Abstract

Pendahuluan: Fraktur yang terjadi pada tulang paha bagian distal merupakan masalah yang serius.fraktur metafisis femoralis distal yang merupakan tempat bertemunya tulang kanselus kondilus femoral dengan tulang kortikal diafisis disebut sebagai fraktur supraconylar femoralis. Peningkatan patah tulang disekitar lutut terbukti dan diprediksi akan erus berlanjut. Fraktur femoralis distal (tepat diatas lutut) menyumbang antara 4-6% dari semua kasus patah tulang pada paha. Fraktur ini dapat menyebabkan kematian dengan angka kejadian yang tinggi (18-30% kematian dalam satu tahun. Presentasi kasus: Pasien seorang perempuan dengan usia 20 tahun yang tidak sedang bekerja,mengeluhkan adanya nyeri pada lutut sebelah kanan serta adanya keterbatasan gerak seperti menekuk lutut.. Manajemen dan hasil: Pasien melakukan program rehabilitasi yang terdiri dari pemberian TENS (Trancutaneous Electrical Nerve Stimulations), active exercise, hold relax dan isometric exercise selama 3 kali dalam 3 minggu. Diskusi:program rehabilitasi yang diberikan oleh fisioterapis kepada pasien yaitu TENS, active exercise, hold and relax exercise dan isometric exercise yang bertujuan untuk mengurani nyeri dan meningkatkan kekuatan otot. Kesimpulan: Pasien yang berjenis kelamin perempuan berusia 20 tahun dengan diagnosis Post Op Fraktur 1/3 distal Femur yang telah menjalani program rehabilitasi berupa pemberian intervensi TENS dan beberapa latihan dari fisioterapi sebanyak 3 kali pertemuan dalam 3 minggu menunjukkan adanya penurunan intensitas nyeri dan adanya penambahan lingkup gerak sendi.

Abstract

Introduction: Bell's palsy merupakan suatu kondisi yang melibatkan kelumpuhan saraf kranial ketujuh atau Facial Nerve. Insiden tahunan Bell's palsy adalah 15-30 kasus per 100.000 orang. Saraf wajah memiliki fungsi sensorik maupun motorik, gejala pada Bell's palsy dapat berupa defisit motorik dan sensorik seperti kelemahan otot wajah, tidak simetris wajah, kehilangan ekspresi wajah dan kemampuan fungsional seperti makan dan berbicara, serta kehilangan rasa pengecapan 2/3 anterior lidah. Case Presentation: Pasien wanita berusia 34 tahun dengan diagnosis medis Bell's palsy sinistra dengan keluhan wajah merot, tidak dapat menutup mata, tersenyum, mengerutkan dahi dan bersiul secara sempurna. Management and Outcome: Pemberian PNF dengan Teknik Rhythmic Initiation dan Repeated stretch sebanyak 4 kali dalam 2 minggu. Bertujuan untuk meningkatkan kekuatan otot, kemampuan fungsional dan inisiasi gerakan pada wajah. Evaluasi kekuatan otot menggunakan MMT dan kemampuan fungsional wajah menggunakan Ugo Fisch Discussion: Intervensi PNF dengan Teknik Rhythmic initiation dan Repeated stretch merangsang terjadinya kontraksi gerakan pada otot wajah serta otot wajah terfasilitasi ketika terjadinya gerakan. Teknik PNF wajah membantu menstimulasi otot-otot wajah yang melemah dengan prinsip iradiasi. Conclusion: Pemberian Teknik Proprioceptive Neuromuscular Facilitation efektif dalam meningkatkan kekuatan otot yang diukur menggunakan MMT wajah dan peningkatan kemampuan fungsional wajah diukur menggunakan Ugo Fisch.

Abstract

Introduction: Morbus Hansen atau biasa dikenal Kusta adalah penyakit menular kronis yang disebabkan oleh sejenis bakteri yang disebut Mycobacterium leprae dan menyerang kulit, saraf perifer, mukosa saluran pernapasan bagian atas, dan mata. Kusta dapat mengganggu kelembapan kulit yang menyebabkan xerosis. kusta biasanya dapat disembuhkan dan jarang menyebabkan kematian, tetapi dapat menyebabkan cacat. Salah satu masalah yang timbul akibat penyakit kusta ini adalah kelemahan atau lesi saraf peroneus, yang merupakan kelumpuhan otot anterior dan lateral pada kaki karena cedera atau kerusakan saraf peroneus. Ini juga dikenal sebagai istilah drop foot. Case Presentation: Penelitian ini dilakukan dengan meneliti seorang pasien jenis kelamin laki-laki usia 42 tahun yang didiagnosa Drop Foot Bilateral et causa Morbus Hansen Multi Basiler dengan Xerosis dan Ulcus. Dengan tujuan untuk mengetahui manfaat dari modalitas fisioterapi berupa Electrical stimulation, Stretching exercise, Terapi Latihan, dan Oiling. Management and Outcome: setelah dilakukan Tindakan fisioterapi sebanyak 3 kali pertemuan selama 3 dengan durasi 45 menit setiap pertemuan. didapatkan peningkatkan kekuatan otot pada dorsi fleksi, eversi dan inversi ankle dextra 1 poin, dan peningkatan lingkup gerak dextra dan sinistra sebanyak 5 derajat. Namun belum mengalami peningkatan pada kelembapan kulit dan belum mengalami perubahan pada kemampuan fungsional. Discussion: Belum adanya peningkatan yang signifikan dalam aspek-aspek pemeriksaan secara keseluruhan. Hal ini dikarenakan kerusakan pada saraf membutuhkan waktu yang lebih lama untuk sembuh dari pada otot. Karena fungsi saraf mengirimkan impuls listrik ke otot, ketika otot mengalami kerusakan, implus listrik yang dikirim ke otot akan terganggu dan berkurang, yang berarti otot tidak dapat melakukan fungsinya dengan baik. Oleh karena itu, sangat memungkinkan terjadinya peningkatan apabila waktu pertemuan untuk evaluasi diperpanjang. Total dari dilakukan terapi hingga evaluasi yaitu 3 kali pertemuan selama 2 minggu dengan durassi 45 menit setiap pertemuan. Conclusion: berdasarkan penelitian pada studi kasus ini menunjukkan bahwa modalitas fisioterapi pada kasus Drop Foot e.c morbus hansen multi basiler dengan xerosis dan ulcus berupa Electrical Stimulation, Stretching exercise, Terapi Latihan, dan Oiling dapat meningkatkan kekuatan otot dan peningkatan lingkup gerak sendi meskipun belum signifikan.

Abstract

Introduction: Lutut adalah sendi yang penting untuk menahan beban terutama pada bidang sagital, terutama pada posisi fleksi dan ekstensi. Disekitar lutut terdapat otot-otot penstabil dinamis sendi yang menjaga integritas lutut yang bergerak. Sedangkan otot-otot yang menjaga sendi saat istirahat adalah penstabil statis. Posterior Cruarite ligament adalah ligament utama pada sendi lutut yang berfungsi untuk menstabilkan tulang tibia pada tulang paha. PCL berasal dari anterolateral condilus femoralis medial area intercondylar notch dan posterior pada area tibialis. Fungsi dari PCL adalah untuk menahan Gerakan varus,valgus, dan rotasi eksternal pada lutut. Case Presentation: Pasien seorang laki-laki berusia 22 tahun dengan diagnosis Posterior cruriatum ligament knee sinistra grade 2 fase 4. Pasien merupakan seorang mahasiswa dan sedang di terapi konservatif di klinik Sport Injury Life Surakarta pada bulan januari 2024. Management and Outcome: Pemberian Terapi konservatif yang dilakukan pada PCL knee sinistra grade 2 fase 4 pada penelitian ini yaitu berupa kompres es, muscle release,hamset, quadset, static cycle,Calf raise/ single calf raise, plank/side plank,wall squat,lunges, trampoline,ABCD cone drill,jumps box, dan stretching. Latihan pada fase 4 di penelitian ini berfokus pada strengthening, proprioception, peningkatan fleksibilitas lower leg, dan peningkatan daya tahan ADL. Pemberian program terapi konservatif ini dilakukan 3 kali setiap minggu selama 2 minggu. Penelitian ini menggunakan kuisioner LEFS untuk mengevaluasi kemampuan fungsional dari ekstremitas bawah. Conclusion: Pada penelitian ini dapat disimpulkan bahwa pada kasus PCL Knee Sinistra fase 4 grade 2 yang diberikan intervensi fisioterapi selama 3x pertemuan menunjukkan efektif dalam peningkatan endurance selama latihan dan adanya peningkatan kemampuan fungsional sehari-hari. Untuk selanjutnya disarankan untuk kasus PCL (Posterior Cruciate Ligament) tidak hanya mengukur kekuatan otot pada kelompok otot Quadriceps dan Hamstring saja dan di penelitian selanjutnya dapat memodifikasi Latihan yang lebih kreatif dan inovatif daripada penelitian sebelumnya untuk menjadi perbandingan.

Abstract

Introduction: Attention-deficit hyperactivity disorder (ADHD) is a chronic condition that affects millions of children and often persists into adulthood. ADHD is one of the most common neurodevelopmental disorders with a worldwide prevalence rate of 5% among children and adolescents. The ratio of men to women is 2:1. The risk factors for ADHD include blood relatives with ADHD or other mental health disorders, exposure to rac un environment , the use of drugs , alcohol or smoking by the mother during pregnancy, maternal exposure to the toxic environment and also birth prematurely . Case Presentation: The method used in this study is a case report from a patient at the Mitra Insan Mandiri (MIM) Clinic in Ponorogo with the identity of An. ANP aged 3 years 5 months. Management and Outcame: Administration of neuro sensory (NS), head massage, expression massage, sensory integration and paravertebral release with myofascial release (MRF) technique showed a fairly good effect on the problems that exist in patients with ADHD. Discussion: Giving n euro senso used to download sensory stimulation and to train children to be quiet. Sensory integration (SI) therapy focuses on increasing the child's capacity to integrate sensory and input. SI therapy uses a neurophysiological approach to implementation and can reduce hyperactivity and attention problems. Head massage is useful for sensory stimulation, reducing children's emotions and providing a sense of comfort so as to make children calmer. M assage expressions may provide sensory stimulation to the face. Paravertebral release with the Myofascial Release (MFR) technique is given to reduce spasm in the paravertebral area. Conclusion: The provision of therapy programs in the form of neuro sensory, sensory integration, head massage , expression massage and paravertebral release using the MFR technique can have a fairly good effect after 4 therapy sessions in dealing with problems caused by patients with ADHD.

Abstract

Introduction : Coronary Artery Bypass Graft (CABG) is a surgical procedure to restore normal blood flow to the heart by bypassing blocked coronary arteries. The purpose of this study was to determine the effectiveness of giving Deep Breathing Exercise in cases of postoperative CABG (Coronary Artery Bypass Graft). Case Presentation: A 55 year old man with a dentist job. The patient complains of pain in the chest that was incised, pain occurs when sneezing or coughing. Management and Outcome : The patient is given deep breathing exercise. Do repetitions for 1 minute with a break of 2 seconds for each repetition, done 2 times a day. Pain was measured using the NPRS (Numeric Pain Rating Scale). Discussion : After being given therapy, it was found that there was a decrease in motion pain as much as 2 points which was measured using NRS where the values at T(beginning):5 and T(end) were:3. Conclusion : Giving therapy in the form of deep breathing exercise modality can reduce chest pain (incise scar) in patients after CABG (Coronary Artery Bypass Graft) surgery.

Abstract

Introduction: Secondary spontaneous pneumothorax (PSS) occurs due to underlying lung disease, in some developing countries pulmonary tuberculosis (TB) is the primary cause of PSS. Case Presentation: A 37 years-old male patient who was farmer from Wonogiri, Central Java, came to RS Paru Dungus with the symptoms of dyspnea, cough, and chest pain for 3 months ago. The patient is an active smoker for 25 years and quits smoking 3 months ago. The patient has a history of pulmonary TB disease. On examination of vital signs, blood pressure and heart rate were good, the patient's breathing was 28x/minute, oxygen saturation was 90%. A chest X-ray showed suggestive pleural TB with secondary infection and pneumothorax in the left apical segment. Based on the inspection, the patient using diaphragmatic breathing, has a barrel chest, and kyphosis. On palpation, there was a spasm of the accessory muscles of respiration. Auscultation showed normal breath sounds and hyper resonance in the left superior area. Vocal fremitus in left superior lobe > left inferior lobe. The dyspnea scale was 6 while the functional activity examination showed the patient was on a scale of 4. Management and Outcome: Physiotherapists give breathing control (BC), deep breathing exercise (DBE), and chest expansion resistance exercise (CERE) training programs for 9 days, getting the results of decreasing dyspnea level and increasing the patient's functional activity. Discussion: Pneumothorax secondary to TB usually occurs after extensive TB involvement of the lung as sudden bronchopleural fistulization and empyema with severe cavitary formations cause pleural rupture. BC is given to patients with a respiratory rate >20x/min, to help improve inefficient or abnormal breathing patterns, while DBE can increase ventilation and oxygenation, and CERE is given to maximize lung expansion by increasing the amount of air that can be pumped by the lungs to maintain the performance of the respiratory accessory muscles. Conclusion: Exercise therapy can reduce shortness of breath and improve functional activity in patients with secondary spontaneous pneumothorax associated pulmonary TB.

Abstract

Introduction: One of the ligaments in the knee is the anterior cruciate ligament where the function of the ligament itself is joint stabilization. 60% sports injuries occur in the knee, one of which is specifically a tear in the anterior cruciate ligament (ACL) ligament with the largest incidence rate of 16% during sports activities then the role of physiotherapy using close chain kinetic is expected to overcome pain, increase range of motion, reduce muscle atrophy and increase muscle strength. Case Presentation: A 23-year-old man last year had an accident while playing soccer and the patient felt the instability of his right knee when he was running, then after an MRI was done at the hospital 6 months ago the patient had ruptured anterior cruciate ligament ligament (ACL) so 3 months ago it was done post anterior cruciate ligament (ACL) reconstruction measures. Management and Outcomes: Giving therapy Exercises such as close chain exercise are effective for reducing pain, increasing muscle strength around the knee joint, reducing atrophy in the muscles around the knee joint as measured by the medline. Discussion: After doing physiotherapy rehabilitation for 6 times therapy, the pain reduction from the first therapy was worth 3 then the last therapy was 2 measured by NRS (numeric rating scale), then the range of motion increased by 5o from before exercise measured with a goneometer, for anthropometry it increased by 0.5 cm to 2 cm and muscle strength was stable at 5. Conclusion: After 2 weeks of therapy for 6 x sessions with intervention of close chain kinetic exercise therapy, the results were decreased pain, increased range of motion, increased muscle strength, reduced repeated falls, and reduced atrophy in the muscles that work on the knee joint.

Abstract

Pneumonia is caused by an infection that occurs in the air sacs caused by viruses, bacteria to fungi, attacking one or both lungs, which results in the alveoli being filled with fluid or pus, making it difficult for the patient to breathe. Pneumonia is the most common cause of death in the world with nearly 3.5 million deaths per year (Wunderink RG, Waterver GW., 2014). According to the incidence of infection with pneumonia, pneumonia is divided into two. Community pneumonia if the patient is infected from the community and nosocomial pneumonia if the patient is infected in the hospital. The goal to be achieved is to find out the benefits of providing physiotherapy interventions in the form of Chest Physiotherapy in reducing patient complaints in Pneumonia conditions at Dungus Lung Hospital. The results obtained after 3 times of therapy showed that shortness of breath decreased from a value of 5 (severe shortness of breath) to a value of 4 (somewhat severe).

Abstract

Introduction: Over the past 30 years, the incidence of cesarean section or sectio caesarea (SC) has increased. One of the complications of SC is postoperative pain. Pain can interfere with the patient's quality of life and reducing mother’s the functional status. One of the physiotherapy management that can be done is mobilization. Case Presentation: A 28-year-old pregnant woman came to Hospital in Madiun, East Java, Indonesia with complaints of ruptured membranes and breech presentation. The doctor performed a cesarean section (SC) operation on the patient. After SC the patient complained of pain at the incision site. Postoperatively the patient was in bed with an IV and catheter attached. The patient's vital signs are normal. Management and Outcome: The patient was mobilized for 2 days. This mobilization can be divided into 3 stages. The first stage (0-8 hours postoperatively) the patient sits on the edge of the bed, proceeds to get out of bed then sits in a chair and ambulates according to the patient's tolerance, the second stage (8-24 hours postoperatively) Ambulates according to the patient's tolerance, continues walking 1- 2 minutes (or more), third stage (24-48 hours postoperatively) Walk down the hall 3-4 times (or more), then get out of bed every 8 hours. Discussion: Mobilization has a good impact on patients after cesarean delivery. Ambulation helps reduce most of the complications by ensuring good blood circulation, improving breathing, and increasing physical strength. This is proven by an increase in the patient's Katz index, where the C score changes to A after mobilization Conclusion: Exercises given after cesarean delivery can increase transfer and ambulation activities from sleeping to sitting, sitting independently, sitting to standing, standing independently, and walking independently.
