Abstract

Strok merupakan penyebab utama kematian dan kecacatan di dunia yang dapat diprediksi dan dicegah. Kegiatan ini bertujuan untuk memberikan edukasi dan meningkatkan pengetahuan masyarakat tentang risiko strok. Selain itu, kegiatan ini juga bertujuan untuk mengenalkan aplikasi berbasis web bernama Sinarsis untuk penilaian risiko strok secara mandiri. Pengabdi melakukan edukasi tentang mengenali dan menangani strok di kondisi pra-rumah sakit dan cara membawa pasien ke rumah sakit kepada masyarakat Desa Karangduren pada tanggal 14 Oktober 2023. Selain itu, juga dilakukan pemaparan tentang aplikasi berbasis web yang dapat membantu menilai risiko strok. Sebanyak 50 peserta mengikuti kegiatan ini. Pre-test dan post-test digunakan untuk menilai peningkatan pengetahuan peserta. Kuesioner digunakan untuk menilai persepsi peserta tentang kegiatan dan menilai penerimaan masyarakat terhadap aplikasi yang diperkenalkan. Aplikasi berbasis web yang disosialisasikan berdasar pada Stroke Risk Score Card yang dikeluarkan oleh National Stroke Association atau saat ini dikenal sebagai American Stroke Association. Terdapat peningkatan nilai post-test dibandingkan pre-test terhadap 14% peserta. Ditemukan bahwa 60% peserta sangat setuju dan 40% setuju bahwa kegiatan ini akan bermanfaat bagi masyarakat umum. Selain itu, seluruh peserta kegiatan telah mencoba menggunakan aplikasi Sinarsis menggunakan Web App. Sebanyak 94% peserta berencana menggunakan aplikasi ini dalam kegiatan promosi kesehatan masyarakat yang dilaksanakan secara rutin di Desa Karang Duren. Aplikasi Sinarsis ini dapat digunakan oleh masyarakat yang lebih luas di berbagai lokasi.

physical activity, and high-risk food consumption. This study aimed to analyze the relationship between these risk factors and stroke prevalence across Indonesia, using data from the Indonesian Health Survey covering 514 districts/cities in 38 provinces. The analysis focused on hypertension prevalence, physical activity levels, and dietary habits (particularly high salt intake). Spearman correlation results showed a significant association between hypertension and stroke prevalence (P < 0.001, r² = 0.451). In contrast, no significant association was found between low physical activity and stroke (P = 0.161, r² = 0.05), nor between high salt intake and stroke (P = 0.110, r² = 0.06). In conclusion, the study confirms that hypertension is significantly associated with an increased risk of stroke in Indonesia, emphasizing the need for interventions targeting hypertension to reduce stroke cases nationwide. While low physical activity and risky dietary habits did not show strong associations with stroke prevalence, these factors still warrant attention.

Abstract

Background: Neurocritical conditions such as traumatic brain injury, stroke, and brain tumor contribute significantly to mortality. Fluid balance is a key determinant of outcome, as overload can exacerbate brain edema and intracranial hypertension. However, diagnosis-specific evidence remains limited. Objective: To investigate the association between early fluid balance and in-hospital mortality across major neurocritical diagnoses through secondary analysis of a large critical care dataset. Methods: We conducted a secondary analysis using a publicly available dataset from Harvard Dataverse. Adult patients with neurocritical diagnoses were identified, and cumulative fluid balance within the first 72 hours was calculated. Patients were stratified by diagnosis and categorized into positive (>0 mL) or negative fluid balance groups. The primary outcome was in-hospital mortality. Results: A positive early fluid balance was likely correlated with higher mortality across the cohort even without having significant correlation (OR 0.205, 95% CI 0.031–1.301, p=0.093). The effect was strongest for SAH (OR 5.35, 95% CI 0.84–34.12, p=0.036) and for SAH, TBI, ICH, and brain tumor, the interaction terms all approached statistical significance (p=0.036-0.051), suggesting that more positive fluid balance might be associated with an increased risk of death. In contrast, infarct showed no such pattern (p = 0.256). Fluid type could not be distinguished in this dataset, but prior studies suggest composition may further modify outcomes. Conclusion: Early fluid balance shows diagnosis-specific prognostic value in neurocritical care, with fluid overload strongly linked to mortality in SAH, TBI, ICH and Brain tumor. Future studies should integrate fluid type alongside balance to refine individualized strategies.

Abstract

Background: Stroke is a leading cause of long-term disability in Indonesia, with rising incidence among individuals of productive age. Rural communities face multiple challenges, including low health literacy, high prevalence of risk factors, and persistent misconceptions about stroke prevention and post-stroke care. Objective: This study aimed to evaluate the effectiveness of a community-based intervention using the Life’s Essential 8 (LE8) framework and the SIGAP 8 pocket-book to strengthen family preparedness for primary and secondary stroke prevention in Besole Village, Tulungagung. Methods: A mixed-methods approach was applied in two phases: (1) pre-intervention assessment through demographic surveys, in-depth interviews, and FGDs with stakeholders to identify community needs; and (2) intervention through structured education, health screening, and pre–post knowledge testing. Participants underwent measurements of blood pressure, BMI, glucose, cholesterol, WHO stroke risk assessment, and Barthel Index evaluation for survivors. User experience of the SIGAP 8 booklet was assessed using the User Experience Questionnaire (UEQ). Results: A total of 34 stroke survivors and 21 non-stroke participants were included. Among survivors, 58.8 percent had stage 2 hypertension and 26.5 percent had glucose ≥200 mg/dL. Most demonstrated moderate dependence on the Barthel Index. Among non-stroke participants, 57.1 percent were classified as moderate risk. UEQ scores indicated positive user experience across dimensions, including dependability (3.82), attractiveness (3.69), and clarity (3.67). The LE8-based intervention effectively improved health literacy, motivation, and family capacity in monitoring stroke risk factors. Conclusion: The SIGAP 8 pocket-book was well-received and shows strong potential as a community-level tool for stroke prevention in rural areas.

Abstract

Background: Absence seizure is one of the most common forms of generalized epilepsy. Wearable devices have been proposed as useful tools to improve clinical care by providing continuous monitoring of patients. This study aimed to evaluate the use of wearable devices for detecting and monitoring absence epilepsy. Methods: This systematic review was performed using PRISMA standards and included cohort studies investigating wearable devices as monitoring tools for absence epilepsy. Literature searches were performed in PubMed, Science Direct, Cochrane, and Google Scholar. Results: Eleven cohort studies published between 2012 and 2025 from France, Turkey, Portugal, Netherlands, Denmark, Belgium, Georgia, USA, and Boston were included, with a total of 1,305 participants. All studies assessed wearable devices for detecting absence seizures and reported diagnostic performance using sensitivity, precision, and F1 score. The pooled descriptive synthesis showed a mean sensitivity of 0.897 (range 0.791–1.000; 11 studies), a mean precision of 0.831 (range 0.540–0.950; 7 studies), and a mean F1 score of 0.858 (range 0.740–0.9639; 7 studies). Only descriptive analysis was possible as most studies did not provide raw diagnostic data or confidence intervals. Conclusion: Wearable devices demonstrate promising performance in detecting absence seizures among patients with absence epilepsy, with high sensitivity, acceptable precision, and overall robust F1 scores across the included studies. However, the lack of standardized data reporting and confidence intervals limits the possibility of meta-analysis. Further large-scale prospective studies are required to validate these findings and support clinical implementation.
