan akar gigi terhadap sinus (p > 0,05). Melalui analisis Coefficient Contingency C diperoleh korelasi variabel jenis kelamin (C = 0,117) yang menunjukkan keeratan hubungan yang lebih tinggi terhadap kedekatan akar gigi terhadap sinus maksilaris dibandingkan dengan jenis gigi (C = 0,036), namun demikian korelasi antar variabel tersebut tidak signifikan.

Abstract

Background: Maxillofacial fractures are rare in children. The treatment of fractures in children differs from that of adults due to the growth process of the jaw and dental development. The intraoral approach does not result in facial scars or nerve injury. Furthermore, it allows direct visualization of occlusion confirmation during the plate placement. Purpose: This case report describes a pediatric comminuted mandibular fracture that underwent open reduction with an intraoral approach. Case: A 13-year-old boy with a mandibular fracture was referred by the neurosurgery department. Clinical examination found a hematoma in the left submental region and discontinuity of the mandibular symphysis without a wound in the extraoral region. Coleman's sign and a vertical step between teeth 41 and 31 and teeth 33 and 34 with anterior open bite were found intraorally. A CT scan disclosed more than two fracture lines that involved the mandibular symphysis and a basal triangle fracture. The patient was diagnosed with a symphysis mandibular comminuted fracture with open bite malocclusion. Case management: Open reduction and reconstruction plate placement with an intraoral approach was performed under general anesthesia. The patient had no complaints during the two-month evaluation, and the plate was planned for removal in the third month postoperatively. Conclusion: In this case, open reduction with an intraoral approach was able to restore bone architecture without functional or aesthetic complications. This may be because the patient could be treated like an adult, as he had almost reached skeletal maturity due to his age. However, periodic observation is necessary to evaluate jaw growth.

Abstract

Cherubism is an autosomal dominant disease that causes abnormal osteoblast-osteoclast function, resulting in progressive enlargement around the maxillofacial area due to replacement of normal bone with fibrous tissue and immature bone. The clinical and radiographic signs of cherubism include bilateral radiolucent multi-ocular lesions of the mandible and maxilla, causing bilateral cheek swelling. A 9-year-old boy came to the Oral Surgery of Sardjito Hospital, Yogyakarta, with a complaint of enlargement of both cheeks nine months ago. The patient complained that the swelling on the maxilla enlarged and felt disturbing for his appearance. The enlargement was painless, not easy to bleed, hard, and disturbing when eating and sleeping. Radiograph examination demonstrated bilateral maxillary and mandibular multi-ocular-radiolucent lesions. Three stages of surgery were performed: biopsy-incision with the histopathological result being benign fibrous histiocytoma, curettage-reshaping of the mandible 1 month after biopsy, and curettage-reshaping of the maxilla 9 months after the mandible surgery. The histopathological test of the lesions in the maxilla and mandible showed the same result: peripheral reparative giant cell granuloma. Twelve months postoperatively, the patient had no complaints, showed no disturbances in eating and sleeping, and regained a symmetrical face. The patient's initial curettage and reshaping resolved the masticatory complaints, improved facial aesthetics, and reduced lesions' expansion at the bilateral mandibular and maxillary bones.
