Abstract

Introduction : Coronary Artery Bypass Graft (CABG) is a surgical procedure to restore normal blood flow to the heart by bypassing blocked coronary arteries. The purpose of this study was to determine the effectiveness of giving Deep Breathing Exercise in cases of postoperative CABG (Coronary Artery Bypass Graft). Case Presentation: A 55 year old man with a dentist job. The patient complains of pain in the chest that was incised, pain occurs when sneezing or coughing. Management and Outcome : The patient is given deep breathing exercise. Do repetitions for 1 minute with a break of 2 seconds for each repetition, done 2 times a day. Pain was measured using the NPRS (Numeric Pain Rating Scale). Discussion : After being given therapy, it was found that there was a decrease in motion pain as much as 2 points which was measured using NRS where the values at T(beginning):5 and T(end) were:3. Conclusion : Giving therapy in the form of deep breathing exercise modality can reduce chest pain (incise scar) in patients after CABG (Coronary Artery Bypass Graft) surgery.

Abstract

Background and Purpose: Breast cancer surgery may cause late effects of pain, range of motion loss, and activity limitation in the upper extremity (UE). The surgical complications was shoulder adhesive capsulitis (AC). AC is a condition of progressive loss of UE range of motion (ROM) and leading to limitations functional mobility. The purpose of this case report is to investigate the implementation of transcutaneous electrical nerve stimulation (TENS) combined with joint mobilization technique and hold relax technique for a patient with post mastectomy surgery and chemotherapy within a year with AC. Method: The patient was a 55-year-old female who was referred to physical therapy, Dr. Sardjito Central General Hospital. The patient had restricted mobility and pain in her left UE with a medical diagnosis of AC. A comprehensive physical treatments have done as well TENS, joint mobilization technique, and hold relax technique for twice a week. The outcomes were measured by goniometer, manual muscle testing (MMT), visual analog scale (VAS) and shoulder pain and disability index (SPADI). Result: The active ROM of shoulder increased significantly, decreased pain intensity (7/10 to 6/10) and increase muscle strength (3/5 to 4/5) and as well better functional performance (73.1% to 66.9%) in patient with AC affected by post mastectomy surgery. Conclusion: A good physiotherapy strategy for post mastectomy, clearly reduced impairments as muscle strength, joint mobility and functional activities of shoulder joint. As well this study could be a reference for clinicians to manage the patients with shoulder problem after breast cancer surgery.

Abstract

Introduction: Drop foot ditandai oleh ketidakmampuan untuk melakukan dorsifleksi pada bagian depan telapak kaki akibat kelemahan otot dorsifleksi. Kondisi ini dapat mengakibatkan gaya berjalan yang tidak stabil dan kompensasi, serta meningkatkan risiko terjatuh.Case Presentation: Seorang pria berusia 69 tahun, dengan diagnosa medis drop foot sinistra. Pasien datang dengan keluhan kelemahan otot kaki/drop foot pada bagian kaki kiri serta adanya rasa nyeri pada ankle kiri pada saat berjalan.Management and Outcome: Intervensi fisioterapi yang diberikan yaitu Electrical Stimulation dan Terapi Latihan. Pemberian Electrical Stimulation diberikan 1x/minggu dan Terapi Latihan dilakukan 2x/setiap hari. Hasil yang didapat adalah nyeri gerak sudah berkurang, MMT (Kekuatan otot) meningkat, Range of Motion meningkat, Kemampuan fungsional pasien juga meningkat, serta massa otot pada kaki kiri yang bertambah. Nyeri gerak pada pertemuan kedua sudah berkurang menjadi nilai 1 (nyeri ringan) dari yang sebelumnya nilai 2 (nyeri ringan) dan untuk nyeri diam serta nyeri tekan tetap di nilai 0; MMT (Kekuatan otot) meningkat yaitu untuk gerakan eversi dari nilai 2 menjadi nilai 3, gerakan plantar fleksi dari nilai 3 menjadi nilai 4, gerakan inversi dari nilai 3 menjadi nilai 4 sedangkan untuk gerakan dorsi fleksi belum ada perubahan dari nilai 2 tetap di nilai 2; Range of Motion meningkat pada gerakan dorsi fleksi-plantar fleksi dan untuk gerakan eversi-inversi; Kemampuan fungsional pasien juga meningkat yaitu dari nilai 74 menjadi nilai 75; serta massa otot pada kaki kiri yang bertambah.Conclusion: Setelah di berikan intervensi fisioterapi 2 kali pertemuan terlihat perubahan yaitu nyeri gerak sudah berkurang, MMT (Kekuatan otot) meningkat, Range of Motion meningkat, Kemampuan fungsional pasien juga meningkat, serta massa otot pada kaki kiri yang bertambah.
