Abstract

Percutaneous Coronary Intervention (PCI) is one of the most effective actions for patients with Acute Coronary Syndrome (ACS), which can improve prognosis and minimize hospitalization time. To optimize medical therapy programs, PCI must be complemented with further preventive measures, such as cardiovascular rehabilitation, including lifestyle modification and risk stratification. This study aims to analyze the relationship between phase I cardiovascular rehabilitation, risk stratification, and length of hospitalization in post-PCI patients. The study design was a retrospective cohort sourced from electronic medical record data. Respondents were divided into groups: those exposed and those not exposed to phase I cardiovascular rehabilitation. Risk stratification and length of hospitalization were studied as study outcomes. Among 201 patients who were followed up during hospitalization, high-risk stratification was more in the unexposed group: exposed group (92.2%: 90.8%; RR 0.85, 95%CI 0.340-2.104). Length of hospitalization ≥3 days was more in the unexposed group: exposed group (63.1%: 59.2%; RR 0.904 95%CI (0.639-1.279). Length of hospitalization ≥3 days was more in the high stratification group: low stratification (63.6%: 35.3%; RR 0.555 95%CI (0.289-1.066). Phase I cardiovascular rehabilitation was not associated with risk stratification and length of hospitalization. Patient risk stratification was related to the length of hospitalization of post-PCI patients. Phase I cardiovascular rehabilitation is an integral part of medical therapy that can reduce adverse events in post-PCI patients.

Abstract

Diabetes Mellitus (DM) is one of the global health problems that continues to increase in number from year to year. Diabetes self-management practice which is an important component in diabetes management, always involving the family in its implementation. This study aimed to explore family perspectives on diabetes delf-management practices. The research design used was a qualitative-descriptive phenomenology. Participants in this study were family members who cared for patients. A total participants of 10 people were recruited by purposive sampling at the Prof.dr. Chairuddin P Lubis Hospital Medan, North Sumatra, from March to Mei 2025. Data was collected through in-depth interviews with a semi-structured questions and analyzed using Colaizzi method. This study found 4 themes and 10 theme clusters related to diabetes self-management practices as follows: diet patterns (non-adherence to healthy diet, food preparation by the patient, and the patient sets his own dietary portions), physical exercise (not regularly doing physical exercise, independent physical exercise, reasons of not doing physical exercise), medications (non-adherence to medication and medication management by patients), and spirituality (patients routinely carry out religious activities and non-religious spiritual needs). According to the family's perspective, diabetic patients have not optimally implemented diabetes self-management practices in diet, medication, and exercise. Health workers are expected to provide support to patients and families so that diabetes self-management practices can run more optimally. Abstract

Type 2 diabetes mellitus (T2DM) is a group of metabolic diseases that occur when the body cannot effectively use the insulin it produces, resulting in hyperglycemia. T2DM mostly affects adults and is usually caused by age, overweight and inactivity. To manage diabetes appropriately or achieve adequate glycemic control, patients must perform diabetes self-care activities by organizing a proper diet, doing physical exercise, monitoring blood sugar levels, complying with prescribed medications, performing foot care, and quitting smoking. The purpose of this study was to determine the relationship of diabetes self-care activities with glycemic control in type 2 diabetes patients in the Padang Bulan Medan Health Center. This research method is quantitative non-experimental with correlation design. The majority of respondents carried out insufficient self-care activities (47.1%) with 79.3% uncontrolled glycemic control. The relationship between diabetes self-care activities and glycemic control was significant with p = 0.000 (<0.001) with a value of r = 0.643. Health center workers are expected to provide diabetes self-care education. Improving the implementation of diabetes self-care activities is aimed at preventing further complications and improving the quality of life of patients. Keywords: Diabetes.
