Abstract

The quality of life of patients becomes an essential part of the success of an intervention since it is used as an outcome measure. Mandible resection will cause discontinuities that can degrade the quality of life of patients. Extensive resectionand reconstruction will affect the quality of life of patients after resection and reconstruction surgery as compared with that before the surgery. On this basis, cross-sectional study was conducted among 27 patients suffering from benignmandibular tumors undergoing surgical procedure of marginal resection, segmental resection, or hemimandibulectomy at Dr. Sardjito genral hospital in 2010-2015. Quality of life of patients after mandibular resection and reconstruction was measured with modified oral health impact profile (OHIP-14). An assessment of the quality of life before and after surgery was based on gender and type of reconstruction and it was analyzed using t-test and one-way ANOVA. It was revealed that the quality of life of patients with benign tumors increased significantly (p=0,000), and that gender differences did not significantly affect the improved quality of life (p=0.433). The mean score of patient’s quality of life was highest on the type of marginal resection (mean value =14.50), and the lowest was on segmental resection (mean value=7.50), but the type of resection did not significantly influence the improved quality of life (p=0.152). Resection and reconstruction procedures under taken by Oral and Maxillofacial Surgeon at Dr. Sardjito general hospital can improve the quality of life of patients with benign tumors of the lower jaw.

Abstract

Kasus bibir dan lelangit sumbing merupakan salah satu kelainan deformitas yang sering terjadi. Keadaan klinis bervariasi mulai dari kasus sumbing tidak komplit sampai dengan komplit. Keadaan klinis dengan lebar celah yang bervariasi membutuhkan teknik pembedahan yang tepat. Pasien-pasien dengan bibir dan atau lelangit sumbing mungkinsebelumnya telah menjalani beberapa intervensi pembedahan, sehingga sering memerlukan koreksi lebih lanjut untuk memperbaiki hasil operasi sebelumnya. Pasien anak laki-laki usia 4 tahun 7 bulan mengeluhkan masih terdapatnya celah di lelangit. Pasien didiagnosis dengan labiognatopalatoschisis bilateral. Sebelumnya pasein sudah menjalani 4 kali operasi penutupan celah bibir dan lelangit. Pasien menjalani 2 kali operasi bibir sumbing dengan metode lip adhesion dan metode Barsky, dan 2 kali operasi lelangit sumbing dengan metode pushback dilanjutkan dengan koreksi dengan z plasty, tetapi hasil akhir masih terdapat celah di palatum mole. Selanjutnya dilakukan operasi repair palatoplasi denganmetode Furlow double opposing z plasty dengan kombinasi insisi lateral, dan didapatkan hasil menutupnya celah di palatum mole sampai dengan uvula. Telah dilakukan operasi repair palatoplasi dengan metode Furlow double opposing z plasty. Teknik ini dilakukan untuk menghindari insisi yang terlalu luas dikarenakan terdapatnya jaringan fibrous yang tebal pada mukosa palatum pasca operasi sebelumnya. Tujuan studi kasus adalah untuk mengetahui kemampuan teknik Furlow Double Opposing Z Plasty sebagai prosedur repair palatoplasty. Repair Palatoplasty Management with Furlow Double Opposing Z Plasty Technique. Cases of cleft lip and palate are one of the deformity disorders that often occur. There are variety of clinical appearance ranging from incomplete to complete cases. Clinical appearance with different width requires proper surgical technique. Patients with cleft lip and palate had undergone surgical intervention, so that they needed surgical correction to repair the result or failure of the previous surgery. A Four year old boy complain there was cleft on the soft palate. The patient was diagnosed with labiognatopalatoscisis. The patient had undergone two stages of cleft lip surgery and twice of cleft palate surgery with pushback method and repair with z plasty, however the result was unsatisfactory. Further, the patient underwent repair palatoplasty surgery with Furlow double opposing z plasty method combined with lateral relaxing insicion. The result in the post surgery was the closure of cleft soft palate up to uvula. Repair palataplasty surgery has been done with Furlow double opposing z plasty method. This technique could avoid extended incision due to thick fibrous tissue on the palatum mucosa as the result of serial previous surgery. The aim of this case case study is to determine the technical capabilities of Furlow Double Opposing Z Plasty as palatoplasty repair procedure.

Abstract

Kista residual merupakan lesi odontogenik yang kerap ditemukan secara insidental pada pemeriksaan radiografi setelah pencabutan gigi. Jenis kista ini umumnya bersifat asimtomatik dan berpotensi menimbulkan komplikasi jika tidak ditangani dengan tepat. Pada tulisan ini dilaporkan perempuan 21 tahun yang menjalani pemeriksaan radiografi panoramik untuk perawatan ortodonti dengan temuan insidental berupa kista pada regio 46 yang edentulous. Pasien sempat merasa kebas pada area bekas pencabutan gigi geraham kanan bawah, namun gejala tersebut hilang setelah mengonsumsi kortikosteroid. Diagnosis awal kista residual ditegakkan melalui pemeriksaan Cone Beam Computed Tomography (CBCT) yang menunjukkan lesi radiolusen dengan tepi terkortikasi berukuran 26 × 16 mm. Pasien menjalani tindakan enukleasi kista melalui prosedur bedah yang meliputi insisi flap, pengurangan tulang, dan pengangkatan kantung kista. Pascaoperasi pasien diberikan medikasi antibiotik dan analgesik. Pemeriksaan histopatologi mengonfirmasi kista residual tanpa tanda keganasan. Hasil penyembuhan menunjukkan regenerasi tulang yang baik tanpa penggunaan bone graft, dan radiografi 30 hari pascaoperasi mengindikasikan pembentukan tulang baru. Teknik enukleasi efektif dan aman, menghasilkan penyembuhan tanpa komplikasi. Deteksi dini melalui pemeriksaan rutin dan tindakan enukleasi yang hati-hati direkomendasikan untuk mencegah komplikasi dan kekambuhan.

Abstract

Silkworm cocoon (Bombyx mori) is a natural polymer composite and largely used as bio-functional material for wound healing. It consists of fibroin and sericin protein that has antibacterial effect. This study aimed to investigate the effect of silkworm cocoons (Bombyx mori) wound dressing on the number of macrophages and VEGF expression in skin excision. The subject of this study was 12 Wistar rats, which were grouped based on the duration of wound dressing application (3rd day and 6th day) and the use of dressing materials (n = 6). The rats were anesthetized with ketamin and xylazine prior to wound excision. A punch biopsy wound excision with 4 mm diameter of subcutaneous depth was made on both sides of the rat’s back, with the right side dressed with silkworm cocoon as the treatment group and the left side dressed with moist gauze application as the control group. Hematoxylin-eosin (HE) staining was performed to observe the number of macrophage cells. Immunohistochemical staining using an anti-VEGF antibody was performed to observe the expression of VEGF. Data were analyzed using a two-way ANOVA and an Independent t-test with confidence interval of 95%. Statistical analysis demonstrated a significantly higher number of macrophages in the silkworm cocoon wound dressing group on 6 days post-application (p = 0.026) and significantly higher VEGF expression in the silkworm cocoon wound dressing group on 3 days post-application (p = 0.002) and on 6 days post-application (p = 0.044). Silkworm cocoon (Bombyx mori) wound dressing can increase the number of macrophages and VEGF expression in wound excision model in Wistar rat.

Abstract

Cherubism is an autosomal dominant disease that causes abnormal osteoblast-osteoclast function, resulting in progressive enlargement around the maxillofacial area due to replacement of normal bone with fibrous tissue and immature bone. The clinical and radiographic signs of cherubism include bilateral radiolucent multi-ocular lesions of the mandible and maxilla, causing bilateral cheek swelling. A 9-year-old boy came to the Oral Surgery of Sardjito Hospital, Yogyakarta, with a complaint of enlargement of both cheeks nine months ago. The patient complained that the swelling on the maxilla enlarged and felt disturbing for his appearance. The enlargement was painless, not easy to bleed, hard, and disturbing when eating and sleeping. Radiograph examination demonstrated bilateral maxillary and mandibular multi-ocular-radiolucent lesions. Three stages of surgery were performed: biopsy-incision with the histopathological result being benign fibrous histiocytoma, curettage-reshaping of the mandible 1 month after biopsy, and curettage-reshaping of the maxilla 9 months after the mandible surgery. The histopathological test of the lesions in the maxilla and mandible showed the same result: peripheral reparative giant cell granuloma. Twelve months postoperatively, the patient had no complaints, showed no disturbances in eating and sleeping, and regained a symmetrical face. The patient's initial curettage and reshaping resolved the masticatory complaints, improved facial aesthetics, and reduced lesions' expansion at the bilateral mandibular and maxillary bones.

Abstract

Oroantral fistula (OAF) is a complication of dental extraction that is defined as open pathological communication between oral cavity and sinus of the maxillary with the involvement of epithelization of a canal. Oroantral fistula will cause infection, impaired healing, and chronic sinusitis. This article reports a case of OAF in tooth 16 which occurred after 11 months following unsuccessful sinus closure treatment. Many approaches are widely used for the closure of OAF, such as using a buccal fat pad (BFP), buccal advancement flap (BAF), or a combination. This study aims to describe the combined techniques of BFP and BAF in OAF closure of large defects with a history of previous closure failure. A 31-year-old male patient came to our hospital for the presence of OAF for 2-3 weeks. The patient underwent OAC closure at another hospital 11 months prior, but the complaint recurred. Diagnosis of OAF was carried out using the Valsalva test. A combination of BFP and BAF techniques was done to close the OAF after the removal of the epithelial. Evaluation of the treatment after 30 days showed adequate healing and full closure of OAF. OAF closure with the combined techniques of BFP and BAF displays promising results for the revision of failed OAF closure treatment.

Abstract

Tooth extraction is a common dental procedure. Osteoblasts (bone-forming cells) and collagen are key indicators of wound healing following tooth extraction. Hydroxyapatite is a calcium-rich material that promotes the secretion of Fibroblast Growth Factor (FGF), Platelet-Derived Growth Factor (PDGF), and Transforming Growth Factor Beta (TGF-β)—all of which play critical roles in the wound healing process. Chicken bones, a natural source of hydroxyapatite, contain approximately 85% calcium phosphate minerals. This study aimed to determine the effect of chicken bone-derived hydroxyapatite on osteoblast cell count and collagen density in post-tooth extraction wounds in Wistar rats. Thirty male Wistar rats were randomly divided into treatment and control groups. Tooth extraction was performed on the lower left incisor of each rat. Hydroxyapatite was prepared by calcining chicken bones at 700 °C to remove organic material. The resulting hydroxyapatite powder was implanted into the tooth sockets of rats in the treatment group, while the control group received no implantation. Both groups were sutured and treated with povidone-iodine. Three rats from each group were sacrificed on days 3, 5, 7, 10, 14, and 21. Histological samples were prepared using hematoxylin-eosin and Mallory’s Trichrome staining. Osteoblast cells (100× magnification) and collagen density (400× magnification) were examined using a light microscope and Optilab Viewer, across five fields of view per sample. Two-way ANOVA showed significant differences in both osteoblast cell counts and collagen density between groups and across observation days (p < 0.05). Least Significant Difference (LSD) post hoc analysis also revealed significant differences between groups on all observation days (p < 0.05). In conclusion, chicken bone-derived hydroxyapatite significantly increases osteoblast numbers and collagen density during the post-extraction wound healing process in Wistar rats.
