Abstract

Latar Belakang: Konsep non-maleficence merupakan prinsip etis penting dalam keperawatan, menekankan kewajiban untuk tidak membahayakan pasien. Studi ini bertujuan menganalisis penerapan prinsip non-maleficence pada pasien dengan Diabetes Melitus (DM).Metode: Metode penelitian meliputi analisis konsep dengan pendekatan kualitatif yang mencakup observasi kasus klinis dan studi literatur. Analisis dilakukan dengan menetapkan karakteristik konsep, mengidentifikasi atribut seperti kewajiban etis, pencegahan cedera, dan pelaksanaan tindakan sesuai protokol medis.Hasil: Hasil analisis menunjukkan bahwa kelalaian dalam pemeriksaan dan diagnosis awal memperburuk kondisi pasien, menyoroti pentingnya pengkajian komprehensif sesuai prosedur. Karakteristik konsep non-maleficence mencakup kewajiban etis, pencegahan kerugian, dan tindakan sesuai protokol medis. Instrumen yang relevan seperti SAQ-AV dan HSOPSC digunakan untuk mengevaluasi penerapan prinsip ini dalam budaya keselamatan pasien.Kesimpulan: Implementasi non-maleficence yang efektif memerlukan peningkatan edukasi perawat tentang standar prosedur dan kepatuhan terhadap protokol untuk mencegah risiko lebih lanjut pada pasien.

Abstract

Background: Ischemic stroke causes impaired motor function, including in the toes, which impacts patient mobility. One intervention to address this issue is Range of Motion (ROM) exercises. Purpose: To analyze the effectiveness of ROM exercises on toe joint range of motion in ischemic stroke patients. Method: This systematic review used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Articles were searched from Google Scholar, PubMed, and ScienceDirect databases, with the inclusion criteria being quantitative studies within the last 10 years. Seven articles were selected and critically analyzed using the JBI instrument. Results: The use of toe ROM exercise therapy devices has the potential to improve toe ROM and muscle strength in stroke patients. Based on the reviewed studies, passive and active ROM therapy have been shown to be effective in reducing joint stiffness, increasing flexibility, and strengthening muscles in patients with limited mobility. These devices are designed to provide consistent and programmed passive movements, thus being expected to be a more efficient substitute for manual therapy. Conclusion: Range of motion (ROM) exercises are a proven and recommended intervention in the rehabilitation of ischemic stroke patients to improve toe joint range of motion.   Keywords: Ischemic Stroke; Range of Motion (ROM); Toe Joint Range of Motion.   Pendahuluan: Stroke iskemik menyebabkan gangguan fungsi motorik termasuk pada jari kaki yang berdampak pada mobilitas pasien. Salah satu intervensi untuk mengatasi masalah ini adalah latihan Range of Motion (ROM). Tujuan: Untuk menganalisis efektivitas latihan ROM terhadap rentang gerak sendi jari kaki pada pasien stroke iskemik. Metode: Penelitian systematic review menggunakan panduan Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Pencarian artikel dari database Google Scholar, PubMed, dan ScienceDirect dengan kriteria inklusi yaitu studi kuantitatif 10 tahun terakhir. Sebanyak tujuh artikel terpilih dan dianalisis secara kritis menggunakan instrumen JBI. Hasil: Penggunaan alat Taromaki memiliki potensial untuk meningkatkan ROM dan kekuatan otot jari kaki pada pasien stroke. Berdasarkan studi yang telah diulas, terapi ROM pasif maupun aktif terbukti efektif dalam menguranggi kekakuan sendi, meningkatkat fleksibelitas, dan memperkuat otot pada pasien dengan keterbatasan mobilitas. Alat tersebut dirancang untuk memberikan gerakan pasif terprogram yang konsisten, sehingga diharapkan mampu menggantikan terapi manual secara lebih efisien. Simpulan: Latihan ROM merupakan intervensi yang terbukti efektif dan direkomendasikan dalam rehabilitasi pasien stroke iskemik untuk meningkatkan rentang gerak sendi jari kaki.   Kata Kunci:  Range of Motion (ROM); Rentang Gerak Sendi Jari Kaki; Stroke Iskemik.

Abstract

Background. Non-maleficence is an ethical principle in the field of nursing. This concept is often used in patient care in carrying out or providing nursing actions. Non-maleficence in patients with diabetes mellitus refers to actions that do not cause harm or harm to the patient Methods. Analyze this concept carried out with the help of literature on non-maleficence obtained throughGoogle Scholar and Online Books database. The keywords used are "Concept of non-maleficence" and "diabetes mellitus patients". Year of publication of books and articles used from 1979 to 2023. Results. In Indonesia, DM is the 3rd largest cause of death with a percentage of 6.7 percent, after stroke, which is 21.1 percent, and heart disease, which is 12.9 percent. In 2013, the prevalence of DM in Indonesia was 1.5%, while in 2018 it reached 2.0%. Based on age categories, the largest number of DM sufferers are in the age range 55-64 years and 65-74 years. In addition, more DM sufferers in Indonesia are women (1.8%) than men (1.2%). Then, for areas of residence, more diabetes mellitus sufferers are in urban areas (1.9%) compared to rural areas (1.0%) Conclusion. Concept analysis was carried out using the methods and steps proposed by Walker and Avant 2011. Methods of identifying the attributes, antecedents and consequences of nonmaleficence. Non Maleficence is an ethical obligation not to cause harm intentionally or through negligence without any justifiable reason, the principle of action should be in accordance with procedures to ensure safety considering benefits.
